New method of using reconstructed images for assessment of patency of intracochlear spaces for cochlear implant candidates.

OBJECTIVE/HYPOTHESIS: A new method of reconstruction using multidetector-row helical computed tomography (MDCT) was applied for assessment of cochlear patency in cochlear implantation candidates. STUDY DESIGN: Descriptive study comparing radiologic and surgical findings in eight radiologically normal cochleas and four abnormal cases. METHODS: In unusual cases including cochlear ossification and misinsertion of electrodes, intracochlear space was evaluated on two types of reconstructed images: the "basal turn plane" that was vertical to the modiolus and contained whole basal turn was effectiv

Comparative analysis of explanted DBS electrodes.

BACKGROUND: Hardware-related complications frequently occur in deep brain stimulation. Microscopy and spectroscopy techniques are effective methods for characterizing the morphological and chemical basis of malfunctioning DBS electrodes. A previous report by our team revealed the morphological and chemical alterations on a malfunctioning explanted electrode when it was compared to a new device. The aim of this preliminary study was to verify whether these morphological and chemical alterations in the materials were a direct result of the hardware malfunctioning or if the failure was correlated

Successful revision of failed cochlear implants in severe labyrinthitis ossificans.

Labyrinthitis ossificans may complicate the insertion of a multichannel cochlear implant in patients deafened after meningitis. Two children who initially underwent partial insertion of a 22-channel cochlear implant because of severe cochlear ossification required revision surgery after several months of unsuccessful device use. At the time of revision, resection of the car canal, tympanic membrane, malleus, and incus provided access to the lateral wall of the cochlea, permitting extensive drilling of the basal turn and a circumodiolar placement of the electrode. Functional integrity of the el

Scala vestibuli cochlear implant supported by 3D modeling of the inner ear.

Patients with scala tympani (ST) ossification present a distinct surgical challenge. Three-dimensional (3D) segmentation of the inner ear offers accurate identification of ossification and surgical planning of the cochleostomy to access the scala vestibuli. The scala vestibuli placement of cochlear implantation electrode is an alternate solution in these patients and is well supported by the literature. The present report describes a case of cochlear implantation in the scala vestibuli assisted by 3D segmentation of the cochlea for a patient with ossification in the ST and reviews the relevant

Spinal cord stimulation electrode design: prospective, randomized, controlled trial comparing percutaneous and laminectomy electrodes-part I: technical outcomes.

OBJECTIVE: The clinical use of spinal cord stimulation for treatment of chronic intractable pain has been increasingly successful because of recent technical improvements, particularly the development of multiple-contact electrodes supported by programmable implanted pulse generators. Contemporary electrodes can be placed percutaneously in some cases and require a limited laminectomy in other cases. METHODS: We performed a prospective, randomized, controlled trial comparing two prototypical electrode designs, using a computerized system that allows direct patient interaction and quantitative

Clinical challenges and opportunities related to the biological responses experienced by indwelling and implantable bioelectronic medical devices.

Implantable electrodes have been utilized for decades to stimulate, sense, or monitor a broad range of biological processes, with examples ranging from glucose monitoring devices to cochlear implants. While the underlying science related to the application of electrodes is a mature field, preclinical and clinical studies have demonstrated that there are still significant challenges in vivo associated with a lack of control over tissue-material interfacial interactions, especially over longer time frames. Herein we discuss the current challenges and opportunities for implantable electrodes and

[Chronic left ventricular epicardial pacing by the perforated endocardial electrode through the cardiac vein].

We report a rare complication of endocardial pacing electrode implantation. A 64-year-old man, who was implanted transvenous pacemaker system by the other hospital one month ago, visited our outpatient clinic with the complaint of diaphragmatic twitching. Left ventricular pacing was highly suspected because of right bandle branch block pattern by 12 leads ECG. Pericardial effusion was observed by echocardiography, and angiography revealed the pacing electrode coursing through the coronary sinus and perforated the cardiac vein towards the posterior wall of the left ventricle. A new generator an

Recurrent Hippocampo-neocortical sleep-state divergence in humans.

Sleep is assumed to be a unitary, global state in humans and most other animals that is coordinated by executive centers in the brain stem, hypothalamus, and basal forebrain. However, the common observation of unihemispheric sleep in birds and marine mammals, as well as the recently discovered nonpathological regional sleep in rodents, calls into question whether the whole human brain might also typically exhibit different states between brain areas at the same time. We analyzed sleep states independently from simultaneously recorded hippocampal depth electrodes and cortical scalp electrodes i

Threshold for Tonic Motor Effects from Random Waveform in a Rat Experimental Model of Frontal Cortex Stimulation.

BACKGROUND: Brain stimulation is utilized to treat a variety of neurological disorders. Clinical brain stimulation technologies currently utilize charge-balanced pulse stimulation. The brain may better respond to other stimulation waveforms. This study was designed to evaluate the motor threshold of the brain to stimulation with various waveforms. METHODS: Three stimulation waveforms were utilized on rats with surgically implanted brain electrodes: pulses, square waves, and random waveform. The peak-to-peak stimulation voltage was increased in a step-wise manner until motor signs were elicite

Electrostimulation for intractable delayed emptying of intrathoracic stomach after esophagectomy.

