Enhanced Ex Vivo Plasmodium vivax Intraerythrocytic Enrichment and Maturation for Rapid and Sensitive Parasite Growth Assays.

Plasmodium vivax chloroquine resistance has been documented in nearly every region where this malaria-causing parasite is endemic. Unfortunately, P. vivax resistance surveillance and drug discovery are challenging due to the low parasitemias of patient isolates and poor parasite survival through ex vivo maturation that reduce the sensitivity and scalability of current P. vivax antimalarial assays. Using cryopreserved patient isolates from Brazil and fresh patient isolates from India, we established a robust enrichment method for P. vivax parasites. We next performed a medium screen for formula

Antifungal mechanism of [RuIII(NH3)4catechol]+ complex on fluconazole-resistant Candida tropicalis.

Candidiasis, a major opportunistic mycosis caused by Candida sp., may comprise life-threatening systemic infections. The incidence of non-albicans species is rising, particularly in South America and they are frequently drug resistant, causing unresponsive cases. Thus, novel antimycotic agents are required. Here we tested the antifungal activity of [RuIII(NH3)4catechol]+ complex (RuCat), approaching possible action mechanisms on fluconazole-resistant Candida tropicalis. RuCat significantly (P < 0.05) inhibited the growth and viability of C. tropicalis dose-dependently (IC50 20.3 μM). Cytotoxic

The role of epidemiology in the regulation of oral contraceptives.

The U.S. Food and Drug Administration (FDA) has relied to a great degree on epidemiologic studies in the regulation of oral contraceptives (OC). These epidemiologic studies range from individual case reports of adverse reactions to case-control studies and cohort studies. Important findings about adverse reactions to OCs have been communicated through "labeling," which includes information leaflets provided as package inserts for physicians and patients. Also, the FDA communicates its position through publications in medical journals, the FDA Drug Bulletin, public advisory committee meetings,

Characterizing Active Pharmaceutical Ingredient Binding to Human Serum Albumin by Spin-Labeling and EPR Spectroscopy.

Drug binding to human serum albumin (HSA) has been characterized by a spin-labeling and continuous-wave (CW) EPR spectroscopic approach. Specifically, the contribution of functional groups (FGs) in a compound on its albumin-binding capabilities is quantitatively described. Molecules from different drug classes are labeled with EPR-active nitroxide radicals (spin-labeled pharmaceuticals (SLPs)) and in a screening approach CW-EPR spectroscopy is used to investigate HSA binding under physiological conditions and at varying ratios of SLP to protein. Spectral simulations of the CW-EPR spectra allow

Serious Adverse Events Associated with Off-Label Use of Azithromycin or Fentanyl in Children in Intensive Care Units: A Retrospective Chart Review.

OBJECTIVES: Half of prescription drugs commonly given to children lack product labeling on pediatric safety, efficacy, and dosing. Two drugs most widely used off-label in pediatrics are azithromycin and fentanyl. We sought to determine the risk of serious adverse events (SAEs) when oral azithromycin or intravenous/intramuscular fentanyl are used off-label compared to on-label in pediatric intensive care units (ICUs). STUDY DESIGN: Six pediatric hospitals participated in a retrospective chart review of patients administered oral azithromycin (n = 241) or intravenous/intramuscular fentanyl (n =

Triazolam--an "abused drug" by the lay press?

For the second time in the past ten years adverse reports in the lay press have questioned the safety of triazolam. Anecdotal reports of central nervous system adverse reactions can be separated into four general categories: (1) delirium in psychiatric patients; (2) delirium in geriatric patients; (3) withdrawal reactions; and (4) anterograde amnesia. Of the four, the amnesia reactions are regarded as serious enough by the Food and Drug Administration that it is requiring hypnotic drug manufacturers to revise the labeling of temazepam and flurazepam along with triazolam to emphasize the potent

Ontogeny and the Application of Pharmacogenomics to Pediatric Drug Development.

Drug product labels are important vehicles for conveying the scientific information needed for the safe and effective use of a medicinal product. Increasingly, pharmacogenomic (PGx) information is being incorporated into US Food and Drug Administration-approved product labels. In the majority of cases, the PGx information in labeling is derived from studies in adults. Observed genotype-phenotype relationships in adults may not always be reflective of those in certain pediatric age groups because of the influence of ontogeny. The quantitative data necessary for modeling certain genomic markers

[KEY POINTS FOR DRUG ERUPTIONS IN DAILY PRACTICE].

Drug eruptions are common mucocutaneous adverse reactions encountered across all medical specialties, occurring in approximately 0.1-1% of patients exposed to medications. However, accurate diagnosis and identification of the culprit drug remain challenging because of heterogeneous clinical presentations and a wide range of differential diagnoses, particularly viral exanthems. Comprehensive evaluation of medication history, infection status, latency period, and clinical phenotype is essential for appropriate narrowing of suspected agents. Among drug eruptions, Stevens-Johnson syndrome and toxi

Hydralazine-pyrimidine interactions may explain hydralazine-induced lupus erythematosus.

