Tuberculous Osteomyelitis in the Hand.

Tuberculous osteomyelitis is recognized as a rare form of extrapulmonary tuberculosis, particularly when involving the small bones of the hand in adults. Early diagnosis is challenging due to its uncommon presentation and nonspecific symptoms. These factors often contribute to the delayed identification and management of the condition. We report a 22-year-old male construction worker from Mexico who presented with swelling and pain localized to his right hand. One week prior to presentation, he experienced minor blunt trauma to the fifth metacarpal region when a piece of wood struck his hand,

[What does the hand surgeon expect from the radiologist in diagnostic imaging of the hand?].

BACKGROUND: For hand surgeons, in addition to anamnesis and clinical examination, imaging is the essential component in diagnostics and the basis for therapy planning. The focus on the special field of the "hand" and the adjacent joints is limited with regard to the anatomical extent, but with regard to the possible clinical pictures it encompasses the entire range of pathological changes. CLINICAL ISSUE: Which imaging techniques support the goal of maintaining the functionality of the hand? PERFORMANCE: The carpal bones can be represented by computer tomography without superimposition. The

The use of chimeric musculocutaneous posterior interosseous artery flaps for treatment of osteomyelitis and soft tissue defect in hand.

INTRODUCTION: There is growing evidence of the superior ability of muscular tissue to clear bacterial bone infection. Unfortunately, in the hand, there are almost no small local muscular flaps, and muscular transfers to the hand are mainly microsurgical free transfers. In this report, we present the results of the use of a chimeric posterior interosseous flap including part(s) of the forearm muscles to treat osteomyelitis and soft tissue defect of hand from a series of patients. PATIENTS AND METHODS: Four male patients with an average age of 32 years (range 20-46 years), were affected by acut

Familial progressive sinoatrial and atrioventricular conduction disease of adult onset with sudden death, dilated cardiomyopathy, and brachydactyly. A new type of heart-hand syndrome?

We identified a family with 10 affected members in four generations suffering from adult-onset progressive sinoatrial and atrioventricular conduction disease, sudden death due to ventricular tachyarrhythmia, dilated cardiomyopathy, and a unique type of brachydactyly with mild hand involvement (short distal, middle, proximal phalanges and clinodactyly) and more severe foot involvement (short distal, proximal phalanges and metatarsal bones, short or absent middle phalanges, terminal symphalangism, duplication of the bases of the second metatarsals, extra ossicles, and syndactyly). The phenotype

Aneurysmal bone cyst involving the lunate.

Aneurysmal bone cysts are rare lesions which even more rarely involve the bones of the hand. We report a case of a cyst involving the lunate which was initially mistaken for interosseous ganglion. Due to delay in diagnosis, marked destruction of the lunate occurred, necessitating excision and scapho-capitate arthrodesis. Although rare, aneurysmal bone cysts should be included in the differential diagnosis and ruled out prior to embarking on a course of treatment.

Acrofacial dysplasia resembling geleophysic dysplasia.

We report on a 12-year-old girl with acrofacial dysplasia, growth retardation, joint contractures, mitral valve incompetence and focal hepatic storage of material reacting histochemically as glycoprotein. The patient's phenotype resembles that of patients with geleophysic dysplasia but differs with respect to facial appearance, milder changes of hand bones and normal capital femoral epiphyses. It is undecided if her disorder is part of a wider phenotypic spectrum of geleophysic dysplasia or a different entity.

Staged Reconstruction for Dorsal Hand and Wrist Shear Injury: A Case Report.

INTRODUCTION: Dorsal hand shear injuries present complex challenges for surgeons in addressing soft tissue coverage, extensor tendon injuries, and bone loss. This case study examines the inherent challenges associated with the treatment of a severe injury to the dorsum of the hand, as well as the selected treatment approach. CASE REPORT: A 41-year-old male experienced a severe injury to the dorsum of his left hand following a road traffic accident. The injury involved skin loss from the wrist to the metacarpophalangeal joints, complete loss of the extensor digitorum communis tendons and exten

Internal fixation of fractures of both bones forearm: Comparison of locked compression and limited contact dynamic compression plate.

BACKGROUND: The locking compression plate (LCP) with combination holes is a newer device in fracture fixation. We undertook a study comparing the LCP with limited contact dynamic compression plate (LC-DCP) in the treatment of diaphyseal fractures of both bones of the forearm. MATERIALS AND METHODS: This is a prospective comparative study, 36 patients (18 in each group) with fractures of both the forearm bones (72 fractures) were treated with one of the two devices. The average age of the patients was 30.5 years (range 16-60 years) with mean followup of 2.1 years (range 1.5-2.8 years). The pat

Free metatarsophalangeal joint to metacarpophalangeal joint transfer - a unique surgical technique.

Giant cell tumors of metacarpal bones is a rare occurrence. The tumor particularly exhibits an aggressive behavior in small bones of the extremities. Different techniques such as curettage with or without bone grafting, en bloc resection (more commonly preferred) and reconstruction have been discussed but a unique way of reconstruction is using the metatarsal for substitution. Other options include the fibular graft or using a portion of the iliac crest. We report the case of a young female with a giant cell tumor of the fourth metacarpal bone where we reconstructed the diseased joint with the

[Hemophilic pseudotumors].

