Case report: Successful anterior temporal lobectomy in drug-resistant temporal lobe epilepsy associated with Sotos syndrome.

The Sotos syndrome is an autosomal dominant disorder characterized by haploinsufficiency of NSD1 gene, with some individuals affected by epilepsy and, rarely, drug-resistant seizures. A 47-years-old female patient with Sotos syndrome was diagnosed with focal-onset seizures in left temporal lobe, left-side hippocampal atrophy, and neuropsychological testing with decreased performance in several cognitive domains. Patient was treated with left-side temporal lobe resection and developed complete awake seizure control in 3-years of follow-up, with marked improvement in quality-of-life. In selected

Temporal relationship between levetiracetam nonadherence and breakthrough seizures in a preclinical model of temporal lobe epilepsy.

OBJECTIVE: Poor medication adherence remains a concern for individuals managing their epilepsy with antiseizure medicines (ASMs); however, ethical concerns around withholding medication make it impossible to study the causal relationship between missed doses and seizures in patients. Previous preclinical studies from our group suggest that mechanistically distinct ASMs have varying degrees of forgiveness when a dose is missed. However, with only a few ASMs studied in the context of nonadherence, we sought to expand on previous work to understand the relationship between levetiracetam (LEV) non

Functional connectivity between mesial temporal and default mode structures may help lateralize surgical temporal lobe epilepsy.

OBJECTIVE: The most common surgically treatable epilepsy syndrome is mesial temporal lobe epilepsy (mTLE). Preoperative noninvasive lateralization of mTLE is challenging in part due to rapid contralateral seizure spread. Abnormal connections in both the mesial temporal lobe and resting-state networks have been described in mTLE, but it is unclear if connectivity between these networks may aid in lateralization. METHODS: In 52 patients with left mTLE (LmTLE) or right mTLE (RmTLE) and 52 matched control subjects, the authors acquired 20 minutes of resting-state functional MRI (fMRI) and evaluat

Facial memory ability and self-awareness in patients with temporal lobe epilepsy after anterior temporal lobectomy.

Anterior temporal lobectomy (ATL) is the most common surgical treatment for drug-resistant temporal lobe epilepsy (TLE). Right ATL has been reported to reduce facial memory ability in patients with TLE, as indicated by poor performance on the Warrington Recognition Memory Test for Faces (RMF), which is commonly used to evaluate visual memory in these patients. However, little is known about whether patients with TLE exhibit difficulties in identifying faces in daily life after ATL. The aim of this study was to investigate facial memory ability and self-awareness of face identification difficul

Late-onset temporal lobe epilepsy with unilateral mesial temporal sclerosis and cognitive decline: a diagnostic dilemma.

We present a patient with new onset temporal lobe epilepsy and cognitive decline in his sixth decade with unilateral hippocampal atrophy on structural brain imaging, compatible with mesial temporal sclerosis. This unusual clinical scenario presented a challenging differential diagnosis since it may overlap with primary cognitive disorders, including early-onset Alzheimer's disease and some forms of frontotemporal dementia, and the recently elucidated syndrome of non-paraneoplastic limbic encephalitis associated with voltage-gated potassium channel antibodies.

Multilayer Validation Reveals a Glia-Associated Secretome Signature in Temporal Lobe Epilepsy.

Although multi-omics studies have increasingly revealed molecular alterations associated with epilepsy, clinically accessible cerebrospinal fluid (CSF) biomarkers that are stably detectable and biologically relevant remain scarce. This study aimed to identify measurable biomarkers associated with epilepsy-related molecular alterations. Kainic acid (KA)-induced mouse model, single-cell RNA sequencing, and public transcriptomic datasets identified secreted protein-coding genes associated with inflammation, glial activation, and metabolic dysregulation. Candidate biomarkers were validated across

Corpora amylacea and glymphatic system in temporal lobe epilepsy patients: Clinicopathological correlation.

OBJECTIVE: Corpora amylacea (CA), now called wasteosomes, are basophilic inclusions associated with aging, neurodegeneration, and impaired glymphatic waste clearance. Their presence is well described in temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) and related to hippocampal neuronal loss and longer epilepsy duration. Here, we assessed CA deposition in TLE patients submitted to epilepsy surgery with any histological diagnosis and explored its relationship with clinical and neuropsychological variables. Histological signs of the glymphatic system were evaluated. METHODS: Qualitati

Kainic acid model of temporal lobe epilepsy: Nanoscale oscillations of hippocampal mitochondria.