Hydralazine, the prototypic drug that induces systemic lupus erythematosus, reacts with thymidine and deoxycytidine. Analysis of a reaction mixture of therapeutic concentrations of hydralazine with labeled thymidine reveals at least four labeled products. At higher concentrations, hydralazine reacts with labeled deoxycytidine to form at least three labeled products. Formation of these products is markedly enhanced by exposure to ultraviolet light. The reaction of hydralazine with thymidine and deoxycytidine may be in part responsible for initiating drug-induced systemic lupus erythematosus.

Localization of enterobacterial common antigen immunoreactivity in the ribosomal cytoplasm of Escherichia coli cells cryosubstituted and embedded at low temperature.

The application of two on-section immunogold labeling techniques, the Lowicryl K4M (progressive lowering of temperature) procedure and the cryosection technique of Tokuyasu, in a previous work to study the topology of enterobacterial common antigen (ECA) biosynthesis revealed the presence of label on the outer membrane and in areas associated with the inner side of the cytoplasmic membrane. However, labeling was also observed in the ribosomal cytoplasm. The question of whether the cytoplasmic label was a result of ECA displacement during the more slowly acting aldehyde fixation or whether cyto

Leveraging COVID-19 vaccine allergy evaluations with coincident drug allergy delabelling: Effectiveness and impact on quality of life.

BACKGROUND: Concerns of potential drug/vaccine-associated allergies significantly impact vaccine safety and hesitancy. Delabelling of incorrect drug allergy in the general public was previously impeded by limited access to allergist services, especially among less frequent healthcare utilisers in the community. COVID-19 vaccine allergy evaluation services have enabled individuals (mis)labelled with allergies to receive vaccinations safely and have provided opportunities to expand drug allergy delabeling access. We investigated the effectiveness of this coincident drug allergy delabelling and i

Retrospective Study on the Pattern of Off-label Use of Misoprostol in Tabuk, Saudi Arabia.

INTRODUCTION: Off-label drug use (OLDU) refers to the prescription of a currently available and marketed medication for a use that has never been approved by the Food and Drug Administration (FDA). Misoprostol is one of the drugs which is used off-label. This drug, authorized for the treatment or prevention of peptic ulcers and other stomach disorders, is commonly used off-label for inducing labor or intrauterine device insertion. This research focuses on identifying the percentage of morbidity and mortality by off-label use of misoprostol; classifying the most common off-label misoprostol use

Global effects of kinase inhibitors on signaling networks revealed by quantitative phosphoproteomics.

Aberrant signaling causes many diseases, and manipulating signaling pathways with kinase inhibitors has emerged as a promising area of drug research. Most kinase inhibitors target the conserved ATP-binding pocket; therefore specificity is a major concern. Proteomics has previously been used to identify the direct targets of kinase inhibitors upon affinity purification from cellular extracts. Here we introduce a complementary approach to evaluate the effects of kinase inhibitors on the entire cell signaling network. We used triple labeling SILAC (stable isotope labeling by amino acids in cell c

Publicly visible regulatory timing from US Food and Drug Administration breakpoint updates to automated antimicrobial susceptibility test clearances: a longitudinal database study, 2017-2026.

OBJECTIVES: Updated antimicrobial susceptibility testing (AST) breakpoints require implementation in laboratory systems, but the timing of device updates across revisions has not been quantified. We measured publicly visible intervals from US Food and Drug Administration (FDA) breakpoint actions to clearances for established automated AST platforms. METHODS: Drug-specific FDA breakpoint actions from January 1, 2017, through August 23, 2026, were linked to 510(k) decision summaries for MicroScan, VITEK 2/VITEK 2 Compact, and BD Phoenix when the public record supported the drug, organism group,

Diagnostic challenges revealed from a neuropsychiatry movement disorders clinic.

BACKGROUND: Abnormal movements are frequently associated with psychiatric disorders. Optimized management and diagnosis of these movements depends on correct labeling. However, there is evidence of reduced accuracy in the labeling of these movements, which could result in sub-optimal care. OBJECTIVE: To determine the consensus inter-rater reliability between a movement disorders neurologist and physicians referring from the community for phenomenology and diagnoses of individuals with co-existing psychiatric conditions and movement disorders. METHOD: Charts of all consecutive patients seen i

Generation of a drug-induced renal injury list to facilitate the development of new approach methodologies for nephrotoxicity.

Drug-induced renal injury (DIRI) causes >1.5 million adverse events annually in the USA alone. Although standard biomarkers exist for DIRI, they lack the sensitivity or specificity to detect nephrotoxicity before the significant loss of renal function. In this study, we describe the creation of DIRIL - a list of drugs associated with DIRI and nephrotoxicity - from two literature datasets with DIRI annotation, confirmed using FDA drug labeling. DIRIL comprises 317 orally administered drugs covering all 14 anatomical, therapeutic and chemical (ATC) classification categories. Of the 317 drugs, 17…
