A 22-year-old man received 4 cycles of bleomycin, etoposide and cisplatin combination chemotherapy for clinical stage IIA embryonal cell carcinoma of the right testis. The treatment resulted in complete remission. Five months following cessation of the chemotherapy first on the left and then 2 weeks later on the right side painful gynecomastia developed. The accumulation of 99mTc(V)-DMSA in bilateral breasts was recognized in a 60-year-old male with drug-induced gynecomastia. There are some reports describing 99mTc(V)-DMSA accumulation in normal female breast, fibrous dysplasia of the breast and metastatic lesions of breast carcinoma, but to my knowledge, there have been no reports describing 99mTc(V)-DMSA accumulation in gynecomastia.

We report 22 cases of prepubertal gynecomastia diagnosed during growth hormone (GH) treatment. The age and dose range were 2 to 12 years and 0.18 [corrected] to 0.3 mg/kg per week, respectively. In most of the patients, gynecomastia appeared between 0.5 and 7 months after GH was started. Three boys were using drugs other than GH. This condition appears to be self-limited and benign.

Gynecomastia and bloody nipple discharge are very rare in childhood. In this report, a case of infantile gynecomastia together with bloody nipple discharge is presented. A hemorrhagic discharge was expressible from the mammary gland. Endocrinologic findings were within the normal limits. Because of persisting bleeding, a subcutaneous mastectomy was performed. The specimen was reported as gynecomastia. This boy is the first reported prepubertal gynecomastia with bloody nipple discharge in the medical literature.

We have evaluated the efficacy of the antiestrogen tamoxifen in six men with painful idiopathic gynecomastia. Subjects were given either tamoxifen or placebo for 2 to 4 months and then were given the other agent for an identical period. Breast size was considered to have been reduced only if it had decreased by one or more Marshall-Tanner stages during the treatment period. A group of 78 patients with gynecomastia was observed between 1974-1987. Idiopathic gynecomastia was referred in 33 cases, cirrhosis in 31 cases, prostatic hypertrophy in 2 cases, while in 12 cases gynecomastia was induced by drugs. All patients with idiopathic form and 11 patients with secondary form were surgically treated by mastectomy (20 bilaterally and 24 unilaterally). The authors emphasize that, in this pathology, is much more important restoring the masculine feature of thorax without esthetic damage than removing the hypertrophic gland.

Immunohistochemical estrogen receptor (ER) analysis using a monoclonal ER antibody was performed on a total of 30 specimens from 26 patients with gynecomastia. Twenty-six specimens were ER-positive, 3 were ER-negative, and the analysis failed in one case. Immunohistochemical analysis proved more sensitive than biochemical ER analysis in a group of 20 patients, and in a group of 6 patients, the immunohistochemical analysis correlated well with in vitro hormonal sensitivity of the gynecomastic tissue. Twenty-three boys with pubertal gynecomastia had higher mean levels of prolactin than boys of similar development but without gynecomastia (10.1 vs. 6.2 ng/ml). In girls of comparable pubertal staging prolactin level was 12.3 ng/ml. In boys no hyperprolactinemia was found, not even after stimulation with TRH, prolactin levels were not elevated in two boys with secreting mammary glands. In casuistic form is reported on a patient with double-sided painful gynaecomasty and development of a node in the right breast under therapy with aldosterone antagonists. In this case the pathogenetic factors in the development of spironolactone-induced gynaecomasties are discussed. After a longer application of spironolactone must be reckoned with a gynaecomasty particularly when chronic liver diseases exist.
