BACKGROUND: The intratympanic application of ototoxic antibiotics and saccotomia have been well documented in severe cases of one-sided Menière's disease for 30 years now. Both treatment methods are still subject to controversial discussion.
PATIENTS AND MATERIAL: This study includes 8 patients suffering from persistent one-sided Menière's disease after treatment with gentamicin application and additional saccotomia (n=2). We performed follow-up visits including the assessment of hearing ability, vestibular function and cranial MRI, 2-10 years after onset of treatment.

We present results at 2 years follow-up of a group of 15 patients with Ménière's disease treated with a low concentration of intratympanic gentamicin (IG group), compared with a group of 15 patients who refused any surgical treatment (NH group). The susceptibility of enterobacteria to gentamicin was studied in the County of North Jutland during 1993-1998. A total of 378 patients had a first-time gentamicin-resistant isolate. The annual number rose from 34 patients in 1993 to 89 in 1998. The prevalence of resistance per 1000 patients with isolates examined was 13.5 for Escherichia coli, 15.8 for Klebsiella pneumoniae, 26.1 for Klebsiella oxytoca, and 150.4 for Citrobacter freundii. Prophylactic antibiotic therapy for radical hysterectomy is still controversial. Although the efficacy of antibiotics have been demonstrated, there remains the question of duration of administration. In this study, we retrospectively reviewed 95 patients who underwent radical hysterectomy and pelvic lymphadenectomy for cervical cancer at our institute. A cataract developed in a 23-year-old man 4 months after a vitrectomy to repair a traumatic retinal detachment. An uneventful phacoemulsification was performed, using antibiotic-fortified infusion solution. A retrospective study in coronary artery bypass graft patients was undertaken to assess the effect of gentamicin and a bypass prime with a high calcium on the incidence of renal failure. Patients who received both Haemaccel (polygeline, Hoechst Marion Roussel) (calcium concentration 6.25 mmol/l) in the bypass prime and gentamicin perioperatively had a higher incidence of renal failure compared with those who received only Haemaccel (P = 0.005), only gentamicin (P = 0.002) or neither (P = 0.0001). We addressed the concern whether retinal toxicity could occur with subconjunctival gentamicin injections after cataract operations or traumatic corneoscleral lacerations. The lacerations were created along the superior limbus of rabbit eyes and the lenses removed. We sutured the right eyes to mimic a cataract operation and the left eyes remained unsutured to mimic a traumatic corneoscleral laceration. Vitreous humor samples were obtained at 1, 2, 4, 8 and 12 hours after the subconjunctival injection, but in no case did vitreal concentration approach retinotoxic levels (> 133 micrograms/ml).

Dermatophytes are usually isolated from house dust by using actidione-chloramphenicol Sabouraud glucose medium (ACS medium). Rats with untreated diabetes mellitus are protected from gentamicin-induced nephrotoxicity.
