Comparative finite element analysis of fixation strategies for sports-related tibial tuberosity avulsion fractures in adolescents: biomechanical evidence for optimized clinical decision-making.

OBJECTIVE: This finite element study aimed to biomechanically compare screw fixation vs. K-wire fixation for adolescent tibial tubercle avulsion fractures (TTAF), investigating the differential biomechanical outcomes and physeal insult associated with each fixation method. METHODS: A finite element model of an adolescent tibia with a Modified Ogden type III fracture was developed. Three fixation types-(A) screws, (B) two non-crossing K-wires, and (C) four crossing K-wires-were tested both as standalone constructs and in their tension band-augmented counterparts. Simulations applied a 1,654 N

Short leg cast versus orthotic removable support for the management of pseudo-Jones avulsion fracture: A systematic review and meta-analysis.

BACKGROUND: The fifth metatarsal base avulsion fracture (i.e., Pseudo-Jones fracture) is one of the most common foot fractures. The management of pseudo-Jones fractures could be carried out surgically or conservatively. This systematic review and meta-analysis aimed to provide an update about the efficacy of orthotic removable support compared to short-leg casting for individuals with pseudo-Jones fracture. METHODS: We searched Embase, Medline, and Cochrane Central register of Controlled Trials (CENTRAL) for randomized controlled trials (RCTs) that compared the clinical outcomes of orthotic r

Avulsion Fractures at the Base of the 2(nd) Metacarpal Due to the Extensor Carpi Radialis Longus Tendon: A Case Report and Review of the Literature.

INTRODUCTION: Simultaneous contraction of the extensor carpi radialis longus (ECRL) with forced hyperflexion of the wrist can result in avulsion of the tendon and its bony attachment at its insertion at the dorsum of the base of the second metacarpal. This is a rare and often unreported fracture pattern. CASE PRESENTATION: We present a 31- year- old male who sustained a hyperflexion injury. He was managed surgically and had good post-operative outcomes. A literature search revealed 16 papers covering 18 cases of similar injuries. 12 were initially managed surgically and 6 were managed conserv

Avulsion fracture of the anterior iliac crest after bone graft harvest: case report and review of techniques, risk factors and treatment.

The anterior iliac crest is the harvest site preferred by many surgeons because of the quantity and quality of bone obtainable and the simplicity of harvesting techniques. Avulsion fracture of the iliac crest following bone grafting is an extremely rare occurrence. We present a case report of avulsion fracture of the anterior iliac crest following bone graft harvesting for anterior cervical fusion in a 63-year-old man. Non-operative treatment was the method of our treatment in the patient. By means of the presented case, iliac crest bone grafting techniques, risk factors of avulsion fracture,

A Distinct Posterior Cruciate Ligament Avulsion Fracture Involving the Weight-bearing Lateral Tibial Plateau as a Single Fragment: A Case Report.

Posterior cruciate ligament (PCL) avulsion fractures usually involve the tibial insertion as a discrete fragment. Although combined PCL avulsion and tibial plateau fractures have been described, a continuous fracture fragment involving both the PCL insertion and the weight-bearing lateral tibial plateau is rarely reported and may require different treatment considerations. We report a woman, aged 38 years, with a neglected PCL avulsion fracture involving the weight-bearing region of the lateral tibial plateau as a single, continuous fragment. This morphology created a specific surgical dilemma

Osteosynthesis using a plate lacking the anterior component and canulated cancellous screws to treat an avulsion fracture of the calcaneal tuberosity: A case report.

INTRODUCTION: Avulsion fractures of the calcaneal tuberosity with significant displacement can cause soft tissue complications in the heel. However, a treatment strategy for calcaneal tuberosity fractures with poor heel-skin condition is yet to be established. Here, we report a case involving avulsion fracture of the calcaneal tuberosity presenting with superficial skin necrosis that was treated with a plate lacking the anterior component and screws inserted percutaneously. PRESENTATION OF CASE: A 74-year-old woman presented with progressive right heel pain following an injury to her heel sus

Avulsion fracture of the supinator crest as an indication for a sustained posterolateral (sub)luxation of the elbow. A case report, anatomical evaluation and review of the literature.

The treatment of elbow injuries can be challenging because of the complexity of both anatomy and pathology. We present a rare traumatic avulsion fracture of the supinator crest of the ulna in a 37-year-old patient. Conservative treatment in a long arm cast for four weeks led to satisfactory results. Reproduction of the fracture on a cadaveric elbow clarified that the avulsed fragment holds the insertion of the lateral ulnar collateral ligament (LUCL). The mechanism of trauma that causes this fracture is a posterolateral (sub) luxation of the elbow, which usually causes the LUCL to rupture, but

Effects of microendoscopy-assisted reduction and screw fixation through a single mini-incision on posterior cruciate ligament tibial avulsion fracture.

INTRODUCTION: There are various surgical approaches for the treatment of posterior cruciate ligament (PCL) injury-associated tibial fracture avulsion, including arthroscopy-assisted surgery and open posterior surgery. However, none of these treatments are perfect. We have established a simple procedure with microendoscopy-assisted reduction and cannulated screw fixation for the treatment of this disease through a single mini-incision. In this study, we delineated the effects of this surgical approach for patients with PCL tibial avulsion fracture. PATIENTS AND METHODS: We retrospectively revi
