Impact of Artificial Intelligence on the Care of Terminally Ill Patients.

Introduction: In recent decades, demographic aging has led to an inversion of the population pyramid, with a marked increase in the proportion of older adults. This shift has been accompanied by a higher prevalence of chronic and life-limiting diseases, while there have also been significant technical and scientific advances. However, these developments have not been matched by a proportional expansion of healthcare human resources, including in palliative care (PC). Consequently, healthcare systems face increasing pressure, particularly in the provision of end-of-life care. Artificial intelli

Optimizing the doses of cancer drugs after usual dose finding.

Since the middle of the 20th century, oncology's dose-finding paradigm has been oriented toward identifying a drug's maximum tolerated dose, which is then carried forward into phase 2 and 3 trials and clinical practice. For most modern precision medicines, however, maximum tolerated dose is far greater than the minimum dose needed to achieve maximal benefit, leading to unnecessary side effects. Regulatory change may decrease maximum tolerated dose's predominance by enforcing dose optimization of new drugs. Dozens of already approved cancer drugs require re-evaluation, however, introducing a ne

To Do or Not to Do: Decompressive Craniectomy for Severe Traumatic Brain Injury.

Globally, severe traumatic brain injury (TBI) is a significant cause of death and disability, particularly among young adults in their productive years. The management of elevated intracranial pressure (ICP) following TBI remains one of the greatest challenges in neurotrauma care, with decompressive craniectomy (DC) being a prominent, albeit contentious, treatment option. DC, a surgical procedure that involves removing a portion of the skull to accommodate brain swelling, has emerged as a potential life-saving intervention in such scenarios. The rationale is that by reducing ICP and enhancing

Comparison of brace to observation in stable, radiological developmental dysplasia of the hip: a protocol for a global multicentre non-inferiority randomised trial.

INTRODUCTION: Brace treatment is common to address radiological dysplasia in infants with developmental dysplasia of the hip (DDH); however, it is unclear whether bracing provides significant benefit above careful observation by ultrasound. If observation alone is non-inferior to bracing for radiological dysplasia, unnecessary treatment may be avoided. Therefore, the purpose of this study is to determine whether observation is non-inferior to bracing for infants with radiological dysplasia. METHODS AND ANALYSIS: This will be a multicentre, global, randomised, non-inferiority trial performed u
