We provide the first report of rhinofacial conidiobolomycosis caused by Conidiobolus coronatus in China. The patient was a 36-year-old male rice farmer from Jiang Xi province who presented with diffuse swelling with classical centrofacial distribution and involvement of the right paranasal sinuses. Histopathology showed granulomas with hyphae surrounded by an eosinophilic sheath (Splendore-Hoeppli phenomenon). Culture of biopsy specimens on Sabouraud's dextrose agar led to the growth of fungi with microscopically visible conidiophores and terminal spherical conidia (primary conidium), with mul

The authors examined 23 precipitin-positive symptomatic patients with Farmer's Lung(FL) and compared them to different groups of exposed asymptomatic precipitin-positive(EAPP) and precipitin- negative(EAPN) farmers. The sera were tested using several techniques (i.e., immunodiffusion and ELISA for specific antibodies; polyethylene glycol [PEG] for circulating immune complexes [CIC]) in an attempt to find an in vitro test correlated with the disease which could also provide an insight into the pathogenic mechanisms of Farmer's Lung. Circulating immune complexes formed by IgG were significantly …

A detailed comparative seroepidemiological study of antibody responses was performed in 271 pigeon fanciers and 100 farmers. Overall 73% of pigeon fanciers had IgG antibodies at a titre greater than or equal to 16 to Chlamydia pneumoniae, 39% to Chlamydia psittaci, and 6.6% to Chlamydia trachomatis. The prevalence of chlamydial antibodies was significantly lower in the farmers at 47% for C. pneumoniae, 6% for C. psittaci, and 2% for C. trachomatis. Both populations were exposed to complex microbiological and antigenic environments: 50.5% of the pigeon fanciers had antibodies to pigeon antigens…

Two cases of farmer's lung disease in siblings are reported. A 54-year-old male farmer, who had been engaged in stock work for 20 years, presented to our clinic for the second episode of fever, productive cough and shortness of breath. Chest roentgenogram revealed diffuse micronodular pattern, and mild hypoxemia was recognized on arterial blood gas analysis. Cytology obtained from BALF showed lymphocytosis, with especially increased OKT3, OKT4 positive cells and OKT4/8 ratio. The diagnosis was confirmed by highly positive reaction of precipitins to Thermoactinomyces vulgaris and granulomatous

IgG subclass reactivities of patients suffering from farmer's lung, healthy family members and blood donors were tested for specific binding to Micropolyspora faeni extract by means of immunoblot techniques (IEF-Print, Western Blot). In patients' sera IgG1 and IgG2 had shown the strongest reactivities while IgG3- and IgG4-reactivities were not found in all cases. Strong IgG2-reactivities against acidic proteins (pl 3-5) of M. faeni seem to distinguish between patients and exposed or not exposed controls, respectively.

The prevalence of serum-precipitating antibodies to Micropolyspora faeni and Thermoactinomyces vulgaris was studied by immunoelectrophoresis in 442 dairy farmers living in Hokkaido, the northernmost district of Japan. The prevalence rates of antibodies to M. faeni and T. vulgaris were 24.2 and 11.6%, respectively. The rate of antibodies to M. faeni was higher among females than males. This difference may be due to the large number of nonsmokers among females. Prevalence of antibodies to M. faeni or M. faeni and T. vulgaris was associated with a higher hay acreage and longer working hours per d…

We performed a follow-up study of 61 patients who had an acute episode of farmer's lung (54 men and seven women). Twenty-four subjects had ceased all contact with the barn, while 37 had continued farming. Pulmonary function tests for all subjects showed an initial improvement after the acute episode: 92.4 +/- 36.9 percent of predicted for carbon monoxide diffusing capacity (Dco) after one year, compared to 61.5 +/- 28.5 percent at diagnosis (p less than 0.01); and 6.01 +/- 1.50 L for total lung capacity (TLC) after three years, compared to 5.35 +/- 1.42 L (p less than 0.05). Subsequently, pulm

The object of this study was to analyze mineral particles in lung tissue from a farmer who underwent lobectomy due to a lung tumor and relate these analytical findings to occupational exposure and histopathological and clinical diagnoses. Despite no clear evidence of previous occupational dust exposure, heavy deposits of birefringent particles and slight pulmonary fibrosis were found during histopathological examination. Extracts from the lung tissue were analyzed by electron microscopy and X-ray microanalysis for mineral particles. The major components of the dust were identified as mica, tal

A farmer specialized in pig-breeding showed acute dermatitis of his hands, forearms and face after using chloropromazine for the sedation of pigs. Epicutaneous and photo-patch tests showed positive reactions to chloropromazine. Further tests with other occupational materials were negative. This observation points out that in cases of contact dermatitis in farmers, besides antibiotics, vitamins and other feeding additives, sedatives of the phenothiazin-type should not be forgotten.

The prevalence of farmer's lung disease and antibodies to farmer's lung antigens were studied in a probability sample of over 1,400 farmers in north central Wisconsin. The prevalence rate of farmer's lung disease was 4.2 per 1,000 farmers. The most prevalent antibody type was directed against Micropolyspora faeni, with 6% of the study population precipitin-positive. Prevalence of antibodies to M. faeni was associated with dairy farming, larger farms, and larger dairy herds. An additive effect of hay acreage and size of dairy herd was observed. An independent relationship was also observed for …

To determine the long-term effects of farmer's lung disease and the factors influening the outcome, 141 patients with farmer's lung disease were evaluated. At the time of the last follow-up, 29 patients had died and 92 (mean age, 54 years) were studied clinically, physiologically, and radiologically. The mean length of disease was 14.8 years (range, 2.25 to 40 years). Symptoms at the time of the last follow-up included complaints of cough (33 per cent of the patients), breathlessness while walking on the level (20 per cent), breathlessness on minor exertion (14 per cent), and breathlessness wh

Precipitating antibodies towards antigens associated with farmer's lung disease were studied in Finnish dairy farming and non-farming rural populations and the results were compared to those of serum samples of patients suspected of having a fungal allergy. The antigen panel consisted of Aspergillus fumigatus, Micropolyspora faeni and Thermoactinomyces vulgaris. All three microbes seemed to be important environmental inducers of antibody formation. The prevalence of antibodies towards all of them was higher in the patient group. The presence of A. fumigatus and T. vulgaris antibodies correlate
