BACKGROUND: The aim of this study was to evaluate the Amplatzer septal occluder (ASO) for transcatheter closure of fenestrations and excluded hepatic veins in patients after modified Fontan operations. Residual right-to-left shunts have improved surgical results of the Fontan operation. Shunt closure may eventually be desirable to eliminate hypoxemia and reduce risk of embolic complications.

Despite advances in Fontan surgery, obstruction in extracardiac ducts may occur and cause clinical deterioration. We report two cases of stent implantation for correction of stenosis in the Fontan surgery. Ascites was a common clinical sign; a patient was protein losing enteropathy. This study evaluated use of Amplatzer fenestrated device to maintain patency of the Fontan fenestration and atrial septal defect. MR imaging allows functional evaluation of the ventricles of the entire heart because it evaluates cardiac anatomy three-dimensionally. Such evaluation is independent of chamber size and shape, and virtually independent of mathematical assumptions. Applying standard measurements of volume and function in children who have undergone multi-staged Fontan operations, we have shown diminished ventricular volumes and mass, and decreased cardiac indices, in post-Fontan procedure. These variations may be predictive of ultimate outcome in these complex patients.
