Clinical and cost-effectiveness of home-based cardiac rehabilitation compared to conventional, centre-based cardiac rehabilitation: Results of the FIT@Home study.

Aim Although cardiac rehabilitation improves physical fitness after a cardiac event, many eligible patients do not participate in cardiac rehabilitation and the beneficial effects of cardiac rehabilitation are often not maintained over time. Home-based training with telemonitoring guidance could improve participation rates and enhance long-term effectiveness. Methods and results We randomised 90 low-to-moderate cardiac risk patients entering cardiac rehabilitation to three months of either home-based training with telemonitoring guidance or centre-based training. Although training adherence wa

Child Welfare System Involvement in the United States: 2016-2023.

BACKGROUND AND OBJECTIVES: Childhood and family contexts and understanding of communicable respiratory diseases shifted during and after the COVID-19 pandemic. Against this backdrop, competing hypotheses have emerged around children's risks of maltreatment and system contact, highlighting a need for greater understanding of trends in child welfare system involvement before, during, and since the COVID-19 pandemic. This study estimates the cumulative prevalence of several stages of child welfare contact for all US children and across racial and ethnic groups between 2016 and 2023. METHODS: We

Exploring success in sub-acute hospital-to-home transitions for palliative patients: a descriptive qualitative study.

BACKGROUND: Transitioning from a sub-acute hospital to home is highly desirable to some palliative and end-of-life individuals and their family caregivers, but the transition process itself is complex and can be logistically challenging. This formative assessment sought to better understand what constitutes a successful sub-acute hospital-to-home transition from the perspectives of health care providers connected to a sub-acute care facility in Ottawa, Canada, with reflections from a patient and a family caregiver. METHODS: Our descriptive qualitative study involved 13 virtual interviews and

Race and Ethnicity Are Related to Undesirable Home Health Care Outcomes in Seriously Ill Older Adults.

OBJECTIVES: Medicare Home Health Care (HHC) services are integral to the care of homebound seriously ill older adults requiring ongoing specialized medical care. Although disparities in health outcomes are well documented in inpatient and primary care, disparities experienced by historically marginalized racial and ethnic groups underrepresented in HHC are understudied. This study aimed to examine the relationship between individual characteristics and differences in HHC health outcomes for seriously ill older adults. DESIGN: Secondary data analysis, repeated measure. SETTING AND PARTICIPANT

A service evaluation comparing home-based testing to clinic-based testing for HIV, syphilis and hepatitis B in Birmingham and Solihull.

The sexual health services in the Birmingham and Solihull area of the United Kingdom, called Umbrella, has been offering home-based testing for sexually transmitted infections to patients since August 2015. The aim of this service evaluation was to evaluate the uptake, return rate and new diagnosis rates of home-based testing in comparison with clinic-based testing for human immunodeficiency virus (HIV), syphilis (STS) and hepatitis B. Home-based testing, although popular, had low uptake amongst high-risk groups such as men who have sex with men (MSM), compared to the clinic-based group (1% ve

Structural and interagency influences on mandated health assessments for children in out-of-home care in Sweden: a qualitative study of child protection services workers.

BACKGROUND: Children in out-of-home care (OOHC) have significantly poorer physical, mental and dental health than their peers. In Sweden, child protection services (CPS) are required to initiate health assessments (HAs) when a child is placed in OOHC. Despite this statutory requirement, fewer than half of eligible children undergo an HA. Factors influencing implementation remain poorly understood. METHODS: This qualitative study explored CPS caseworkers'change experiences of initiating and coordinating HAs for children placed in OOHC in Sweden. Five semi-structured focus group discussions wer

A longitudinal study of active treatment of adaptive skills of individuals with profound mental retardation.

A longitudinal study indicated that a single group of adults living in a large state institution (O'Berry Center in Goldsboro, North Carolina) benefited from receiving active treatment, although no control group was permitted. After five years, 59 individuals with profound mental retardation improved their skills in six general areas of dressing, grooming, eating, toileting, communication, and social interaction. After five more years of active treatment, 51 persons who had remained in the program maintained their skills in dressing, grooming, eating, and communication. Significant improvement

How we live: participatory research with six people with learning difficulties.

Drawing from the Social Model of disability, six people with learning difficulties participated over 18 months in researching their own lived experiences. The method involved weekly group discussions supplemented by participant observation. The study's value stems from in-depth inquiry, which included the participants in the data collection, analysis, dissemination and consequences that they found meaningful. The participants lived in nurse managed community homes. They described how they had faced social and economic exclusion, for example, concerning marriage, child rearing and decently paid

Reliability of ratings of consumers with mental retardation and their staff on multiple measures of social support.

Reliability of self-reports of social support with staff ratings was compared through determining the internal consistency of the measures, consistency across measures, and consistency across raters. Fifty adults with mild mental retardation and their staff in supported living residential settings were interviewed. Self-report ratings had moderate internal consistency, were consistent across rating scales, and were significantly correlated with staff ratings, although staff members tended to agree more with each other than with consumers. Results suggest that individuals with mild mental retar
