PALPABLE: enhanced bending stiffness and proprioception in soft actuators for laparoscopic palpation.

The assessment of tissue properties via direct palpation is essential for localizing abnormalities during surgery. However, Minimally Invasive Surgery (MIS) eliminates this tactile feedback, creating a critical sensory gap. To address this, the EU Horizon project PALPABLE is developing novel fiber-optic sensing modalities for stiffness assessment. Successful integration of this technology requires an articulation interface that balances high maneuverability with the structural stability necessary for deflection-free tissue palpation. We present a single-degree-of-freedom (DOF) soft silicone ac

Real-Time Imaging of Single Retinal Cell Apoptosis in a Non-Human Primate Ocular Hypertension Model.

PURPOSE: To evaluate the feasibility of using DARC (detection of apoptosing retinal cells) technology as a biomarker for preclinical assessment of glaucomatous damage in a non-human primate (NHP) model of ocular hypertension (OHT). METHODS: Elevated intraocular pressure (IOP) was induced by applying a laser to the trabecular meshwork in each eye of NHPs. Changes in DARC counts in the retina, identified as fluorescent-tagged annexin V (ANX776)-positive cells, were evaluated together with optic nerve damage, assessed using spectral domain-optical coherence tomography. The pharmacokinetic proper

A Multiplex "Disposable Photonics" Biosensor Platform and Its Application to Antibody Profiling in Upper Respiratory Disease.

Photonic technologies promise to deliver quantitative, multiplex, and inexpensive medical diagnostic platforms by leveraging the highly scalable processes developed for the fabrication of semiconductor microchips. However, in practice, the affordability of these platforms is limited by complex and expensive sample handling and optical alignment. We previously reported the development of a disposable photonic assay that incorporates inexpensive plastic micropillar microfluidic cards for sample delivery. That system as developed was limited to singleplex assays due to its optical configuration.

Innovative surgical guidance for label-free real-time parathyroid identification.

BACKGROUND: Difficulty in identifying the parathyroid gland during neck operations can lead to accidental parathyroid gland excisions and postsurgical hypocalcemia. A clinical prototype called as PTeye was developed to guide parathyroid gland identification using a fiber-optic probe that detects near-infrared autofluorescence from parathyroid glands as operating room lights remain on. An Overlay Tissue Imaging System was designed concurrently to detect near-infrared autofluorescence and project visible light precisely onto parathyroid gland location. METHODS: The PTeye and the Overlay Tissue

Multicolor Imaging for the Detection of Inner Nuclear Layer Microcysts Secondary to Optic Nerve Atrophy.

Inner nuclear layer (INL) microcysts have been reported in diseases affecting the optic nerve. The new ocular imaging techniques detect minimal structural alterations at the macula and correlate these findings to different etiologies with less invasive procedures. The relationship between ganglion cells distribution at the macula and chiasmal nerve fibers enables the diagnosis and location of neurological lesions by new generation optical coherence tomography (SD-OCT) imaging devices. We report the evaluation of a patient with a history of optic nerve trauma and macular INL microcysts with mul

Clinical Application of Adaptive Optics Imaging in Diagnosis, Management, and Monitoring of Ophthalmological Diseases: A Narrative Review.

Visualization of the retinal structure is crucial for understanding the pathophysiology of ophthalmic diseases, as well as for monitoring their course and treatment effects. Until recently, evaluation of the retina at the cellular level was only possible using histological methods, because the available retinal imaging technology had insufficient resolution due to aberrations caused by the optics of the eye. Adaptive optics (AO) technology improved the resolution of optical systems to 2 µm by correcting optical wave-front aberrations, thereby revolutionizing methods for studying eye structures

A new method for long-term lumbar pressure monitoring with a fiber optic catheter.

A new method for long-term monitoring of cerebrospinal fluid (CSF) pressure in the spinal canal using a catheter with a fiberoptic pressure monitor is described. This method has been used in the investigation of 16 patients with communicating hydrocephalus (CH). Duration of monitoring ranged from 2-48 hours without any complications. In 6 cases the recording was interrupted prematurely. Valuable data on the patterns of pressure variation could be attained and the occurrence of B-waves showed a good correlation with the clinical outcome of shunt operation. No sign of external or internal leakag

Fiber optic probe augmented sonic scaling versus conventional sonic scaling.

Several factors, including access and visualization problems, make total deposit removal during scaling and root planing procedures extremely difficult. This study examined the effectiveness of a mode of therapy designed to improve access and visualization for sonic scaling compared to closed sonic instrumentation. Teeth with moderate to deep probing depths in six patients scheduled to receive immediate dentures were divided into three experimental groups: Group I, sonic scaling with access augmented by interdental papilla reflection and fiber optic illumination/transillumination (34 surfaces)

Findings of Optical Coherence Tomography of Retinal Nerve Fiber Layer in Two Common Types of Multiple Sclerosis.