PURPOSE: The use of the denervated intrathoracic stomach as esophageal substitute can rarely lead to severe delayed gastric emptying. We describe the use of electrostimulation for this condition. DESCRIPTION: Gastric electrical stimulation (GES) is used to treat medically refractory gastroparesis and uses a battery powered neurostimulator connected to the gastric antrum with two electrodes. We implant the electrodes through a right thoracotomy and tunnel them to the right subcostal area where the pacemaker is placed. EVALUATION: Medically refractory gastroparesis developed in 2 male patients

Toward a battery of behavioral and objective measures to achieve optimal cochlear implant stimulation levels in children.

OBJECTIVES: Children require audible and comfortable stimulation from their cochlear implants immediately after device activation. To accomplish this, a battery of objective measures may be needed that could include the electrically evoked stapedius reflex (ESR), compound action potential from the auditory nerve (ECAP), and/or auditory brain stem response (EABR). In the present study, the following specific research questions were asked: In children using cochlear implants, 1) Can the ECAP, EABR, and ESR be recorded at the time of cochlear implantation? 2) What is the feasibility of measuring

Reducing the foreign body response on human cochlear implants and their materials in vivo with photografted zwitterionic hydrogel coatings.

The foreign body response to implanted materials often complicates the functionality of sensitive biomedical devices. For cochlear implants, this response can reduce device performance, battery life and preservation of residual acoustic hearing. As a permanent and passive solution to the foreign body response, this work investigates ultra-low-fouling poly(carboxybetaine methacrylate) (pCBMA) thin film hydrogels that are simultaneously photo-grafted and photo-polymerized onto polydimethylsiloxane (PDMS). The cellular anti-fouling properties of these coatings are robustly maintained even after s

Electrode Type and Long-Term Clinical Response to Responsive Neurostimulation: A Single-Center Cohort With 5-Year Follow-up.

BACKGROUND AND OBJECTIVES: Responsive neurostimulation (RNS) is a neuromodulatory treatment of medically refractory epilepsy involving the implantation of either subdural or depth electrodes and a neurostimulator. Previous studies suggest improved outcomes associated with subdural electrodes, although this relationship is incompletely elucidated. We aimed to measure long-term response to RNS including the role of lead type and baseline patient characteristics. METHODS: We performed a retrospective chart review of 57 patients who underwent RNS implantation. Data collected included lead configu

Real-time glucose monitoring system containing enzymatic sensor and enzymatic reference electrodes.

Every electrochemical biosensor uses two or three electrode setup, which involves sensing electrode for a specific reaction, metal/salt reference electrode (i.e., Ag/AgCl or Hg/Hg2Cl2) for the control of the potential and, is some cases, counter electrode for the compensation of the current. This setup has significant flaws related to metal/salt reference electrodes: they are bulky and difficult to miniaturize, leak electrolyte to the medium, lose the ability to define the electrochemical potential precisely in time, consequently, have to be updated or replaced. This causes problems when the b

The role of EEG in epilepsy: a critical review.

Electroencephalography (EEG) is the most specific method to define epileptogenic cortex. Its sensitivity and specificity depend on several factors such as age and recording procedures, for example, sleep recordings and activation procedures (hyperventilation, photic stimulation). EEG reveals characteristic findings in several epilepsy syndromes. Rarely, epileptiform discharges are recorded in healthy, particularly young individuals. Ictal video/EEG recording is considered to be critical in localizing the epileptogenic zone. A careful analysis of the first clinical signs and symptoms of a seizu

Predicting most comfortable listening levels of ESRT from ECAP thresholds in paediatric cochlear implant users.

Studies have reported a varied correlation strength between the electrically evoked compound action potential (ECAP) and electrically evoked stapedial reflex thresholds (ESRT) in cochlear implant recipients. However, there is a lack of information on the relationship between the two measures in paediatric cochlear implant users. This study was aimed to compare the ESRT and ECAP measures and determine where ECAP thresholds fall within the dynamic range of ESRT-based Maps in paediatric cochlear implant users. The study involved 40 children aged between 3 and 6 years and were implanted with MedEL

Accidental Explantation of a Cochlear Implant in a Child Who Developed Cholesteatoma as a Late Complication of Cochlear Implantation.

INTRODUCTION: Although rare, cholesteatoma can develop as a late complication of cochlear implantation. The electrode array may then be exposed in the external auditory canal surrounded by cholesteatoma debris. Case Report. The cochlear implant of a child was inadvertently explanted by a clinician during a routine aural toilet procedure. The child had previously reported recurrent ear infections, pain, and unexplained implant function degradation. Reimplantation was carried out 2 days later with good postoperative hearing results. Part of the electrode array was observed to be embedded in chol

Polyimide cuff electrodes for peripheral nerve stimulation.

This paper describes a new tripolar spiral cuff electrode, composed of a thin (10 microm) and flexible polyimide insulating carrier and three circumneural platinum electrodes, suitable for stimulation of peripheral nerves. The cuffs were implanted around the sciatic nerve of two groups of ten rats each, one in which the polyimide ribbon was attached to a plastic connector to characterize the in vivo stimulating properties of the electrode, and one without a connector for testing possible mechanical nerve damage by means of functional and histological methods. The polyimide cuff electrodes indu
