Development and growth of auricular cartilage and muscles: A study using human fetuses.

OBJECTIVES: The auricle is a key target in pediatric plastic surgery and is considered to develop from a ring- or funnel-like arrangement of six hillocks in the embryo. However, there has been no report showing the morphologies of the auricular muscle and cartilage after midterm in humans. METHODS: We examined histological sections of 20 near-term human fetuses (29-40 weeks) and those from 7 midterm fetuses (15-16 weeks). RESULTS: At midterm, the auricular cartilage was a single wavy plate with the helicis major muscle (HMM). The superior and posterior auricular muscles (SAM, PAM) were inser

Treatment of recurrent anal fistula using an autologous cartilage plug: a pilot study.

BACKGROUND: The aim of the present study was to assess a novel autologous cartilage plug technique used to treat anal fistula in ten patients. METHODS: All ten patients had undergone at least two prior operations for recurrent fistulas. The plugs were prepared using the patients' own cartilage, which was obtained from either the nose or the ear, diced into pieces, and wrapped with oxidized regenerated cellulose. During the same session, fistula tracts were curetted using cytology brushes, and then, the cartilage plug was inserted into the tract. Routine postoperative examinations were perform

[The lateral attic wall reconstruction with tragal cartilage and temporalis fascia graft].

OBJECTIVE: To investigate the reconstruction method of lateral attic wall with tragal cartilage and temporalis fascia graft. And analyze the postoperative result of its clinical application. METHOD: From Jan 2005 to Jul 2014, 45 patients whose middle ear disease were limited to attic received this surgery in our department. Among 31 cases of cholesteatoma otitis media and 14 cases of external auditory canal cholesteatoma were included. In order to expose the attic fully, we operated epitympanotomy through retroauricular incision and then removed the scutum and lateral attic bone wall. After e

Replantation of a totally amputated ear by means of a platysma musculocutaneous "sandwich" flap.

A total amputation of an ear from an automobile accident was salvaged by suturing the fragments of the skeletonized auricular cartilage and reattaching it to its original position "sandwiched" in a platysma musculocutaneous flap. The anterior portion of this flap was subsequently removed to apply a skin graft to the thin platysma muscle left over the convolutions of the auricular cartilage. This technique represents the application of the principle of bringing blood supply to avascular tissue in order to salvage it.

Suture technique for caudal septal deviations.

The "buckled" or severely deviated nasal septum has resisted a variety of techniques devised for its correction. This paper will present a new technique which the author has found useful in many of these cases. This method utilizes in reverse the procedure popularized by Mustardé for managing the protruding ear. Instead of bending the cartilage into a harmonious shape, the bent cartilage is straightened with submucosal permanent sutures. The paper will illustrate this technique.

Long-Term Comparison between Human Normal Conchal and Microtia Chondrocytes Regenerated by Tissue Engineering on Nanofiber Polyglycolic Acid Scaffolds.

BACKGROUND: Previous regeneration studies of auricle-shaped cartilage by tissue engineering leave unresolved whether the chondrocyte phenotype from human auricular chondrocytes seeded onto polymeric scaffolds is retained over the long term and whether microtia remnants may be a viable cell source for auricular reconstruction. METHODS: Chondrocytes were isolated from human ears, either normal conchal ear or microtia cartilage remnants, expanded in vitro, and seeded onto nanoscale-diameter polyglycolic acid sheets. These tissue-engineered constructs were implanted into athymic mice for up to 40

Prominent Ear Correction With Anterior Rasping.

PURPOSE: Prominent ear deformity is usually characterized by antihelical fold deficiency, concha hypertrophy and earlobe deformity. MATERIALS AND METHODS: Between 2000 and 2018, 375 otoplasty patients aged 6 to 58 who underwent surgery in our clinic were included in this study. Among these 375 cases, 25 were unilateral and 350 were bilateral. As surgical techniques anterior rasping, concha-mastoid sutures and concha-scaphoid sutures were used. The average cephaloauricular angle, the upper pole-mastoid distance and the distance between the middle 1/3 of the ear and mastoid was measured preoper

A Modification of the Posterior Perichondrio-Adipo-Dermal Flap for Protruding Ear Correction: A Customized Technique.

Prominent ear deformity is a common congenital deformity of the external ear affecting 5% of the general population. Because of the deformity's displeasing nature, corrective prominent ear surgery is widely common and performed as a prophylactic surgery. More than 200 surgical techniques for the correction of prominent ear deformity have been described to improve the cosmetic outcome and decrease complication rates. The aim of the study was to present a modified post auricular flap technique that increases the control of reshaping the ear cartilages, decreases the operation time, and provides …

Microtia Ear Reconstruction with Patient-Specific 3D Models-A Segmentation Protocol.

