Fistulae in the membranous labyrinth.

Fistulization at various parts of the membranous labyrinth has been currently in use as one of the treatments for the symptoms of Meniere's disease. The present report concerns our experience with membranous fistulae in experimental animal ears with and without manifestation of endolymphatic hydrops. The materials presented are mostly new; however, our previous data are also incorporated in order to bring this issue into better perspective. The first part describes the nonproduction aspect of endolymphatic hydrops in the rat, chinchilla, and squirrel monkey after obliteration of the endolympha…

Evaluation of low-frequency biasing as a diagnostic tool in Menière patients.

Although it is generally accepted that endolymphatic hydrops is the cause of complaints in patients suffering from Menière's disease, it has not been possible up to now to prove the presence of an endolymphatic hydrops in living humans. This study evaluated the psychophysical method introduced by Mrowinski et al. [Mrowinski D., Gerull G., Nubel K., Scholz G., 1995. Masking and pitch shift of tone bursts and clicks by low-frequency tones. Hear. Res. 85, 95-102; Mrowinski D., Scholz G., Krompass S., Nubel K., 1996. Expression of caspase-activated deoxyribonuclease (CAD) and caspase 3 (CPP32) in the hydropic cochlea of guinea pigs - second report.

Apoptosis was induced in the cochlea by the injection of keyhole limpet hemocyanin (KLH) into the endolymphatic sac of guinea pigs and immunohistochemically examined. Keyhole limpet hemocyanin was injected into the right endolymphatic sac. The temporal bones were fixed via cardiac infusion of fixative and immunohistochemically stained for caspase-activated deoxyribonuclease or caspase 3. Endolymphatic hydrops became evident in the cochlea 1 day after the injection of keyhole limpet hemocyanin (n=6). The temporal bones in the control group did not show any caspase-activated deoxyribonuclease or…

Diagnosis of vestibular Menière's disease with electrocochleography.

The existence of vestibular Menière's disease, once considered a variant of classic Menière's disease, but without hearing loss, has been questioned because of lack of objective evidence that endolymphatic hydrops is involved with the disease process. Transtympanic electrocochleography (TT ECoG) has emerged as a useful tool for electrophysiologic monitoring of the inner ear, and is especially valuable in assessing endolymphatic hydrops. A retrospective chart review was performed to identify those patients with a diagnosis consistent with vestibular Menière's disease in order to determine the p…

The furosemide VOR test for Menière's disease. A preliminary report.

In order to easily detect endolymphatic hydrops and to avoid the side effects, we applied a furosemide VOR test in 24 patients with unilateral Meniere's disease, 6 patients with delayed endolymphatic hydrops, and 2 patients with labyrinthine syphilis. Immediately preceding an i.v. injection of 20 mg furosemide, and then 30, 60, 90 min after the administration, the VOR test was repeatedly performed. The slow-phase velocity in the VOR test was measured during harmonic sinusoidal rotation with eyes closed. Increased or decreased VOR-DP% beyond the normal range (10%) was referred to as positive. I…

Menière's Disease and Disorders of the Carbohydrate Metabolism Involving the Inner Ear.

Introduction  Menière's disease was described in 1861, but there are still uncertainties regarding its pathophysiology and treatment. Endolymphatic hydrops is recognized as a fundamental pathological characteristic of the disease, as a result of an inadequate absorption of the endolymph. A milder type of endolymphatic hydrops results from an altered chemical composition of the endolymph, due to disorders of the carbohydrate metabolism. Objective  To describe the association of both types of hydrops in patients with Menière disease. Methods  This was a retrospective study of 98 patients with Me…

Aquaporin-2 regulation by vasopressin in the rat inner ear.

Our previous studies have suggested a close relationship between vasopressin and endolymphatic hydrops, or the increased volume of endolymph in the inner ear. Endolymphatic hydrops is also thought to occur in Ménière's disease patients. In the kidney collecting duct, vasopressin induces the expression of aquaporin-2 (AQP2), resulting in increased water reabsorption. We explored the possibility, using a quantitative PCR method, that vasopressin regulates the expression of AQP2 mRNA in the rat inner ear, as it does in the kidney. The levels of AQP2 mRNA in the cochlea and endolymphatic sac were …

Magnetic resonance imaging of the inner ear in the diagnostics of Ménière's disease.

