[Fulminant necrotizing fasciitis secondary to Crohn's disease].

Necrotizing fasciitis is a life-threatening infection, commonly caused by group A streptococci, which has to be treated by surgical exploration and debridement during the first 24 h. Clinical clues are severe pain, in some cases followed by the appearance of bullous formations, and the detection of gas in the soft tissues by computed tomography or MRI. In addition to that, the infection is characterized by rapid inflammatory progression, producing a highly life-threatening situation. Diagnosis is finally based on surgical exploration obtaining specimens for culture and histopathologic examinat

Rapidly fatal necrotizing fasciitis after aesthetic liposuction.

Necrotizing fasciitis (NF) is a rapidly progressive soft tissue infection involving primarily the superficial fascia and subcutaneous tissue. The disease is caused by Streptococcus pyogenes or synergistic infection of anaerobic and facultative anaerobic bacteria. Further characteristics are severe, intolerable pain and a mortality rate of 30 to 50%. The NF can be initiated after surgical procedures, minor trauma, trivial scratches, in the setting of a chronic wound, or even in apparently intact skin. The age of the patient is not relevant for the prognosis of NF. As it is shown in this reporte

Systemic lupus erythematosus complicated by necrotizing fasciitis.

A case of necrotizing fasciitis (NF) is described in a 46-year-old woman with recent onset systemic lupus erythematosus (SLE). Deep-tissue infections are more common in SLE patients on high-dose corticosteroids, but, to our knowledge, this is the second case described in association with SLE. Although NF may initially be difficult to diagnose, the presence of marked systemic symptoms out of proportion to the local findings should suggest the correct diagnosis. NF diagnostic criteria, treatment and prognosis are discussed.

Pseudomonas aeruginosa necrotizing fasciitis: a case report.

A renal transplant recipient with necrotizing fasciitis due to Pseudomonas aeruginosa is presented. Only three other cases of necrotizing fasciitis due to this organism have been found in the literature. This is the first case of P. aeruginosa necrotizing fasciitis in a renal transplant patient to be reported. Prompt diagnosis, enabling treatment with appropriate antibiotics and early surgical intervention, was the key factor in his recovery.

Skin and soft tissue infection: necrotizing fasciitis.

The incidence of necrotizing fasciitis has increased in the past decade as a result of the resurgence severe group A streptococcal infection. Mortality has remained unchanged over the past 60 years, supporting the notion that immune modulators, such as intravenous immunoglobulin, are required to alter the physiological process during the early stages of infection.

Nonsteroidal anti-inflammatory drugs and necrotizing fasciitis.

The clinical features, etiology, and treatment of necrotizing fasciitis are reviewed, and the possible involvement of nonsteroidal anti-inflammatory drugs (NSAIDs) in the pathogenesis of this disease is discussed. Symptoms of necrotizing fasciitis include edema, erythema, warmth, and tenderness in the affected area, spreading rapidly to involve entire limbs or portions of the abdominal wall. Gangrene may develop, followed by multi-organ failure and death. The most common causative organisms are group A beta-hemolytic streptococci. Treatment includes tissue debridement, intravenous antibiotics,

[Fournier gangrene (manifestation of anogenital necrotizing fasciitis)].

In connection with a typical case, clinical features of genitaly extended Fournier gangrene--its pathogenic factors and the standpoints of treatment--are demonstrated. This infection of foudroyant nature, going on with significant mortality is held to be a characteristic form of necrotizing fasciitis, constituting main manifestation of toxic shock-like syndrome. Attention is drawn to the fact, that in case of internal diseases, predisposing factors respectively-even by adequate therapy-outcome of the infection may be lethal nowadays as well.

Cervical necrotizing fasciitis: clinical manifestations and management.

