Bilateral Shoulder Dislocation with Fracture of Major Tubercles - A Case Report.

INTRODUCTION: Bilateral dislocations of the glenohumeral joint are most often posterior, secondary to convulsive seizures. We report a case of bilateral anterior dislocation with fracture of the two major tubercles following a very rare mechanism. CASE REPORT: A 52-year-old patient admitted at the emergency department for bilateral closed shoulder trauma following a tonic-clonic seizure. The patient had a blood pressure of 160/100 mm Hg and a clear consciousness. Symmetrical and bilateral clinical examination of the shoulders shows the signs of bilateral anterior dislocation of the shoulders.

Unexpected intercostal fusion of total thoracic fracture dislocation with ankylosing spondylitis.

BACKGROUND: Fractures associated with ankylosing spondylitis (AS) are often challenging due to long-segment rigidity and severe displacement. Here, a 50-year-old male presented with a total T10 fracture/dislocation. After posterior fusion instrumentation was removed due to wound dehiscence, spontaneous intercostal fusion between the T10 and T11 ribs provided sufficient stability. CASE DESCRIPTION: A 50-year-old male with AS after sustaining a severe T10 fracture/dislocation underwent an emergency posterior fusion. At surgery, the dural sac was ruptured and the spinal cord was completely trans

Open total talus dislocation without concomitant malleoli fracture: a case report.

BACKGROUND: Total talus dislocation without ankle (malleoli) fracture is a very rare injury with prevalence of only 0.06% of all dislocations and only 2% of talar injuries, and are usually associated with common complications such as infection, avascular necrosis, and posttraumatic arthritis. The treatment usually involves debridement, reduction, stabilization of the ankle joint, and primary or secondary closure of the wound. CASE PRESENTATION: We present the case of a 40-year-old South Asian woman who was involved in an accident. She was rushed to our hospital, whereby subsequent examination

Dorsomedial fracture dislocation of the tarsal navicular: A rare orthopaedic injury.

Dorsomedial fracture dislocations of the tarsal navicular are extremely rare injuries which commonly occur secondary to high energy, complex multidirectional forces involving an abduction force to a pronated and plantar-flexed foot. A 44-year-old female presented in the emergency room following a motor vehicle accident with a dorsomedial fracture dislocation of her navicular. Management required acute fusion of the talonavicular joint. There are few reports in the literature discussing the treating, outcomes, and complications of these injuries. Our report represents one method of treating the…

Trans-olecranon fracture posterior dislocation: a novel type of elbow injury.

BACKGROUND: Based on our experiences, we found that a kind of elbow injury is characterized by an olecranon fracture accompanied by elbow joint posterior dislocation with the proximal radioulnar joint intact. The aim of this study was to better define this kind of severe elbow instability, which has not been previously reported. METHODS: We retrospectively analyzed all patients with olecranon fractures who were treated at our institution from January 2013 to April 2021. Data on these patient demographics, injury characteristics, preoperative and postoperative imaging, surgical management, and

Retching as an unusual cause of bilateral posterior shoulder fracture dislocation.

Bilateral posterior shoulder fracture dislocation is a debilitating injury and is quite rare. This injury pattern has been described in the literature as either secondary to seizure, electrocution or major trauma. We present a case of an elderly man who appears to have sustained the injury after an episode of retching and vomiting. With our patient witnessed closely throughout and no trauma, electrocution or seizure activity reported, we propose that the violent action of retching and vomiting not only has the potential to cause this injury pattern but also was the only credible cause in our p…

Talus Fracture Accompanied by Irreducible Dislocation of Talar Trochlea - Treatment Strategy.

A talus fracture accompanied by trochlear dislocation is an ever-present therapeutic problem as it requires immediate surgery since any delay may result in necrosis of soft tissues and the talus, which, consequently, leads to permanent disability. Of crucial importance in the treatment of such injury is accurate analysis of fracture morphology, immediate institution of treatment and minimally invasive surgery allowing early movement of the affected limb. We operated on a male patient after a high-energy injury using a low-invasive method we had modified, which allowed for early kinesiotherapy.