Ménière's disease is characterized by sudden episodes of vertigo accompanied by tinnitus and/or feeling of fullness in the ear as well as fluctuating sensorineural hearing loss. Despite numerous studies, the etiology of this disease remains unknown. However, the enlargement of the inner ear's endolymphatic spaces, referred to as endolymphatic hydrops, is considered the underlying condition. Thanks to recent advances in magnetic resonance (MR) technology, it is now possible to obtain in vivo imaging of endolymphatic hydrops in patients presenting with Ménière's disease symptoms. Visualization o…

Computing Endolymph Hydrodynamics During Head Impulse Test on Normal and Hydropic Vestibular Labyrinth Models.

Hypothesis: Build a biologic geometry based computational model to test the hypothesis that, in some circumstances, endolymphatic hydrops can mechanically cause enhanced eye velocity responses during clinical conditions of the head impulse test. Background: Some recent clinical and experimental findings had suggested that enhanced eye velocity responses measured with the video head impulse test could not only be caused by recording artifacts or central disfunction but also could be directly caused by the mechanical effect of endolymphatic hydrops on horizontal semicircular canal receptor. Meth…

Delayed contrast MRI for the evaluation of Ménière's disease: an Australian perspective.

OBJECTIVES: Ménière's disease is a chronic inner-ear disease attributed to endolymphatic hydrops. Magnetic resonance imaging with gadolinium allows visualisation of endolymphatic hydrops in vivo and may be an adjunct to diagnosis. METHODS: Thirty-eight patients suspected of having Ménière's disease underwent T2 weighted three-dimensional fluid-attenuated inversion recovery and true inversion recovery sequence magnetic resonance imaging 4 hours post double-dose intra-venous gadolinium. Presence of endolymphatic hydrops was graded by two radiologists at 0 and 4 months. Correlation to clinical d…

Cross-channel feature transfer 3D U-Net for automatic segmentation of the perilymph and endolymph fluid spaces in hydrops MRI.

The identification of endolymphatic hydrops (EH) using magnetic resonance imaging (MRI) is crucial for understanding inner ear disorders such as Meniere's disease and sudden low-frequency hearing loss. The EH ratio is calculated as the ratio of the endolymphatic fluid space to the perilymphatic fluid space. We propose a novel cross-channel feature transfer (CCFT) 3D U-Net for fully automated segmentation of the perilymphatic and endolymphatic fluid spaces in hydrops MRI. The model exhibits state-of-the-art performance in segmenting the endolymphatic fluid space by transferring magnetic resonan…

[Clinical studies on the furosemide VOR test].

In order to detect vestibular endolymphatic hydrops, the directional preponderance of the VOR gain (VOR-DP%) during sinusoidal rotation (freq. 0.1Hz; Amp. 120deg) was calculated and compared between pre- and post-intravenous injections of furosemide (20mg). Abnormal changes in VOR-DP% (> = 10%) from the pre-injection level were referred to as positive in this furosemide VOR test (FVOR). The following results were obtained from this study. (1) Positive responses were obtained in 31 of 61 (51%) patients with definite Meniere's disease, in 11 of 21 (52%) patients with delayed endolymphatic hydrop…

Streptomycin perfusion of the labyrinth.

The combination of fluctuant hearing loss, fullness, tinnitus and dizzy spells we call Meniere's disease is thought to be caused by endolymphatic hydrops. Most patients with the clinical picture of Meniere's disease do have endolymphatic hydrops but some patients with endolymphatic hydrops do not have the clinical picture of Meniere's disease. It would appear there is an, as yet unknown, immune-mediated, cause for Meniere's disease, in addition to endolymphatic hydrops, and this immune-mediated cause may aggravate those ears with endolymphatic hydrops. While medical treatment with a low-salt d…

Obliteration of the ductus reuniens.

The ductus reuniens was successfully obliterated in 52 guinea pig ears. Histopathological study showed that a majority of these specimens demonstrated cochlear hydrops, saccular collapse and normal utricle. These results support the theory of longitudinal flow of endolymph from the cochlea toward the endolymphatic sac via the ductus reuniens and saccule. A major source of endolymph in the saccule appears to be the scala media. In another set of 11 animals in which the ductus reuniens was first obstructed and two months later the endolymphatic duct was blocked, endolymphatic hydrops was shown i…

[Magnetic resonance imaging research progress of hydropic ear disease].

Hydropic ear disease is an otological disease caused by hydrops in the labyrinth of the inner ear membrane. According to the medical history and imaging classification system, it is mainly divided into two types: primary hydropic ear disease and secondary hydropic ear disease. The clinical manifestations vary depending on the anatomical localization of hydrops. Cochlear endolymphatic hydrops can lead to hearing loss, tinnitus, and ear fullness. Vestibular endolymphatic hydrops can cause dizziness, vertigo, nausea, vomiting, and gait instability. A number of studies on magnetic resonance imagin…
