Fractional excretion of filtered sodium (FENa) is typically elevated (greater than 3%) in acute tubular necrosis and decreased (less than 1%) with volume depletion. We describe a patient who developed acute tubular necrosis with a FENa of 13%. Four weeks later, he was still oliguric but had also become volume-depleted. FENa was 0.4% to 0.8%, considerably lower than FENa's measured in eight nonvolume-depleted patients in acute renal failure. Vigorous intravenous fluid therapy in this patient produced a prompt increase in urine volume and improvement in renal function. We conclude the following:

An isotonic fluid load was given to rats by infusing 12 ml saline i.v. in 60 min. The plasma water and extracellular fluid volumes of the whole animal and selected tissues were subsequently studied with 125I human serum albumin and 51Cr EDTA. Fourteen thermally injured patients with severe inhalation injury were sequentially studied with the thermal-green dye double indicator dilution technique of extravascular lung water (EVLW) measurement. Eight females and six males (average age, 49 years, and average thermal burn, 37% body surface) were studied for 2-31 days postinjury. All were burned in a closed space, had facial burns, soot in their sputum, and a mean carboxyhemoglobin level of 30%. Nine patients died, six of sepsis, one each of acute renal failure, hepatorenal syndrome, and anoxic brain damage. Mean EVLW on admission was 7.

Fluid retention and ascites are rarely seen in patients with primary biliary cirrhosis (PBC). Total body water (T.B.W), extra-cellular fluid volume (E.C.F.), exchangeable sodium (NaE) and potassium (KE) were measured in one typical case of Bartter's syndrome with growth retardation. Intracellular potassium depletion was associated with a slight impaired carbohydrate tolerance and with decreased growth hormone response to arginine infusion and insulin-induced hypoglycemia. These observations demonstrate that impairement of growth hormone responses to stimulation occur in Bartter's syndrome with potassium depletion.

An ultrastructural study of 2 patients with Netherton's syndrome (bamboo hair, scaling dermatosis, and an atopic diathesis) showed features of psoriasis and a dermatitis. Although the biopsies from the patients showed histological and ultrastructural similarities, the clinical presentations were different. One patient had ichthyosis linearis circumflexa, while the other had a generalized ichthyosiform eruption. The ultrastructural findings in the 2 patients, while not specific, may help distinguish the eruption of Netherton's syndrome from other scaling dermatoses.

Changes in transthoracic electrical impedance (T.E.I.) due to high-altitude hypoxia (3,658 m) have been measured in 20 young, healthy Indian soldiers. They were first studied at sea level (198 m) and then rapidly transported by air to 3,658 m, where they were studied daily from day 1 to day 5 and then on days 8 and 10. The mean (+/-S.D.) T.E.I. at sea level (34.6+/-0.6Omega) fell sharply to 29.6+/-0.8Omega, 30.3+/-0.9Omega, and 30.5+/-1.1Omega on days 1, 2, and 3 (P <0.001) and levelled off at 31.5+/-0.7Omega on day 10, which was comparable to the mean value obtained in 13 persons permanently