Forty-five cases of cervical necrotizing fasciitis are reported, and their clinical, bacteriologic, and therapeutic implications are considered. Fasciitis was of dental origin in 78% of cases, pharyngeal in 16%, and surgical or posttraumatic in 6%. The condition extended to the face in 22% of cases, to the lower part of the neck in 56%, and to the mediastinum in 40%. Soft-tissue cultures were positive in 78% of cases. Anaerobes were isolated along with aerobes in 49% of cases (mean, 2.2 isolates per patient) and in pure culture in 22%. Treatment included surgical debridement and drainage and t

Fatal necrotizing fasciitis following a mandibular fracture.

Necrotizing fasciitis of the head and neck is an uncommon, insidious infection that usually occurs secondarily to odontogenic infections, although blunt and penetrating trauma can be another cause. Of 65 total reported cases of necrotizing fasciitis, 10 (15.4%) have been fatal. This article presents a case of a mandibular fracture in a severely immunocompromised elderly patient in whom delayed treatment proved fatal. The clinician must remain suspicious of any infection refractory to antibiotic therapy alone, since necrotizing fasciitis is a rapidly progressing condition with high morbidity an

Spontaneous vulvar necrotizing fasciitis in Cushing's syndrome.

Necrotizing fasciitis is a potentially fatal condition, and the prognosis is worsened by immunosuppression. We describe what appears to be the first reported case of spontaneous vulvar necrotizing fasciitis occurring in a woman with uncontrolled hypercortisolism due to an ACTH-secreting thymic carcinoid tumor.

Necrotizing fasciitis secondary to discoid lupus erythematosus.

Necrotizing fasciitis (NF) is an uncommon, but devastating, disease with a significant morbidity and mortality, unchanged in the last several decades. This case report is the first successful management of a patient with NF secondary to discoid lupus erythematosus. A review of the literature describes current concepts of etiology, pathophysiology, diagnosis, and treatment of NF. This case report represents a growing class of patients at increased risk of NF due to iatrogenic immune compromise.

Zygomycotic necrotizing fasciitis caused by Apophysomyces elegans.

A case of necrotizing fasciitis of the anterior abdominal wall caused by the zygomycete Apophysomyces elegans in a healthy male following inguinal herniorrhaphy is reported. The portal of entry of the fungus into the incised skin and subcutaneous tissues was probably through either contaminated surgical sutures or postoperative surgical dressings. Broad, aseptate fungal hyphae were seen in the necrosed tissues with an associated necrotizing vasculitis. Extensive tissue debridements and a low dose of amphotericin B were not successful in controlling the rapid invasion of the tissues by the fung

Carpal tunnel release complicated by necrotizing fasciitis.

We report a 31-year-old diabetic woman who underwent carpal tunnel release for median nerve compression followed by a laparoscopic tubal ligation. The procedure was complicated by a severe postoperative necrotizing fasciitis infection of the carpal tunnel release incision. This has not been previously reported. The wound was poorly responsive to antibiotic therapy and serial wound debridements. Control of the woman's infection required total excision of the palmar skin and fascia. Complicating factors in this case included the woman's long history of insulin-dependent diabetes and a concomitan

Necrotizing fasciitis rapidly diagnosed by aspiration cytology.

A case of necrotizing fasciitis caused by beta-hemolytic streptococci is reported. A 66-year-old man was admitted because of pain and swelling in the right buttock. Rapid application of aspiration cytology made it possible to diagnose necrotizing fasciitis with bacterial infection. Unfortunately, however, the patient died of cardiac arrest due to hyperpotassemia 11 h after admission. Mortality from this disease is most often related to failure in recognizing it early. Rapid diagnosis and early treatment is mandatory in order to save the patients' life. We emphasize the usefulness of rapid aspi

Management of perineal necrotizing fasciitis (Fournier's gangrene).

Eleven cases of perineal necrotizing fasciitis were treated in the Department of Surgery, University of Turku, Finland. In 10 cases the diagnosis was made on the basis of the fulminating progression of the infection to scrotal gangrene, identification of multiple underlying pathogenic organisms and toxaemia. In one female patient a corresponding infection developed in the labia majora. Each patient suffered nonspecific symptoms before the gangrene became evident. The management included surgical debridement of the necrotic tissue with incisions and drainage of the involved areas, antibiotic th
