Listeriosis is a rare food borne infection which, in the invasive form, presents as bloodstream infection, central nervous system infection, materno-fetal infection, or focal infection. Certain immunosuppressive conditions have been identified as risk factors for severe invasive disease. The invasive forms of listeriosis are associated with a high case fatality rate. We present the case of a 62-year-old male with an unremarkable medical history admitted to the Iasi Infectious Diseases Hospital for fever. headache, ataxia, and diplopia. Physical examination revealed high temperature, confusion,…

A case is described of pericardial effusion due to Listeria monocytogenes infection in a woman with advanced carcinoma of the cervix, rheumatoid arthritis and on corticosteroid therapy. Focal infection with listeria in immunocompromised adults has a high mortality unless promptly diagnosed. The correct diagnosis may have been made if an unexplained pericardial effusion had been tapped.

Two infants whose malformations fulfilled the clinical, radiographic and pathologic criteria of campomelic syndrome, were additionally found to have features suggestive of intrauterine viral infection. The presence of hydrocephalus and hydromyelia in both neonates and focal proliferative villitis in the placenta of one suggests that intrauterine viral infection may play a role in the pathogenesis of campomelic syndrome.

Neonatal group B streptococcus meningitis causes neurologic morbidity and mortality. Cerebrovascular involvement is a common, poorly studied, and potentially modifiable pathologic process. We hypothesized that imaging patterns of focal brain infarction are recognizable in neonatal group B streptococcal meningitis. A consecutive case series included term neonates with the following: (1) bacterial meningitis, (2) acute group B streptococcal infection (positive cerebrospinal fluid/blood culture), (3) brain magnetic resonance imaging within 14 days, and (4) acute intraparenchymal focal infarctions

Splenic infarction (SI) is often associated with circulatory and hematological diseases and infections. Here, we report a rare case of SI in an adult with infectious mononucleosis (IM) caused by the Epstein-Barr (EB) virus. A 31-year-old male with an unremarkable medical history presented with abdominal pain and fever. Contrast-enhanced computed tomography revealed focal SI. The splenic artery branching from the superior mesenteric artery was <5 mm in diameter. The diagnosis of EB virus infection was made based on physical examination and blood test results. As no evidence of cardiogenic disea

In rats, CL-Brener clone caused high mortality, severe acute myocarditis, and myositis that subsided completely in surviving animals. Accordingly, no parasite kDNA could be amplified in several organs after 4 months. The monoclonal JG strain caused null mortality, acute predominantly focal myocarditis, discrete and focal myositis, and a chronic phase with sparse inflammatory foci. Double infection with both Trypanosoma cruzi populations turned mortality very low or null. At the end of the acute phase, the heart exhibited only JG strain kDNA (LSSP-PCR), while skeletal muscles and rectum exhibit

OBJECTIVE: To analyze the ultrasonographic features of focal xanthogranulomatous pyelonephritis.
METHODS: Ultrasonographic features of 15 patients with pathologically proved focal xanthogranulomatous pyelonephritis were retrospectively analyzed by 2 radiologists who reached a consensus, in terms of the location, margin, size, and echo texture of the mass, associated calculi, lymphadenopathy, or local extension, in comparison with computed tomographic and clinical findings.
RESULTS: At ultrasonography, 12 (80%) of 15 masses were well circumscribed. The maximal sizes of the masses ranged from

We studied the clinical utility of in situ hybridization (Hybrisep) in diagnosing respiratory infection and/ or sepsis. Peripheral blood was taken from patients with respiratory infections and suspected sepsis for both routine blood culture and in situ hybridization, and focal samples including sputum, bronchoalveolar lavage, and central and thoracic catheter, were simultaneously examined for bacterial culture. Specimens numbered 46. The clinical diagnosis was 20 cases of septicemia, with 26 specimens diagnosed as respiratory infectious diseases including hospital-acquired pneumonia and pleuri

Papillomaviruses are known to be oncogenic in animals. In humans they are associated with benign squamous tumors (verruca, condylomata acuminata, and papillomas) in a variety of body sites. Human papillomavirus (HPV) infection of the esophagus, however, has not previously been documented. Recent reports of condylomatous changes in esophageal epithelium adjacent to esophageal carcinoma and the sporadic descriptions of esophageal papillomas in the literature for many years, lend credence to the assumption that HPV may affect the squamous mucous membrane of the esophagus. In the current study 75

