The first-pass effect of furosemide was investigated in rats. Furosemide intravenous solution (20 mg kg(-1) Lasix), was administered via the jugular vein and the portal vein, orally, and instilled directly into the duodenum of rats. The first-pass effects of furosemide by lung, heart, and liver seemed to be negligible in rats. The absolute bioavailability of furosemide was 28.9 and 48.3% after oral and intraduodenal administration, respectively. Sorbitol, a polyalcohol sugar, is the sweetener in most "sugar-free" products and may produce an osmotic diarrhea if ingested in large amounts (20-50 g). Whether or not smaller amounts of ingested sorbitol may be associated with other symptoms characteristic of carbohydrate malabsorption has not been determined. A modified regimen of oral enteric-coated budesonide (EC-BUD), in combination with other antidiarrheal and anti-inflammatory therapies, was prescribed. The authors report the development of a rapidly progressive encephalopathy marked by confusion, myoclonus, seizures, coma, and death in a group of women with renal failure who received an oral solution of citrate and aluminum hydroxide gel concurrently. Two patients were documented as having marked hyperaluminemia far exceeding blood aluminum levels encountered in the chronic state of aluminum intoxication. We ascribe the toxicity to enhanced gastrointestinal absorption of aluminum when complexed with citrate.

The antimycobacterial activity of rifaximin was studied in the guinea pig, after oral administration. The experiment confirmed the scanty absorption of rifaximin in the gastrointestinal tract, as the two dosages used (60 and 30 mg/kg) did not modify the tubercular illness after four months of therapy. Furthermore, sensitivity to rifaximin of the callenging Mycobacterium tuberculosis isolated at the end of the study from the spleen, lung, liver and lymph nodes of the guinea pigs that received rifaximin did not change.

Isolated copper deficiency occurred in a 57-yr-old man who had ingested massive quantities of zinc for more than 18 months. The patient presented with microcytic anemia and had normal gastrointestinal function with no evidence of occult blood loss. Dietary zinc in large doses leads to copper deficiency by antagonizing copper absorption. With discontinuance of zinc, the anemia and hypocupremia promptly corrected. The relationship between zinc, copper deficiency, and iron metabolism is reviewed.

Eleven patients suffering from arthritis received, in a randomized cross-over study, anti-inflammatory doses of two aspirin formulations: Enpryn, capsules containing enteric-coated pellets; Rhusal, an enteric-coated tablet. No significant difference was found between the two formulations with respect to gastrointestinal microbleeding, plasma salicylate levels and urinary recovery of salicylate. Bioavailability studies carried out on 10 healthy male volunteers demonstrated that absorption from the enteric-coated pellet capsules was sustained and complete.