(1) Background: In recent years, three-dimensional (3D) templates have replaced traditional two-dimensional (2D) templates as visual guides during intra-operative carving of the autogenous cartilage framework in microtia reconstruction. This study aims to introduce a protocol of the fabrication of patient-specific, 3D printed and sterilizable auricular models for autogenous auricular reconstruction. (2) Methods: The patient's unaffected ear was captured with a high-resolution surface 3D scan (Artec Eva) and post-processed in order to obtain a clean surface model (STL format). In the next step,

Otoplasty: the case for skin incision by higher volume operators.

Prominent ear correction in the paediatric population is a common and popular procedure, with demonstrable benefit in alleviating psychological distress. Posterior cartilage-sparing techniques for otoplasty have been shown to be consistently safer and better than cartilage-scoring alternatives, however excision of the posterior auricular skin during otoplasty, although a common first step, has not been shown to have any benefits over skin incision alone. In this study, we examined the association between skin excision and recurrence of ear prominence, by examining the collective surgical outco

Regeneration of articular cartilage in healer and non-healer mice.

Mammals rarely regenerate their lost or injured tissues into adulthood. MRL/MpJ mouse strain initially identified to heal full-thickness ear wounds now represents a classical example of mammalian wound regeneration since it can heal a spectrum of injuries such as skin and cardiac wounds, nerve injuries and knee articular cartilage lesions. In addition to MRL/MpJ, a few other mouse strains such as LG/J (a parent of MRL/MpJ) and LGXSM-6 (arising from an intercross between LG/J and SM/J mouse strains) have now been recognized to possess regenerative/healing abilities for articular cartilage and e

Reconstruction of Upper Third Defect of the Auricle by Conchal Cartilage Graft and Coverage by Postauricular Flap.

BACKGROUND: The defects of the upper third of the auricle are considered significant reconstructive challenges, as they require frequent operations with a high risk of morbidity at the donor site and result in unacceptable cosmetic abnormalities. OBJECTIVE: Is to perform the reconstruction of a full-thickness auricular defect located in the upper third of the ear using a conchal cartilage graft with postauricular flap coverage, aiming to minimize both donor and recipient morbidity. PATIENTS AND METHODS: The current study included 20 patients with unilateral upper-third auricular defects. The

Reducing Surgical Risks for Otoplasty.

Prominauris is a common auricular deformity most often due to underdevelopment of the antihelical fold or overdevelopment of the conchal bowl. Significant psychosocial distress may result from prominent ears, leading to the development of a variety of surgical techniques over the years. A thorough understanding of the anatomy of both the normal and prominent ear is crucial for accurate analysis and surgical correction of the deformity. The procedure is well-tolerated and careful preoperative evaluation and adherence to meticulous cartilage-sparing techniques will lead to good results and low c

Chondrodermatitis nodularis helicis in an adolescent boy: not just for old men.

Chondrodermatitis nodularis helicis (CNH) is a solitary, chronic, painful, often ulcerated lesion on the upper portion of the ear, mostly in older men. We present an unusual case of CNH in a 17-year-old adolescent boy who was a lifeguard. Correlation between clinical and histopathologic characteristics of CNH is outlined in the context of the existing literature to increase awareness of this condition in younger patients as well as to guide appropriate management and surveillance.

Composite graft tympanoplasty in the treatment of ears with advanced middle ear pathology.

In ears with advanced pathology, the functional and anatomical results of surgery are compromised by such factors as total perforation, tympanosclerosis, atelectasis, suppuration, or previous surgery. Perichondrium cartilage composite grafts (PCCGs) were used for membrane grafting in 550 such cases over a period of 5 years. In all, 197 tympanoplastic procedures of type I (25% with previous surgery) and 353 of type III (47% with previous surgery) were carried out. The graft was obtained from the tragus and/or the concha. Two slightly different surgical techniques of grafting were used. Closure

[Polychondritis recidivans et atrophicans].

A typical case of chronic atrophic polychondritis with chondritis of the ear and episcleritis is presented. The inflammation subsided on treatment with colchicine. This entity is rare. It has an uncertain prognosis and causes a wide variety of symptoms. Accordingly, the spectrum of conditions that need to be considered in the differential diagnosis is also wide.
