OBJECTIVE: To demonstrate the utility of accurate clinical and electroencephalographic characterization of provoked cataplexy spells in the diagnosis of narcolepsy.
METHODS: Four individuals, three with suspected and one with known narcolepsy, were clinically assessed during split-screen, video polysomnographic monitoring sessions after cataplectic events were induced by emotional provocation.
RESULTS: The subjects experienced a total of nine cataplectic-like events, one occurring spontaneously (sleep paralysis) in association with a hypnagogic hallucination. During all events, the patients

The survey shows the frequency and distribution of diseases evaluated by electroophthalmological methods. Patients with retinal diseases (51.2%) and those with diseases of the optic nerve (21.8%) were examined most frequently. In a high percentage these investigations lead to a clinically useful assessment: described as confirmation or exclusion of a clinical diagnosis, as establishing a possible differential diagnosis or clearing up formerly unknown aspects of a disease. In cases of hereditary retinal disorders only 11% remained unclear, with presumed optic neuritis only 6%. The importance of

EEG power variations were evaluated in 9 healthy young adults from 8.45 a.m. to 9 p.m. and at 7 a.m. the next day. EEG signals were obtained from 16 electrodes in closed eyes and open eyes situations. Diurnal power variations were calculated for each frequency component, according to the recording situation (RS) and to the scalp site. Regarding values in the early morning on the first day, the power of almost all the frequency components showed an important diurnal increase. It came back close to initial values at 7 a.m. on the second day, which is in agreement with the existence of EEG circad

BACKGROUND: Alport syndrome refers to the clinical triad of hereditary nephritis, sensorineural deafness, and ocular abnormalities. Ultrastructural findings in the lens capsule and in the renal glomeruli have provided evidence that abnormal basement membranes are elaborated in affected tissues of patients with this disorder. Recently, the results of several linkage studies have allowed the genetic defect in Alport syndrome to be mapped to a locus that codes for a subtype of type IV collagen (alpha 5) known to be present in glomerular basement membranes. In spite of these advances, the nature o

Four male patients and one female patient of a new family with Joseph disease are reported. Their disease was characterized by autosomal dominant inheritance, bulging eyes, rigidity and spasticity of the lower extremities, dystonia, and bradykinesia. Cerebrospinal fluid homovanillic acid level was markedly reduced. Levodopa improved dystonia. Magnetic resonance imaging revealed mild atrophy of the frontal lobe and the cerebellum and marked atrophy of the lenticular nucleus and the brain stem. Polysomnographic studies revealed non-rapid eye movement stage central type sleep apnea syndrome. This

Since the non-invasive technque of retinal densitometry became available in 1955, rhodopsin kinetics could be studied in vivo. It was obvious that with this new tool investigators focussed attention on the aetiology of night blindness in various diseases. A brief review about the clinical developments in the past two decades is given. Also three case-reports are presented, which suggest that in some cases of congenital stationary night blindness (CSNB) the night blindness might arise from the absence of rhodopsin. This is contrary to the standing opinion and present problems regarding the inte

Electroretinography (ERG) and electro-oculography (EOG) were performed in 98 patients affected by alopecia areata (AA) free from ophthalmologic disorders, and in 40 healthy subjects, in order to evaluate whether or not the retinal pigment epithelium of AA patients shows bioelectrical changes. ERG was normal. EOG, on the contrary, showed a significantly depressed mean value in the AA patient group. Furthermore, depressed EOG occurred more frequently in patients affected by alopecia totalis or universalis as compared with multiple patchy alopecia. These findings prompt the authors to believe tha

Persons with diminished or absent vestibulo-ocular reflexes (VORs) are thought to have difficulty with ocular stabilization during sufficiently rapid head movements. This suggests the hypothesis that labyrinthine defective individuals might have impaired visual abilities when in a moving motor vehicle. Ten licensed drivers likely to have abnormal VORs were recruited to assess this possibility. Their labyrinthine function was indexed by measurements of ocular counterroll responses to static head tilt and the nystagmus induced with caloric stimulations. Each person was evaluated on a visual task

Fifteen patients with left ventricular heart failure (LVF) without known breathing disorders during sleep had full-night recordings of sleep and breathing to study the incidence and impact of the apnea of Cheyne-Stokes breathing. This group showed a marked degree of sleep-related breathing abnormalities, 40% demonstrating Cheyne-Stokes breathing with five or more central apneas per hour of sleep. Cheyne-Stokes breathing during sleep in patients with LVF predicted an increased short-term mortality rate. All six patients with LVF and Cheyne-Stokes breathing with more than five apneas per hour of

Various noninvasive test procedures were used to evaluate retinal function in a patient who had become night blind following vincristine chemotherapy. The results obtained were strikingly similar to those reported previously in subjects with recessively inherited stationary night blindness; the dark-adaptation curve was monophasic (ie, no evidence of a scotopic branch), rhodopsin kinetics were entirely normal, and spectral threshold data revealed the presence of residual rod-mediated vision. Also like the heritable condition, the b-wave of the ERG was depressed grossly despite normal a-wave po

1. Experiments were made on the cortex of the inferior parietal lobule in 10 hemispheres of six alert, behaving monkeys. The electrical signs of the impulse discharges of single cortical cells were recorded as the monkeys executed tasks requiring them to fixate stationary visual targets, track those which moved slowly, and to make saccadic movements to foveate those which suddenly jumped from one locus to another within the field of view. A total of 907 neurons of area 7 were identified in terms of their physiological properties, particularly the correlation of their activity with the oculomot

In two families with congenital amaurosis of Leber, keratoglobus was found in all affected members and posterior subcapsular cataracts in most of them. Consanguinity was present in one family. Pathologic findings in one enucleated eye were presented. The literature on this disease was briefly reviewed. Whether the disease is a definite clinical or genetic entity and whether it might be an agenesis or an abiotrophy, or both, were discussed.
