Clinical application of an expanded reverse-island flap with two dorsal metacarpal arteries and dorsal metacarpal nerves in index- and middle-finger-degloving injury repair and amputation reconstruction.

The dorsal metacarpal artery flap (DMAF) is irrefutable as an effective way of repairing long finger defects, and hand surgeons might consider using it for long finger reconstruction or degloved injury repair. Unfortunately, the DMAF containing a single dorsal metacarpal artery (DMA) hinders the treatment effect. The sensory restoration of long fingers and the reconstruction of phalangeal joints and tendon grafts are unsolved challenges as well. We reported our experience in reconstructing the index and middle finger by a reverse-island flap with two DMAs and dorsal metacarpal nerves (DMNs) wi

A Pilot Study of Compensatory Strategies for Reach-to-Grasp-Pen in Patients with Stroke.

Coordinated reaching and grasping movements may be impaired in patients with poststroke hemiplegia. Patients frequently adopt compensatory strategies, which require investigation. This pilot study used kinematic parameters to examine compensatory strategies by assessing the reach-to-grasp-pen movements in patients with stroke and unaffected participants. Twelve patients with stroke with mild impairment (45.16 ± 12.62 years, 2.41 ± 1.97 months), twelve with moderate impairment (50.41 ± 12.92 years, 3.83 ± 3.58 months), and ten healthy individuals (20.6 ± 0.69 years) performed a reach-to-grasp-p

Autogenic heterotopic vascularized proximal interphalangeal joint transplantation in children.

The proximal interphalangeal joint (PIP) joint is the most crucial joint for the functionality of a finger. For a child with complex injury of the hand every effort should be exercised to maximize function restoration. If the PIP joint is irreparably damaged, its reconstruction is indicated. The technique of autogenic heterotopic vascularized toe joint transplantation provides unique advantage of a composite transfer of skin, tendons, bone and joint alone with growth plate and its efficacy has been affirmed in children. It has been suggested that such transfers require intact flexor tendon to

Spontaneous recurrent ulnar dislocation of the long extensor tendons of the fingers.

Ulnar dislocation of the long extensor tendons of one or more fingers occurred in five elderly women. There was no history of trauma or any of the stigmata of rheumatoid arthritis. In all fingers there was loss of active extension in the metacarpophalangeal joints following flexion. It is postulated that this is a definite clinical entity occurring in the elderly associated with excessive joint laxity.

Solitary periosteal chondroma presenting as a snapping finger: an unusual location.

We report an unusual case of a solitary periosteal chondroma presenting as a snapping finger and pain in the ring finger in a 37-year-old man. The snapping symptom was caused by impingement of this solitary chondroma. In this case, the periosteal chondroma was detached due to a finger sprain. Thus, the solitary chondroma was impinged between basal phalanx and extensor tendon, causing the pain and snapping finger. When patients with snapping finger present, CT scans can be helpful to make a correct diagnosis. To the best of our knowledge, this condition has not been described in the PIP joint.

[Functional value of the hand after digital replantation (author's transl)].

The absolute and relative indications and contraindications of fingerreplantation are exposed. The function of the replanted fingers depends on the level of amputation, the bone-joint and tendon lesions and the presence of protective sensibility. The range of joint movement of the replanted fingers is related to the amputation wound. The residual motion depends also on the level of amputation giving better results as the amputation is more distally situated. Limited range of movement in multiple amputations is not an obstacle to a useful grasp and pinch, which results in a more or less functio

Mallet Finger in a Toddler: A Rare But Easily Missed Injury.

A mallet finger is a flexion deformity of a finger at the distal interphalangeal joint due to an injury of the extensor mechanism at the base of the distal phalanx. Most common in middle-aged men, injuries in the pediatric population are less common and rare in toddlers. We describe a case of missed mallet finger and its subsequent treatment in a female toddler.

Simultaneous double second toe transfer for reconstruction of adjacent fingers.

BACKGROUND: The authors reviewed their results with simultaneous double second toe transfer for reconstruction of adjacent fingers. METHODS: From January of 1984 to January of 2003, simultaneous double second toe transfer was performed for reconstruction of adjacent fingers in 140 patients (48 before wound closure, 92 after). Functional thumbs were present in all of them. There were 104 male and 36 female patients, with an average age of 29.1 years (range, 2 to 57 years). Index and middle fingers were reconstructed in 90 patients, middle and ring fingers in 49 patients, and ring and little fi

Ultrasonographic assessment of clinically diagnosed trigger fingers.

This study examines flexor digitorum tendons and A1 pulley in patients presenting trigger fingers using high-resolution ultrasonography (US), determines the accurate causes of trigger fingers, and analyzes the relationship between clinical data and US findings. As much as 50 trigger fingers of 41 patients were examined by high-resolution US, and the US findings were analyzed as tendon thickness, fibrillar echotexture, tendon margin, fluid collection, A1 pulley thickening, tendon sheath cyst, and metacarpophalageal (MCP) joint abnormalities. The affected fingers were compared with the asymptoma

Modified Thompson-Littler Method for Chronic Instability of the Proximal Interphalangeal Joint: A Report of Two Cases.

A 40-year-old patient and a 45-year-old patient presented with chronic dorsal and lateral subluxation of the proximal interphalangeal (PIP) joint of the little finger and underwent surgeries. Through a dorsal approach, the ulnar lateral band was transected and transferred to the radial side through the volar side of the PIP joint. The transferred lateral band and the remnant of the radial collateral ligament were secured with an anchor on the radial side of the proximal phalanx. Satisfactory results were obtained without the loss of flexion and recurrence of subluxation of the finger. This met

Mallet finger.

