Value and limitations of intracranial recordings for validating electric field modeling for transcranial brain stimulation.

Comparing electric field simulations from individualized head models against in-vivo intra-cranial recordings is considered the gold standard for direct validation of computational field modeling for transcranial brain stimulation and brain mapping techniques such as electro- and magnetoencephalography. The measurements also help to improve simulation accuracy by pinning down the factors having the largest influence on the simulations. Here we compare field simulations from four different automated pipelines against intracranial voltage recordings in an existing dataset of 14 epilepsy patients

MEG-guided analysis of 7T-MRI in patients with epilepsy.

PURPOSE: To study possible detection of structural abnormalities on 7T MRI that were not detected on 3T MRI and estimate the added value of MEG-guidance. For abnormalities found, analysis of convergence between clinical, MEG and 7T MRI localization of suspected epileptogenic foci. METHODS: In adult patients with well-documented localization-related epilepsy in whom a previous 3T MRI did not demonstrate an epileptogenic lesion but MEG indicated a plausible epileptogenic focus, 7T MRI was performed. Based on semiologic data, visual analysis of the 7T images was performed as well as based on pri

The relationship between morphological lesion, magnetic source imaging, and intracranial stereo-electroencephalography in focal cortical dysplasia.

Magnetoencephalography (MEG) is a useful non-invasive technique for presurgical evaluation of focal cortical dysplasia patients. We aimed at clarifying the precise spatial relationship between the spiking volume determined with MEG, the seizure onset zone and the lesional volume in patients with focal cortical dysplasia. We studied the spatial relationships between the MEG spiking volume determined with a recent analysis pipeline, the seizure-onset zone location determined with a quantitative index calculated from intracranial EEG signals ('Epileptogenicity Index') and the lesional volume deli

Long-term correlation of the electrocorticogram as a bioindicator of brain exposure to ionizing radiation.

Understanding the effects of radiation and its possible influence on the nervous system are of great clinical interest. However, there have been few electrophysiological studies on brain activity after exposure to ionizing radiation (IR). A new methodological approach regarding the assessment of the possible effects of IR on brain activity is the use of linear and nonlinear mathematical methods in the analysis of complex time series, such as brain oscillations measured using the electrocorticogram (ECoG). The objective of this study was to use linear and nonlinear mathematical methods as bioma

Detecting high-frequency oscillations in real time during epilepsy surgery with neuromorphic hardware validated to predict postoperative seizure outcome.

OBJECTIVE: High-frequency oscillations (HFOs; 250-500 Hz) in electrocorticography (ECoG) are promising biomarkers for delineating the epileptogenic zone during epilepsy surgery. Accurate, intraoperative real-time detection of HFOs may guide surgical resection to improve seizure outcome. METHODS: We used intraoperative ECoG from 22 patients from Zurich (Nicolet recording device, recording under Sevoflurane) with annotated HFOs to train an innovative automated HFO detector implemented in a spiking neural network (SNN) on neuromorphic hardware. We validated whether the detector could identify cl…

Real-time brain-computer interface control of walking exoskeleton with bilateral sensory feedback.

PURPOSE: Brain-computer interfaces (BCIs) offer a pathway to restore ambulation in individuals with spinal cord injury (SCI). However, existing BCI systems for gait are unidirectional and lack sensory feedback. This study aimed to demonstrate that a bidirectional brain-computer interface (BDBCI) can simultaneously enable real-time brain-controlled walking and artificial leg sensation via electrical stimulation of the sensory cortex. METHODS: Epilepsy patients undergoing bilateral interhemispheric subdural electrocorticography (ECoG) implantation were recruited for this proof-of-concept study.…

Intraoperative Electrocorticographic Monitoring for Predicting Post-Traumatic Seizures and Prognosticating Patients With Head Injury: A Prospective Observational Study.

BACKGROUND AND OBJECTIVES: Post-traumatic epilepsy is a major complication of traumatic brain injury (TBI), with limited predictive tools and preventive strategies. Although intraoperative electrocorticography (ECoG) is widely used in epilepsy and tumor surgeries, its role in brain injury remains underexplored. This study evaluates the predictive value of intraoperative ECoG for seizure development and functional outcomes in patients with TBI. METHODS: This prospective observational study included 110 patients with head injury who underwent surgery and intraoperative ECoG evaluation. The ECoG…

Dynamic changes in phase-amplitude coupling facilitate spatial attention control in fronto-parietal cortex.

Attention is a core cognitive mechanism that allows the brain to allocate limited resources depending on current task demands. A number of f … fronto-parietal regions control visuospatial attention on a fine spatiotemporal scale by recording electrocorticography (ECoG) signals measured directly from subdural electrode arrays that were implanted in patients undergoing intracranial monitoring for localization of epileptic foci. Subjects (n = 8) performed a spatial-cuing task, in which they allocated visuospat …

Network hubs supporting memory encoding: Implications for cognitive preservation in epilepsy surgery.

