Pulmonary involvement is considered to be rare in giant cell arteritis (GCA), usually occurring in the course of the disease. We describe the case of a patient who developed left pleural effusion revealing GCA. Thoracic CT scan demonstrated an abundant left pleural effusion and a thickening of the aortic wall. The patient's condition improved rapidly, with regression of pulmonary clinical features and complete clearance of pleural effusion, after institution of steroid therapy. Our case report reinforces the possibility of unusual presentation of GCA; such a diagnosis should, therefore, be con

We report a case of giant-cell granuloma located in the maxilla of a 9-year-old child. The pathogenesis of giant-cell reparative granuloma (GCRG) remains obscure. Radiological features are variable. Radiolucency or well or ill-defined uni-or multilocular aspects with trabeculations can be observed. Symptoms of compression on the adjacent tooth's root and pathology findings confirm the diagnosis. Curettage is effective and recurrence is rare.

A very rare case of the liver lesion characterized by formation of multinucleated giant hepatocytes with inflammatory cell infiltration were observed in two young (1.5 years and 2 years old) cats bearing thymic malignant lymphoma. Histopathological features of this liver lesion were very similar to giant cell hepatitis (GCH) in human neonates and infants. Therefore, the lesion was diagnosed as feline GCH.

Giant cell reparative granuloma (GCRG) is an uncommon non-neoplastic lesion that typically occurs in the mandible and maxilla: however, its involvement with the temporal bone is rare. It is usually misdiagnosed as a giant cell tumor. Although regarded as a benign process, GCRG may be locally aggressive. In this paper, we describe two cases of GCRG of the temporal bone and review the pertinent literature published in English. The clinical course, histological evaluation, diagnosis, differential diagnosis, treatment and prognosis of GCRG of the temporal bone were investigated.

BACKGROUND: Giant cell myocarditis causes essentially irreversible fulminant left ventricular dysfunction with associated conduction abnormalities and congestive failure. Response to immunosuppressive therapy is poor and cardiac transplantation is the only viable treatment option. The histologic hallmarks of giant cell myocarditis include a polymorphous inflammatory response with numerous multinucleated giant cells and extensive myocyte necrosis in a geographic pattern. There were 38 patients who received a cardiac transplant for giant cell myocarditis in the Giant Cell Myocarditis Registry. A…

This case reports a central giant cell granuloma, an uncommon but potentially troublesome idiopathic benign lesion. Its radiographic similarity to a routine inflammatory periradicular lesion led to not only delay in diagnosis, but also further delay in successful treatment.

BACKGROUND: Giant cell tumor of the small bones of the hand and foot is suspected of having some peculiar features compared with giant cell tumor in other sites. Moreover, it could share some features with other giant cell rich lesions involving the hand and foot, and this may affect the differential diagnosis. The aim of this study was to analyze the features of lesions such as these in the files of the Rizzoli Orthopedic Institute.
METHODS: The incidence of giant cell tumors of the bones of the hand and foot seen at the Rizzoli Orthopedic Institute over 50 years (1947-1997) was taken into c

The light-microscopic, electron-microscopic and immunocytochemical characteristics of giant cell arteritis (GCA) have been investigated in a number of studies on temporal arteries. Arterial atrophy and calcification of the internal elastic membrane appear to be prerequisites for the evolution of the inflammatory process. Foreign body giant cells form close to calcifications, apparently without connection with other inflammatory cells and probably by the fusion of modified vascular smooth muscle cells. The foreign body giant cells attack the calcifications. Lymphocytes accumulate around them an…

Ultrasonography of the temporal arteries detects characteristic signs of vasculitis with a high sensitivity and specificity: a hypoechoic halo due to an oedema of the artery wall and stenoses. The use of modern scanners with high resolution is essential. A halo has to be displayed in two planes. It is always circumferential. The colour intensity must not be too strong, otherwise the colour may cover the halo. A hypoechoic halo is also seen in large vessel giant cell arteritis at the axillary and brachial arteries and at the temporal arteries in polyarteritis nodosa.

A 69-year-old man presented with a complaint of pollakisuria, dysuria and anal pain. Digital rectal examination, transrectal ultrasonography and tumor marker of the prostate indicated a giant benign prostate hypertrophy (BPH), but urethrocystogram and magnetic resonance imaging (MRI) of the prostate was not typical of BPH. A diagnosis of giant BPH was made by transrectal needle biopsy of the prostate. However, symptoms of dysuria and anal pain became severe and then a catheter was indwelt into his bladder. A biopsy performed, 3 months later revealed transitional cell carcinoma in the specimens

The authors present a brief review of giant cell tumor of tendon sheath and three case reports. A discussion emphasizing the histologic characteristics of this lesion demonstrates the benign-to-malignant variability of these neoplastic growths. Special attention is directed to a case with aggressive histologic characteristics. Reexcision after surgery should be considered in cases where microscopic examination reveals a lesion with characteristics suggestive of potentially aggressive behavior.

An otherwise healthy, 9-year-old boy had gradual onset of a mass deformity in the region of the left medial canthus with resulting superior and lateral globe displacement. Radiographic evaluation demonstrated an osteolytic, expansile lesion involving the bones of the ethmoid and maxillary sinuses. Combined anterior orbitotomy and nasal endoscopic surgical resection revealed additional involvement of the nasolacrimal sac wall. Histopathology was consistent with a giant cell reparative granuloma. The authors know of 15 case reports of sino-orbital giant cell reparative granulomas with ophthalmic

The case of a giant-cell epulis as an initial feature of primary hyperparathyroidism is presented. A 58-year-old woman appeared for dental treatment with a buccal alveolar swelling that was confirmed by means of biopsies to be a multinucleated giant-cell lesion. High calcium levels warranted investigation for a parathyroid adenoma, which was located in an ectopic position and surgically removed. This case delineates the importance of referring patients with oral lesions of doubtful cause to specialists for further diagnostic workup.

Giant cell arteritis is an inflammatory disease that can affect the arteries anywhere in the body. Two cases are reported in which the arteries of the lower limbs were involved. Intermittent claudication with a walking distance of only 30 m was the inaugural manifestation in both cases. A biopsy of the superficial femoral artery provided the diagnosis in the first case. Ergotamine toxicity was considered initially in the second case. Acute ischemia and gangrene requiring amputation can complicate giant cell arteritis of the lower limbs and consequently corticosteroid therapy in an effective do

The autopsy findings of a 74-year-old women who unexpectedly died are presented. She had suffered from polymyalgia but typical symptoms of temporal arteritis had been absent and tentative treatment with prednisone had not improved the polymyalgic pain. The autopsy revealed a generalized giant cell arteritis involving the left temporal artery, the left common carotid artery, the ascending aorta, a submucosal artery of the ileum and the left anterior descending branch of the coronary arteries. Histology showed typical granulomatous infiltrates including giant cells, followed by thrombotical occl…

Subependymal giant cell astrocytoma (SEGA) is the most common neoplastic process involving the brain in patients with tuberous sclerosis complex (TSC). Morphologically, these tumors exhibit a wide range of cytoarchitecture with spindle and epithelioid cells resembling astrocytes, and also large, occasionally giant cells, some of which have a distinctly ganglion-like appearance. Unresolved questions regarding SEGAs center on: (a) their cytogenesis, i.e., whether they are derived from single or multiple precursors; and (b) their differentiating capacity along glial or neuronal lines. We sought t…
