OBJECTIVE: The study aimed to describe the auditory phenotype of patients with bilateral enlarged vestibular aqueduct, including benefits of … the ears. Sudden sensorineural hearing loss concerned 54% of patients. Both vestibular aqueduct and endolymphatic sac showed symmetric measurements (P < .001). Patients with SLC26A4 biallelic alterations (n = 22) had more associated thyroid disorders (14/22 vs 3/31, P < .001), earlier (2 ± 1.6 years old [yo] vs 5 ± 4.5 yo, P < .001) and lower age-adjusted pure-tone …

Fluctuant hearing loss, a very real and common cause of sensorineural hearing loss, is probably due to cochlear hydrops resulting from an anatomically inadequate endolymphatic sac, poor circulation, and one or more metabolic disorders. In the early stages at least, it often responds to treatment.

Our understanding of Meniere's disease is being revamped. For decades, the condition was thought to be caused by excessive fluid retention (hydrops) in the endolymphatic spaces of the inner ear, which led to tears or ruptures of the membranous structures that affect hearing and balance. More recently, research has shown that hydrops is not always associated with Meniere's and ought not to be considered the ultimate cause of its symptoms. New theories center on the fact that Meniere's disease may not have a single cause but may well be a common endpoint of a variety of anatomic or physiologic v

Endolymphatic-mastoid shunt operation due to Kitahara's method was performed on 24 patients with Meniere's disease. In 14 patients, the vertiginous attack was controlled with satisfactory preservation of hearing for longer than one year. In 2 patients, the vertiginous attack subsided but the hearing worsened. In the remaining 8 patients, long-term control of the vertiginous attack was not attained. Of these 8 recurrent cases, revision was performed on 4, revealing shunt obstruction with granulation or scar tissue. After reopening the endolymphatic sac, a large triangular Silastic sheet was pla…

The majority of patients with vertigo secondary to inner ear problems can be cured by surgery (80%). The results after neurectomy procedures such as cochleovestibular neurectomy or vestibular neurectomy appear to be much better (85%) than after the endolymphatic subarachnoid shunt procedure (70%). If hearing is worth saving, a conservative procedure should be used first in an effort to preserve the patient's hearing. If this fails, a cochleovestibular neurectomy will usually resolve the problem. At present we are using the retrolabyrinthine vestibular neurectomy as a primary procedure for Meni

PURPOSE OF REVIEW: Review of the role of aquaporins in inner ear homeostasis and potential role in the pathogenesis of Meniere's disease.
RECENT FINDINGS: Recent findings include the immunolocalization of aquaporins in the inner ear of mouse, rat, and human to cell types that are likely to undergo high ionic perturbances (e.g. potassium flux) and to putative areas of endolymph resorption or cycling.
SUMMARY: The expression of aquaporins and related proteins in the human cochlea and vestibular periphery resembles the distribution found in animal models, suggesting a critical role of aquaporin

In order to establish how endolymph volume is regulated, it is essential to be able to measure endolymph volume or cross-sectional area in v … site. In a previous study we quantified the degree of hydrops in animals following ablation of the endolymphatic sac. In the present study we validated the technique by comparing the endolymphatic cross-sectional area measured in vivo with AsF6 with that measured by established histologic procedures. The correlation between the two measures was good, with a coeffi …

This study was designed to investigate an immunologically induced endolymphatic hydrops (ELH) and to focus on the issue of its pathogenesis in relation to Menière's disease. The time course of ELH was evaluated by light microscopy in a 2-hour to 7-month period following direct antigen challenge to the endolymphatic sac (ELS) in systemically pre-sensitized guinea pigs. ELH began to appear in the vestibule and the basal turn 5-7 hours after inner ear challenge and developed gradually. During the interval from the second day to the first week, ELH rapidly developed in all the cochlear turns and r…

INTRODUCTION: Middle ear application of gentamicin is a common medical treatment for uncontrolled Ménière's disease. The objective of the study was to evaluate the impact of endolymphatic hydrops on inner ear delivery.
METHODS: Perilymph gentamicin concentrations and correlation with endolymphatic hydrops in an animal model were assessed. A group of 24 guinea pigs was submitted to surgical obstruction of the endolymphatic sac and duct of the right ear. Gentamicin was applied either to the right ear's round window niche or through a transtympanic injection. Perilymph specimens were collected a…

We aimed to determine the prevalence of radiological temporal bone features that in previous studies showed only a weak or an inconsistent association with the clinical diagnosis of Meniere's disease (MD), in two groups of MD patients (n = 71) with previously established distinct endolymphatic sac pathologies; i.e. the group MD-dg (ES degeneration) and the group MD-hp (ES hypoplasia). Delayed gadolinium-enhanced MRI and high-resolution CT data were used to determine and compare between and within (affected vs. non-affected side) groups geometric temporal bone features (lengths, widths, contour…

Gordon Smyth had a deep emotional investment in closed cavity surgery for cholesteatoma but, nonetheless, later acknowledged that he believed that he had been mistaken. Emotional investments create problems for all surgeons. Sometimes they have difficulty in recognizing that they need to change what they are doing. This is especially important in the management of Ménière's disease where unproven surgical procedures are often perpetuated. Surgery on the endolymphatic sac is of doubtful value but still continues to be the most frequently performed operation for this condition. Surgeons need to …

From review of recent findings, the pathology of Meniere's disease is described and correlated with clinical signs. Since Meniere's disease can be seen only in humans, assessing its natural history is important. A recent survey of 500 patients demonstrated the three major symptoms to be vestibular, auditory, and aural pressure. Meniere's disease (idiopathic) is distinguished from Meniere's syndrome (symptoms with likely cause), which accounted for approximately one fourth of the patients. Atypical forms include vestibular and cochlear Meniere's disease. A recent review of our temporal bone col

We here present a clinical study of 37 patients with Meniere's syndrome. Meniere's syndrome can occur subsequent to and in some cases simultaneously with chronic otitis media. When otitis media which has occurred many years earlier in childhood becomes inactive, leading to sequelae of Meniere's later in life, full-blown Meniere's symptom-complex with vertigo tends to occur; whereas when active chronic otitis media accompanies Meniere's, cochlear Meniere's syndrome tends to predominate. Endolymphatic hydrops is described in pathological cases of labyrinthitis and in 11 human temporal bone cases

Debate surrounding the need for evidence-based research in all fields of otolaryngology has recently emerged. A review of English language articles dealing with the treatment of Menière's disease published from 1989 to 1999 was performed. The papers were classified according to article type (observational studies, non-clinical studies, controlled trials, randomised controlled trials) and according to the grade of evidence that was offered. A total of 128 publications were reviewed, of which only three were randomised controlled trials and a further three were well-designed controlled studies w

Obliteration of the right endolymphatic sac was performed by Kimura's method in 57 guinea pigs with normal hearing and vestibular function. However, 43 animals exhibited postoperative cerebellopontile disturbance and labyrinthitis detected by gait test, ABR test and histological examination. The remaining 14 guinea pigs were evaluated periodically by the trapezoid rotation test. Two weeks after the operation, 11 of these 14 guinea pigs exhibited prolonged right beating nystagmus based on the labyrinthine preponderance of impaired side (Lpi). In addition, endolymphatic hydrops was histological …