OBJECTIVE: Memory impairments are common in neurological disorders, especially drug-resistant epilepsy, where cognitive deficits such as accelerated long-term forgetting may occur before surgery due to interictal/ictal discharges, antiseizure medications, and disease progression. Postsurgical memory decline affects 40%-60% of patients. Understanding the neural mechanisms of memory encoding is essential for identifying critical network hubs and guiding surgical interventions that preserve cognitive function. METHODS: We analyzed intracranial electroencephalographic recordings from epilepsy pat

Abnormal Synchronization Between Cortical Delta Power and Ripples in Hippocampal Sclerosis.

OBJECTIVE: Discriminating between epileptogenic and physiological ripples in the hippocampus is important for identifying epileptogenic (EP) zones; however, distinguishing these ripples on the basis of their waveforms is difficult. We hypothesized that the nocturnal synchronization of hippocampal ripples and cortical delta power could be used to classify epileptogenic and physiological ripples in the hippocampus. METHODS: We enrolled 38 patients with electrodes implanted in the hippocampus or parahippocampal gyrus between April 2014 and March 2023 at our institution. We divided 11 patients (1

Factors correlated with intracranial interictal epileptiform discharges in refractory epilepsy.

OBJECTIVE: This study was undertaken to evaluate the influence that subject-specific factors have on intracranial interictal epileptiform discharge (IED) rates in persons with refractory epilepsy. METHODS: One hundred fifty subjects with intracranial electrodes performed multiple sessions of a free recall memory task; this standardized task controlled for subject attention levels. We utilized a dominance analysis to rank the importance of subject-specific factors based on their relative influence on IED rates. Linear mixed-effects models were employed to comprehensively examine factors with h

Aberrant Connectivity During Pilocarpine-Induced Status Epilepticus.

Status epilepticus (SE) is a common, life-threatening neurological disorder that may lead to permanent brain damage. In rodent models, SE is an acute phase of seizures that could be reproduced by injecting with pilocarpine and then induce chronic temporal lobe epilepsy (TLE) seizures. However, how SE disrupts brain activity, especially communications among brain regions, is still unclear. In this study, we aimed to identify the characteristic abnormalities of network connections among the frontal cortex, hippocampus and thalamus during the SE episodes in a pilocarpine model with functional and

Effect of tadalafil and nitric oxide agonist sodium nitroprusside on penicillin-induced epileptiform activity.

Objective: Comorbidity of erectile dysfunction (ED) and epilepsy is not rare. Tadalafil is widely used in the treatment of ED and shows its effect by increasing nitric oxide (NO) level. Previous studies demonstrated that ED treatment drugs increased epileptiform activity in clinical studies and various experimental epilepsy models. Therefore, it is important to know whether an ED treatment drug has proconvulsion or anticonvulsant properties. This study was designed to demonstrate the effect of tadalafil and NO agonist sodium nitroprusside during penicillin-induced seizures in rats. The experim

Simulation of spreading depolarization trajectories in cerebral cortex: Correlation of velocity and susceptibility in patients with aneurysmal subarachnoid hemorrhage.

In many cerebral grey matter structures including the neocortex, spreading depolarization (SD) is the principal mechanism of the near-complete breakdown of the transcellular ion gradients with abrupt water influx into neurons. Accordingly, SDs are abundantly recorded in patients with traumatic brain injury, spontaneous intracerebral hemorrhage, aneurysmal subarachnoid hemorrhage (aSAH) and malignant hemispheric stroke using subdural electrode strips. SD is observed as a large slow potential change, spreading in the cortex at velocities between 2 and 9 mm/min. Velocity and SD susceptibility typ

The 'when' and 'where' of semantic coding in the anterior temporal lobe: Temporal representational similarity analysis of electrocorticogram data.

Electrocorticograms (ECoG) provide a unique opportunity to monitor neural activity directly at the cortical surface. Ten patients with subdural electrodes covering ventral and lateral anterior temporal regions (ATL) performed a picture naming task. Temporal representational similarity analysis (RSA) was used, for the first time, to compare spatio-temporal neural patterns from the ATL surface with pre-defined theoretical models. The results indicate that the neural activity in the ventral subregion of the ATL codes semantic representations from 250 msec after picture onset. The observed activat

Delineation of somatosensory finger areas using vibrotactile stimulation, an ECoG study.

BACKGROUND: In surgical planning for epileptic focus resection, functional mapping of eloquent cortex is attained through direct electrical stimulation of the brain. This procedure is uncomfortable, can trigger seizures or nausea, and relies on subjective evaluation. We hypothesize that a method combining vibrotactile stimulation and statistical clustering may provide improved somatosensory mapping. METHODS: Seven pediatric candidates for surgical resection underwent a task in which their fingers were independently stimulated using a custom designed finger pad, during electrocorticographic mo

Patterns of Cortical Synchronization in Isolated Dystonia Compared With Parkinson Disease.

IMPORTANCE: Isolated dystonia and Parkinson disease (PD) are disorders of the basal gangliothalamocortical network. They have largely distinct clinical profiles, but both disorders respond to deep brain stimulation (DBS) in the same subcortical targets using similar stimulation paradigms, suggesting pathophysiologic overlap. We hypothesized that, similar to PD, isolated dystonia is associated with elevated cortical neuronal synchronization. OBJECTIVE: To investigate the electrophysiologic characteristics of the sensorimotor cortex arm-related area using a temporary subdural electrode strip in