Clavicle Shaft Fracture After Surgery for Bipolar Dislocation of the Clavicle.

BACKGROUND Bipolar dislocation of the clavicle is a rare disease that is often associated with some high-energy injuries. It refers to concomitant dislocation of the ipsilateral acromioclavicular joint and sternoclavicular joint. Because of its rarity, the diagnosis of bipolar dislocation of the clavicle is often difficult. Additionally, few reports are available on its treatment. Here, we describe a case of bipolar dislocation of the clavicle in which a secondary operation was needed because of a missed diagnosis. However, after surgery for bipolar dislocation of the clavicle, the clavicle sh

Autostabilization of neglected high-grade fracture-dislocation in the cervical spine.

Neglected cervical spine trauma is rare with very few case series reported in literature. There are no clear guidelines for its treatment. If operative treatment is sought, the role of skull traction, type of approach (anterior only, posterior only, or combined), sequence of surgery, type of instrumentation, etc., have to be considered. Hence, the treatment is challenging. Autostabilization has been described in the natural history of degenerative lumbar spondylolisthesis. As a result, many patients are treated nonoperatively, obviating need of surgery. We report two cases of neglected/untreat

Open Dislocation of Ankle without Fracture Treated with an External Fixator.

Ankle dislocations are orthopedic emergencies that require immediate treatment to avoid neurovascular impairment. They are usually accompanied by one or more comminuted fractures of the ankle mortis. In rare circumstances, such as high-energy trauma, the ankle dislocations may not be accompanied by concomitant malleolar fractures and, thus, are named "pure ankle dislocations". We presented a very rare and interesting case of an open medial dislocation of the ankle without associated fracture in an 18-year-old man with no known predisposing risk factors. The patient was admitted to the emergenc

Biomechanics of Internal Fixation Modalities for Middle Phalangeal Base Fracture Dislocation.

BACKGROUND: Internal fixation modalities of unstable (>50 percent articular involvement) middle phalangeal volar lip fracture-dislocations include interfragmentary screw and volar buttress plating. This study investigates the mechanical properties (yield strength, ultimate tensile strength, and stiffness) of interfragmentary screw (IS), simple buttress plating (BP) and buttress plating with subchondral screw (BP+S). METHODS: Fifteen cadaveric digits (5 index, 5 middle, and 5 ring) were prepared by excising its skin envelope and flexor tendons while preserving the structures around the proxima

Anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process.

Anterior subtalar dislocation is a very rare injury. We report a case of an 81-year-old woman who had her right foot injured during a motor vehicle accident. Radiographs showed anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process. The dislocation was closely reduced and protected by a short leg cast. One year postinjury, the patient had only mild pain when walking on uneven ground. There was mild tenderness over the lateral heel. Subtalar motion was mildly painful. There was no pain with ankle motion.

Manubriosternal dislocation with spinal fracture: A rare cause for delayed haemothorax.

Type 2 manubriosternal dislocations with concomitant spinal fracture are rare and may be associated with thoracic visceral injuries. The complication of delayed haemothorax has not been reported yet. We report a case of a young male who suffered manubriosternal dislocation with chance type thoracic spine fracture due to fall of a tree branch over his back. The haemothorax presented late on day three. The possible injury mechanism is discussed along with review of literature. We conclude that a lateral chest radiograph is indicated in spinal fracture patients complaining of midsternal pain. Com

Radial head reconstruction in elbow fracture-dislocation: monopolar or bipolar prosthesis?

BACKGROUND: Monopolar and bipolar radial head prosthetic arthroplasties have been used successfully to treat elbow fracture-dislocation with unsalvageable radial head fractures. The relative stability of these two designs in different clinical situations is a topic of ongoing investigation. QUESTIONS/PURPOSES: We tested the effects of monopolar and bipolar fixed-neck prosthetic radial head implants on improvement in elbow coronal and axial plane laxity in a terrible triad biomechanical model that accounted for lateral collateral ligament integrity and the presence of a transverse coronoid fra