Multiple sclerosis (MS) is the most prevalent disease caused by the inflammatory demyelinating process that causes progressive nervous system degeneration over the time. Optical Coherence Tomography (OCT) is a non-invasive optical imaging technology, which can measure the thickness of retinal nerve fiber layer as well as the diameter of the macula. The purpose of the study is evaluation OCT findings in two common types of multiple sclerosis. For doing the cross-sectional study, 63 patients with two prevalent types of multiple sclerosis (35 patients with Relapse Remitting Multiple Sclerosis (RR

Phase optimization approach to control high peak-to-average power ratio in a second-order soliton-based NFDM system.

Fiber-optic communication is an emerging technology that enables high-capacity data transmission using light waves. However, the inherent nonlinearity of the optical fiber limits the attainable capacity. As a result, nonlinearity compensation has become a significant focus in optical fiber communication, for which nonlinear frequency-division multiplexing (NFDM) is an effective solution. The signals generated in an NFDM system are fundamentally optical soliton signals and hence undergo the effect of multi-soliton periodicity. But this results in a higher signal peak-to-average ratio (PAPR) at

Plasmonic Coupling on an Optical Microfiber Surface: Enabling Single-Molecule and Noninvasive Dopamine Detection.

Optical fibers can be effective biosensors when employed in early-stage diagnostic point-of-care devices as they can avoid interference from molecules with similar redox potentials. Nevertheless, their sensitivity needs to be improved for real-world applications, especially for small-molecule detection. This work demonstrates an optical microfiber biosensor for dopamine (DA) detection based on the DA-binding-induced aptamer conformational transitions that occur at plasmonic coupling sites on a double-amplified nanointerface. The sensor exhibits ultrahigh sensitivity when detecting DA molecules

Navigating Intraoperative Dislodgement: Anaesthetic Management in Foreign Body Retrieval with Bronchotomy and Fibre-optic Bronchoscopy.

Foreign body aspiration, particularly in the lower airway, presents unique challenges for anaesthesiologists, especially when dislodgement occurs during surgical manipulation. We report the case of a 21-year-old male who presented with an iron nail lodged in the left lower lobe of the lung, with a history of failed removal using flexible and rigid bronchoscopy. After unsuccessful attempts, the patient underwent left thoracotomy for foreign body retrieval. During the procedure, the foreign body dislodged into the tracheal wall through the Murphy eye of the endotracheal tube. A multimodal approa

Measurement of Airway Length in Neonates Using Fiberoptic Bronchoscopy.

INTRODUCTION: Optimal positioning of the endotracheal tube is a crucial step in the airway management of neonates. Short airway length creates a narrow margin of safety and thus a higher chance of ETT malpositioning. Published literature reports airway length of 5-7.5 cm, which is primarily based on autopsy and radiologic studies. We decided to measure the airway length in neonates undergoing surgeries using flexible fiberoptic bronchoscopy (FFOB). METHODOLOGY: Sixty full-term neonates, scheduled for surgery under general anesthesia with endotracheal intubation, were included in the study. Ne

Point-of-care therapeutic drug monitoring of adalimumab by integrating a FO-SPR biosensor in a self-powered microfluidic cartridge.

Disease treatment with advanced biological therapies such as adalimumab (ADM), although largely beneficial, is still costly and suffers from loss of response. To tackle these aspects, therapeutic drug monitoring (TDM) is proposed to improve treatment dosing and efficacy, but is often associated with long sampling-to-result workflows. Here, we present an in-house constructed ADM-sensor, allowing TDM of ADM at the doctor's office. This biosensor brings fiber optic surface plasmon resonance (FO-SPR), combined with self-powered microfluidics, to a point of care (POC) setting for the first time. Af

Videolaryngoscopy vs. fibreoptic bronchoscopy for awake tracheal intubation: a systematic review and meta-analysis.

Awake fibreoptic intubation is often considered the technique of choice when a difficult airway is anticipated. However, videolaryngoscopes are being used more commonly. We searched the current literature and performed a meta-analysis to compare the use of videolaryngoscopy and fibreoptic bronchoscopy for awake tracheal intubation. Our primary outcome was the time needed to intubate the patient's trachea. Secondary outcomes included: failed intubation; the rate of successful intubation at the first attempt; patient-reported satisfaction with the technique; and any complications resulting from