The clinical features in 54 juvenile and adult patients with Listeria monocytogenes infection of the central nervous system are described. Thirty-one of the patients had pre-existing chronic disease; the remainder were previously healthy. Twenty of the patients, the 'meningo-encephalitic group' developed focal neurological signs. The remaining 34, the 'meningitic group' exhibited no focal signs other than those caused by increased intracranial pressure. The 'meningitic group' differed from the 'meningo-encephalitic group' in that the cerebrospinal fluid white cell count, protein and glucose le

We report a case of a 9-month-old baby admitted to the hospital because of low-grade fever, focal seizures in a context of watery diarrhea for 14 days' duration. The patient workup revealed a mild neutrophilic pleocytosis on cerebrospinal fluid (46 cells/microl), a positive stool culture for Salmonella pomona sensitive to ceftriaxone and ciprofloxacin, and a subdural empyema (SDE) on the cerebral MRI. The child received an intravenous third-generation cephalosporin for 4 weeks which resulted in cure. This case highlights an unusual extra-intestinal complication of non-typhoid salmonella infect

BACKGROUND: Intrauterine infection is a recognized cause of adverse pregnancy outcome, but the source of infection is often undetermined. We report a case of stillbirth caused by Fusobacterium nucleatum that originated in the mother's mouth.
CASE: A woman with pregnancy-associated gingivitis experienced an upper respiratory tract infection at term, followed by stillbirth a few days later. F. nucleatum was isolated from the placenta and the fetus. Examination of different microbial floras from the mother identified the same clone in her subgingival plaque but not in the supragingival plaque, v

Inadequate treatment or neglect of odontogenic infections can have serious consequences. The potential for spreading through fascial planes and intracranially can cause compromise of the airway and cavernous sinus thrombosis respectively. On rare occasions this can lead to a rapidly progressing necrotising fasciitis, with destruction of soft tissue, making reconstruction difficult. Antibiotic administration without removal of the cause is inadequate and can complicate subsequent management. We report the presentation and successful management of a 13-year-old boy, who developed necrotising fas

Recently, it has been recognized, that oral infection especially periodontitis may affect the pathomechanism and course of a number of systemic diseases, such as: cardiovascular, cerebrovascular diseases, atheromatous peripheral vascular disease bacterial pneumonia, diabetes mellitus, osteoporosis, or cause adverse pregnancy outcome. This review will focus on the current knowledge linking periodontal infections to a set of above mentioned systemic diseases. While a number of their mutual interactions have been already identified, additional research will be required to determine with certainty

A common sequela of certain malignancies is nonbacterial thrombotic endocarditis (NBTE), a phenomenon in which sterile fibrin/platelet aggregates are deposited onto normal cardiac valves. These verrucae represent a predisposing factor for the initiation of infective endocarditis following a bacteremia. This paper presents a case history which is highly suggestive of infective endocarditis which occurred as a result of multiple odontogenic abscesses in a patient with Hodgkin's lymphoma. The case illustrates the important role that the dentist can play in the management of cancer patients and em

In 4 cases of mediastinitis caused by anaerobes and consecutive to dental abscess (2 cases), perforation of the oesophagus and tonsillar abscess, the diagnosis was based on widening of the mediastinum associated, in 2 cases, with pneumomediastinum. II all 4 cases, Bacteroides fragilis was present among other germs. Treatment consisted of surgical drainage by thoracotomy (2 cases), or cervicotomy and antibiotic therapy. Two of the patients, who were old and had underlying diseases, died.

The generalized choroidal and retinal response to a focal nonreplicating infection of the eye with ascarid larvae was examined in an animal model. Intravitreal injection of Ascaris suum larvae in guinea pigs induced a diffuse eosinophilic choroiditis, retinal pigment epitheliopathy and neuroretinal degeneration, distant from focal reactions about larvae. As the choroiditis progressed, inflammatory cells separated the choriocapillaris from Bruch's membrane, and the endothelial cells lost their fenestrations. Focal disruption of the elastic and outer collagenous layers of Bruch's membrane occurr

A retrospective case note survey of 67 surgically proven cases of intracranial abscess formation has been carried out. Males predominated, especially in the third decade. Only 15 per cent had a predisposing condition such as cyanotic heart disease though the majority had some evidence of a systemic infection or fever. A peripheral leucocytosis and elevated erythrocyte sedimentation rate (ESR) were also more frequently encountered than in a comparable group of cases of cerebral tumour. Despite these pointers to an infective process the correct diagnosis was commonly not considered on admission.