Mitochondrial dysfunction is increasingly recognized as a central component of epileptogenesis, yet sensitive biophysical markers capable of capturing early mitochondrial alterations remain limited. Kainic acid (KA) model of temporal lobe epilepsy (TLE) is one of the most reliable tools for understanding the mechanisms of epileptogenesis and induces behavioral, electro-physiological, chemical and histological effects that are similar to those observed in patients with TLE. In the present study, we evaluate the nanoscale oscillatory behavior of mitochondria isolated from primary hippocampal neu

EEG signature orchestrating expression of ictal behavior in mesial temporal lobe epilepsy.

OBJECTIVES: We investigated EEG features differentiating clinical seizures (CSs) from subclinical seizures (SCSs) to explore the mechanisms underlying the generation of ictal behavior in mesial temporal lobe epilepsy (mTLE). METHODS: Peri-ictal state of power spectral density (PSD) within seizure onset zone (SOZ) and propagation zone (PZ) were compared between SCSs and CSs. Functional connectivity was analyzed using the nonlinear correlation coefficient h2, outgoing links (OUT) and ingoing links (IN). The EEG epochs of CSs-early part and SCSs were equally divided into four segments to reveal

Amygdalar volume asymmetry informs laterality in temporal lobe epilepsy: MRI-SEEG study.

OBJECTIVE: Amygdalar volumes are right-left asymmetric in normal humans. Asymmetric amygdalar hyperplasia is described in temporal lobe epilepsy (TLE), but has unclear lateralizing significance. In this study of TLE patients undergoing stereo-electroencephalography (SEEG) we examined the lateralizing value of amygdalar volume (AV) asymmetry, and its relationship to amygdalar involvement in seizures. METHODS: Amygdalar volumes of 30 TLE patients without radiological hippocampal sclerosis undergoing SEEG were compared to those from a normative database. Devising a novel amygdalar (volume) asymm

Cardiac structural and molecular alterations in rodent models of temporal lobe epilepsy.

OBJECTIVE: Cardiac structural and molecular changes are prevalent in people with chronic epilepsy, possibly contributing to an increased risk of premature mortality. However, understanding of the underlying pathophysiological mechanisms is limited. Here, we investigated the subacute and chronic changes in cardiac structure and ion channel/exchanger expression in different rodent models of temporal lobe epilepsy (TLE). METHODS: Two models of TLE were used: the kainic acid-induced post-status epilepticus (KASE) model in Wistar rats and the electrical self-sustained status epilepticus (SSSE) mod

Clinical daily rhythms of seizure in different subtypes of temporal lobe epilepsy.

INTRODUCTION: The relationship between circadian rhythms and seizures in temporal lobe epilepsy (TLE) has been well recognized, but it remains poorly understood how the interaction between the endogenous clock system and seizures may affect seizure patterns and patient management. This study investigates the circadian rhythm patterns of clinical seizures in TLE, with a focus on different subtypes and clinical variables. METHODS: We retrospectively analyzed the seizure rhythms of patients diagnosed with TLE who were admitted to the video-EEG ward. Patients were categorized based on clinical da

Transcriptomic profiling unveils novel therapeutic options for drug-resistant temporal lobe epilepsy.

BACKGROUND: Epilepsy drug treatments fail in 25-30 % of patients, leading to drug resistance. Temporal lobe epilepsy is the most prevalent subtype associated with drug resistance. Classical drug discovery is a long and extremely costly process with a high failure rate in clinical trials. Drug repurposing is a more cost- and time-effective strategy. Hence, the main objective of this study is to propose drug candidates for the treatment of drug-resistant temporal lobe epilepsy (DR-TLE) through drug repurposing based on transcriptomic profiling. METHODS: Total RNA-sequencing (RNA-Seq) was perfor

Elevated Apolipoprotein E Expression in Hippocampal Microglia Drives Temporal Lobe Epilepsy Progression.

Temporal lobe epilepsy (TLE), the most common form of epilepsy, is primarily characterized by hippocampal sclerosis (HS). Microglia reactivity is a critical component of TLE pathogenesis, and apolipoprotein E (APOE) may be a potential mediator of these processes. However, its role in TLE progression remains unclear. Bioinformatics approaches with biomarker validation are integrated to elucidate APOE's role and hippocampal microglia in the mechanisms underlying TLE. APOE expression is significantly elevated in the hippocampal tissues of patients with TLE-HS and in TLE mouse models. Single-cell