Mallet finger involves loss of continuity of the extensor tendon over the distal interphalangeal joint. This common hand injury results in a flexion deformity of the distal finger joint and may lead to an imbalance between flexion and extension forces more proximally in the digit. Mallet injuries can be classified into four types, based on skin integrity and the presence or absence of bony involvement. Although various treatment protocols have been proposed, splinting of the distal interphalangeal joint for 6 to 8 weeks has yielded good results while minimizing morbidity in the majority of pat

Tendon and nerve displacement at the wrist during finger movements.

BACKGROUND: Repetitive motion of the hand has been suggested as a major factor of pathogenesis of cumulative trauma disorders (e.g., carpal tunnel syndrome). The purpose of this study was to investigate the 3D displacement of the median nerve and extrinsic finger flexor tendons (flexor digitorum superficialis; flexor digitorum profundus) as a function of flexion/extension of metacarpophalangeal joints of the index and middle fingers. METHODS: Shim markers were placed on the median nerve, flexor digitorum superficialis, and flexor digitorum profundus tendons at the wrist region of seven cadave

Debulking and Osteotomy Procedures to Correct Severe Macrodactyly Deformity of the Hand in Young Patients.

BACKGROUND: This study evaluated early debulking combined with corrective osteotomy for the treatment of hand macrodactyly in young patients. METHODS: This retrospective study included 31 young patients (mean age, 12.7 years) with severe hand macrodactyly. All the patients underwent microsurgical debulking based on multiple pedicled flaps and osteotomy to reduce soft-tissue volume and correct finger deformity. The mean follow-up period was 3.1 years. Clinical evaluations included ratios of finger length, circumference, and nail dimensions compared with the intact fingers; metacarpophalangeal

A Rare Case of Trigger Finger Caused by Tumoral Calcinosis in Zone III of the Palm.

Tumoral calcinosis (TC) is a pathological condition characterized by tumor-like calcific deposits in the soft tissues surrounding the joints. This condition typically affects large joints; however, TC involving hand tendons is extremely rare. There are no previously reported cases of TC occurring in Zone III of the palm, and no prior reports of trigger finger secondary to TC. We present a case of TC arising in Zone III of the palmar aspect of the right ring finger in a 65-year-old man who was referred for treatment of trigger finger. Surgical excision of the lesion resulted in symptomatic impr

Metacarpophalangeal subluxation: locked finger.

The so-called locked finger is a rarely reported condition that can be easily recognized when its typical picture is borne in mind: painful locking of a finger, making it impossible to fully extend the metacarpophalangeal joint, and a lateral displacement of the extensor tendon at this level. Radiological findings may include abnormal shape of the metacarpal head, free intra-articular bodies, sesamoid bones at the volar plate level and degenerative changes in the metacarpophalangeal joint. There is consistently a previous history of repeated light manual effort. This condition generally affect

Examination of the traction effect in an artificial muscle-type dynamic traction orthosis using computed tomography.

BACKGROUND: Dart-throwing motion from radial dorsiflexion to palmar flexion has recently attracted attention as a functional movement direction of the wrist joint. We developed a novel artificial muscle-type dynamic traction orthosis (DTSaM) that includes these movements. This prospective crossover controlled study aimed to compare the traction effects in the presence and absence of DTSaM using computed tomography. METHODS: Healthy participants with no history of finger disease (6 men: 6 fingers, 4 women: 4 fingers; average age [range]: 29.4 [34-24] years) were examined. The distance and area

Bilateral volleyball-related deformity of the little fingers: mallet finger and clinodactyly mimic.

A 14-year-old male high school volleyball player was seen to evaluate right- and left-hand little-finger distal interphalangeal joint deformity and pain. His symptoms began during his second season of competitive play. The distal interphalangeal (DIP) joints of the little fingers flexed 20-30°, and a 10-15° valgus deformity was seen at the same joints. Pain was relieved with rest but returned immediately after playing volleyball, so plain radiographs were obtained. The flexion and valgus deformity was obvious on plain radiographs and through a clinical examination. Thus, a bilateral little-fin

A new dynamic splint for postoperative treatment of flexor tendon injury.

This dynamic splint lessens resistance to finger extension and increases the arc of motion through full passive flexion of the injured fingers. Rubber bands run from the tips of the injured fingers under a spring-loaded roller bar at the metacarpophalangeal joint level to a coiled lever at the distal flexor surface of the forearm. The wrist is positioned in 45 degrees of flexion with 40 degrees to full flexion of the metacarpophalangeal joints and full flexion to full extension of the interphalangeal joints. We retrospectively compared patients treated in the new and traditional splints. Patie

Rupture of flexor tendons due to pisotriquetral osteoarthritis.

We are reporting a case of a flexor profundus tendon rupture of the little finger due to pisotriquetral osteoarthrosis. Resection of the pisiform bone and resurfacing of the carpal tunnel with the joint capsule was performed for the osteoarthrosis and free tendon grafting for the tendon rupture. The tendon grafting and a subsequent tenolysis restored the function of the little finger. Other than radiological changes in the pisotriquetral joint, the diagnostic value of preoperative radiocarpal arthrography was emphasized in this rare complication of pisotriquetral arthrosis.
