A report is presented of the course of a case of the myelodysplastic syndrome which transformed after 16 months into Philadelphia chromosome-negative chronic granulocytic leukaemia. Such a transformation into the myeloproliferative syndrome is considered to be a rare event and some views on the mechanism of this metamorphosis are discussed.

Although human endothelial cells (EC) and granulocytes interact in several ways, the factors that regulate such interactions are not well defined. In this study we found that EC and their products directly altered granulocyte adherence (GA) and chemotactic activity. The spontaneous adherence of granulocytes to human umbilical vein EC monolayers was significantly reduced at 5, 15 and 30 min if the granulocytes were preincubated with EC that were stimulated by rocking. At 30 min the spontaneous adherence of EC-preincubated granulocytes was 36% of that of control granulocytes (P = 0.004). The aug…

A case of granulocytic sarcoma of the hard palate in an elderly woman is presented. The diagnosis was suspected on the basis of histologic findings in routine tissue sections and confirmed by cytochemical and electron microscopic studies. No systemic evidence of myeloid leukemia or of any other myeloproliferative disorder was documented in the patient, who died of an unrelated cause shortly after diagnosis.

Colony formation by haematopoietic progenitors from the bone marrow was studied in 44 patients with a myelodysplastic syndrome. Erythroid progenitors BFU-E and CFU-E were cultured in methyl cellulose, and granulocyte-macrophage precursors CFU-GM in agar. 3 of 32 patients showed normal numbers of BFU-E colonies; in all the other cases the number of these colonies was below the normal range. CFU-E colony formation was subnormal in all cases. 23 of 44 patients grew normal numbers of colonies and clusters in CFU-GM cultures. These patients had refractory anaemia with ring sideroblasts (FAB-classif

Granulocytic sarcoma (chloroma) is a localized tumefaction of immature granulocytes that is typically seen in association with myelogenous leukemia. The primitive cell population seen in biopsy material may be misinterpreted as histiocytic lymphoma or other sarcoma unless additional studies are performed. We saw a 36-year-old woman with promyelocytic leukemia in remission who had the signs and symptoms of an acute coalescent mastoiditis. Histologic examination of the surgical specimen, however, demonstrated a granulocytic sarcoma. Our case exemplifies some of the difficulties that may be encou…

Granulocyte transfusions (n = 222) were administered to 41 patients with granulocytopenia (less than 500 X 10(6)/1) and antibiotics-resistant infections. The average granulocyte content per transfusion was 2.1 X 10(10), 212 transfusions had been obtained by filter leucapheresis. Infections improved in 25 episodes, and 18 patients survived the 30th day. There was no influence in 18 episodes and only 2 patients survived beyond the 30th day. Extent and duration of bone marrow insufficiency, and type and grade of infection, clearly influenced prognosis. Further prognostically relevant risk factors

Neutropenia may be caused by neutrophil autoantibodies. The detection of such antibodies has always been difficult. Recently, we developed a sensitive indirect immunofluorescence technique applicable to granulocytes which proved to be of value in the detection of granulocyte alloantibodies. We have now used this method to investigate the serum and cells of 29 patients with idiopathic or secondary neutropenia. In four patients neutrophil-antigen-specific autoantibodies were detected in the serum and the patient's own granulocytes showed direct fluorescence. Furthermore, differences in the immun…

The first case of interdonor incompatibility associated with granulocyte transfusion is presented. The patient received Kell positive granulocyte transfusions containing about 30 ml of red cells 36 and 132 h prior to receiving a granulocyte transfusion containing anti-Kell 1/128. The chills, fever, vomiting and hypotension resulting from the red cell incompatibility, cleared with appropriate fluid therapy. Antibody detection methods must be applied to each unit of granulocytes. The patients blood should be tested with reagent grade antibody to detect small numbers of antigen-containing cells i…

A pilot study of granulocyte support for neutropenic infected patients by means of white cells collected from the Aminco Celltrifuge at the Peter MacCallum Clinic showed acceptable side effects in donor and recipients, and an encouraging recovery occurred in two out of four patients treated. White cell filtration leukopheresis was introduced at the Alfred Hospital Haematology and Medical Oncology Unit in November, 1975, for supportive therapy of all patients with white cell counts below 500/mm3 who had sustained febrile episodes (temperatures greater than 38 degrees C for more than 48 hours) w

Autologous plasma collected either during or immediately after continuous flow filtration leukapheresis (CFFL) was infused prior to subsequent donations in an attempt to induce granulocytosis and to augment granulocyte yields. Granulocytosis did not occur in the 15 to 20 minutes following plasma administration and no increase in granulocyte yields could be demonstrated. In addition, the plasma infusion did not affect the timing or magnitude of the transient granulocytopenia which characteristically occurs during CFFL. Although infusion of postfiltration leukapheresis plasma does not appear to

A randomized clinical trial to determine the efficacy of granulocyte transfusions in neutropenic patients with infection was conducted. Criteria for patient selection included a proved infection, a granulocyte count of less than 300/mm3, availability of a suitable donor and failure to respond to at least 72 horus of appropriate antibiotic therapy. Thirty patients were assigned at random to receive either granulocyte transfusions or to serve as a control group. Antibiotic therapy was continued in both groups. Responses were judged by the degree of diminution of infectious episodes and survival.

Twenty-three children with various stages and morphologic types of leukemia were treated with multiple granulocyte transfusions obtained by filtration leukapheresis when neutropenia-associated infection appeared unresponsive to antibiotics. All children meeting the above qualifications were given granulocyte transfusions during this time period. Twenty-one of 23 became afebrile during or shortly after the transfusions; one died with disseminated Herpes simplex; and one became well enough to be discharged, although he was never free of fever. Frequent mild to moderate fever and chills were note
