1. Atrial fibrillation is an inefficient cardiac rhythm associated with impaired exercise tolerance, exertional dyspnoea, palpitation and a substantial risk of thromboembolism. 2. The first decision in management is to consider cardioversion which can be achieved in suitable cases electrically, or pharmacologically with a class Ic antiarrhythmic drug like flecainide or propafenone. 3. Prophylaxis in paroxysmal atrial fibrillation is best achieved with a class Ic drug or a class III drug such as sotalol or amiodarone. 4. Control of ventricular rate in chronic atrial fibrillation can be achieved…

A newborn infant with familial Wolff-Parkinson-White (WPW) syndrome presented with a supraventricular tachycardia of 300 beats/min, refractory to digoxin and flecainide administration. Serial electropharmacologic tests were performed via the esophagus before and during oral therapy with verapamil at 40, 80, and 60 mg daily. Before treatment, tachycardia could be induced with programmed stimulation. A regimen of verapamil at 60 mg daily, which resulted in the initiation of nonsustained (less than 10 s) reciprocating tachycardia only, without clinical recurrences, was identified as suitable long…

Atrial fibrillation (AF) is the most common cardiac arrhythmia encountered in clinical practice projected to affect 12.1 million individuals … the patients was 71.06 years (SD = 7.66). About 45 patients were given DAAM of either amiodarone + flecainide or dronedarone + flecainide and were treated for an average of 15.4 months, followed by catheter ablation, if needed. Also, 41 patients received a SAAM followed by catheter ablation, if needed. A Mann-Whitney test indicated that electrical cardiover …

BACKGROUND: [corrected] Bepridil, a multiple ion-channel blocker, has been reported to prevent paroxysmal atrial fibrillation (PAF). The f-f interval of PAF during treatment with bepridil versus class Ic antiarrhythmic drugs was compared.
METHODS: Fifty-two patients with PAF were randomized to bepridil, 200 mg/day (n = 14) versus flecainide, 100 to 200 mg/day (n = 15) or pilsicainide, 75 to 150 mg/day (n = 23). The drug was considered effective when symptomatic episodes of PAF were decreased to < 50% during a follow-up of 2 to 6 months. The f-f interval was measured in 12-lead ECGs of initial…

Fetal tachyarrhythmias are uncommon but are associated with significant perinatal mortality and morbidity. Fetal echocardiography permits the accurate determination of the nature of the arrhythmia. Transplacental or direct fetal therapy with anti-arrhythmic drugs can successfully cardiovert the fetus in utero with a subsequent reduction in perinatal problems. The investigation and management of these unusual conditions is reviewed.

His bundle tachycardia is a rare arrhythmia in infants and children and is resistant to therapy. There is now sufficient evidence from reported cases and a further patient reported below to suggest that drug management of this arrhythmia is a reasonable alternative to His bundle ablation with life-long pacing from infancy.

Human atrial tissue exhibits specific electrophysiological properties such as the presence of plateau shaped action potentials characterized by an initial notch followed by a long lasting plateau. In these cells, the notch and the plateau are due to the activation of transient outward currents Ito and slow inward current Isi. The effects of antiarrhythmic agents on this tissue depend on the morphology of the action potentials. We demonstrated that, in these plateau shaped fibers, action potential duration at 50 and 90% of repolarization was increased by flecainide at the concentration of 5 x 1…

In cases of non-immune hydrops fetalis caused by tachyarrhythmias, the transplacental passage of antiarrhythmic drugs may be hampered. When this is proven by fetal blood sampling in cases of tachyarrhythmia refractory to transplacental treatment, additional administration of antiarrhythmic drugs into the fetus is necessary and seems to improve the results. Although injections of antiarrhythmic agents in fetal ascites are also highly effective, intravascular administration by sonographic guidance is to be preferred. Then, simultaneous measurements of fetal and maternal drug levels are possible

Heat stroke is an acute life-threatening disease requiring therapy immediate to cope with the severe pathophysiologic disturbances. Rapid lowering of the high core temperature seems to be the most important therapeutic measure. Besides methods of physical cooling, Dantrolene should also be used in this hyperthermic state. By consequent lowering of the body temperature it will be possible to stabilise circulation and minimise disease-related injuries of different organs.

Brugada syndrome is characterized by episodes of near sudden death or syncope. The heart is normal on physical, angiographic and echocardiographic examination. The characteristic ECG abnormalities consist of ST-segment elevation in leads V1-V3 and pseudo right bundle branch block. The ECG changes can be transient, causing the diagnosis to be missed. Provocative testing with Flecainide or other sodium channel blocking drugs can be used to unmask the ECG abnormalities. This is a case study of a young male who had been diagnosed as suffering from Brugada syndrome and had a defibrillator implanted…

Brugada syndrome is one of the important causes of sudden cardiac death in young adults. The condition is associated with typical ECG changes in anteroseptal leads V1 and V2 that can be unmasked by various medications, electrolyte disturbances, and even by fever in susceptible individuals. We here report the case of a 22-year-old female admitted to the emergency room with a typical atrial flutter who developed Brugada-like ECG changes after conversion to sinus rhythm following flecainide infusion with subsequent degeneration in ventricular fibrillation. The patient converted to sinus rhythm af…
