Peak pressure and temporal parameters of foot function were determined in 21 men and 11 women; few differences between men and women in any of the parameters were observed, either when walking barefoot or when wearing shoes. However, significant differences were observed when barefoot and shod walking were compared. The main influence of shoes appears to be in modifying the behaviour of the forefoot, by changing the pressure distribution across the metatarsal heads and increasing the contact times for the toes. In the surgical management of congenital pes valgus, vertical talus is the hardiest deformity to be reduced. By opening the lower tibio-fibular joint, associated with classic capsulotomies and tendons-lengthening, the authors have reduced vertical talus easily in three feet, for two children. One of them had a myopathy (central core disease), the other had a muscular hypotonia by cerebral palsy. Morphological results are good, but the function remains fair, due to the muscular weakness, that is usual in these lesions.

Myotonic dystrophy is a progressive multisystem disorder that may be inherited from either parent, although only children with affected mothers usually show the more severe congenital form of the condition; others are classified as having adult myotonic dystrophy. In the child between 4 and 12 years of age presenting with cavus deformity associated with spastic cerebral palsy, symptomatic feet may be improved by soft tissue release of plantar structures followed by appropriate tendon lengthenings or transfers and good cast correction. It is more common for the deformity to present in teenage patients, however, with a more rigid foot usually complicated by some degree of varus and equinus. In these situations, triple arthrodesis is often necessary and is a predictable procedure for correction and stabilization of the deformity.

One third of a patient population with idiopathic parkinsonism was found to suffer from debilitating, painful dystonic movements of the lower extremities. The prevalence of this involuntary movement disorder was found to be positively correlated with the duration of dopaminergic treatment, but it also occurred occasionally in untreated persons. A survey was conducted of 143 cases of equinovarus feet associated with myelomeningocele, most of which had been treated with the minimum of operative treatment, until the age of 6, when service and persistent deformities were corrected with the Dillwyn Evans collateral operation. A prosthesis has been designed with which to resurface the painful, degenerated first metatarsophalangeal joint. Clinical evaluation thus far has indicated satisfactory results in 16 of 18 instances. A patient coming from an area of dry tropical forest was studied. This patient had in his right foot two tumor-like growths localized, one in the anterior and superior part of the foot, the other near the malleoli. From the first tumor, light-colored grains were obtained, and Actinomadura madurae was cultured. From the tumor near the malleoli only black grains were isolated from which a Pyrenochaeta sp. was cultured.