A review of the literature was made with special interest in morphology, pathogenesis and treatment of hemophilic pseudotumors. 110 reports of hemophilic pseudotumors were evaluated and compared with 25 own cases. The separation according to the inflicted site and the primarily involved anatomical structure revealed three types of hemophilic pseudotumors: 1. Intramuscular hematomas which enter secondarily into the periosteum. a) In the diaphyses of long tubular bones (femur 49, lower leg 14, humerus 4, forearm 2). b) In the iliac bone (37). 2. Congenital radio-ulnar synostosis.

Thirty-three patients with congenital radio-ulnar synostosis were examined. There was one familial predisposition. Chromosomal patterns were examined in seventeen patients and were normal. Bone maturation appeared normal. The radial deviation angle, in the patients whose distal radial epiphysis had closed, was elevated (Madelung's deformity). However, in the patients whose distal epiphysis maturation score was 8, it was not elevated. On the other hand, elongation of the ulna (plus variant) and/or dorsal dislocation of the distal end of the ulna was seen in early life. In terms of treatment, al

Skeletal age evaluation using hand X-rays to determine growth problems.

A common clinical method for identifying anomalies in bone growth in infants and newborns is skeletal age estimation with X-ray images. Children's bone abnormalities can result from several conditions including wounds, infections, or tumors. One of the most frequent reasons for bone issues is that most youngsters are affected by the slow displacement of bones caused by pressure applied to the growth plates as youngsters develop. The growth plate can be harmed by a lack of blood supply, separation from other parts of the bone, or slight misalignment. Problems with the growth plate prevent bones

Metacarpophalangeal analysis in Crouzon syndrome: additional evidence for phenotypic convergence with the acrocephalosyndactyly syndromes.

Crouzon syndrome (CS) is characterized by premature craniosynostosis, orbital proptosis, and midfacial hypoplasia and is related to the acrocephalosyndactylies (ACS) with limb abnormalities. In CS the hands are considered to be normal, but a previous report indicated that there is consistent alteration in the proportions of the bones of the hands, as shown by metacarpophalangeal analysis (MCPP), in at least some cases of CS. The purpose of this study was to extend these findings by examining affected individuals and their supposedly unaffected first degree relatives. If CS does have an effect

Ollier Disease: A Case Series and Literature Review.

SUMMARY BACKGROUND: Ollier disease is the most common nonhereditary type of enchondromatosis. Enchondromas are common, usually benign intraosseous cartilaginous tumors that form near the growth plate cartilage predominantly unilaterally in the metaphyses and diaphyses of tubular bones. They usually affect the long bones of the hand, the humerus, and the tibia, followed by flat bones, such as the pelvis. The estimated prevalence of Ollier disease is 1 in 100,000 and while it is linked with somatic heterozygous mutations in IDH1 or IDH2 genes, exact etiology is unknown. The risk of malignant tra

Age-related changes in densitometry and histomorphometry of long bone during the pubertal growth spurt in male rats.

Because experimental studies to determine the developmental toxicity of exposure to various substances in children are impossible, many studies use immature male rats. This study aimed to provide normative data for longitudinal bone growth with age during the puberty in male rats. In order to evaluate long bone growth and mineralization we examined bone size and bone density by dual-energy X-ray absorptiometry, analyzed histomorphometry of the growth plate, and serum hormone levels relevant to bone growth from postnatal day (PD)20 to PD60. The length and weight of long bones increased strongly

A patient with scleroderma associated with severe acro-osteolysis: A case report.

Systemic sclerosis is an autoimmune chronic sclerotic disease that can damage organs and cause serious complications for the patient such as musculoskeletal manifestations, Gastrointestinal involvement, pulmonary involvement, and renal disease. Acro-osteolysis is one of the musculoskeletal manifestations that causes corrosion of the bones in the fingertips of the hand and feet. In this paper, we have reported the rarely current evidence of severe Acro-osteolysis of the distal phalanges of the hands by radiological x-ray.

Ultrasonography for Hand and Wrist Conditions.

Ultrasonography facilitates dynamic, real-time evaluation of bones, joints, tendons, nerves, and vessels, making it an ideal imaging modality for hand and wrist conditions. Ultrasonography can depict masses and fluid collections, help locate radiolucent foreign bodies, characterize traumatic or overuse tendon or ligament pathology, and help evaluate compressive peripheral neuropathy and microvascular blood flow. Additionally, this modality improves the accuracy of therapeutic intra-articular or peritendinous injections and facilitates aspiration of fluid collections, such as ganglia.

[Histiocytosis X. 3 different cases: eosinophilic granuloma, Hand-Schueller-Christian disease, Abt-Letterer-Siwe disease].

The authors present three different cases of histiocytosis X: the eosinophilic granuloma, the Hand-Schuller-Christian syndrome and the Letterer-Siwe syndrome. As it appears in the main articles concerning this argument, the Aa. emphasize the common features belonging to the three different forms of histiocytosis: the involvement of the bones and of other tissues. Moreover the prognosis is defined as good in the first case, uncertain in the second case and severe in the third.

In vivo assessment of magnesium status in human body using accelerator-based neutron activation measurement of hands: a pilot study.

Magnesium (Mg) is an element essential for many enzymatic reactions in the human body. Various human and animal studies suggest that changes in Mg status are linked to diseases such as cardiac arrhythmia, coronary heart disease, hypertension, premenstrual syndrome, and diabetes mellitus. Thus, knowledge of Mg levels in the human body is needed. A direct measurement of human blood serum, which contains only 0.3% of the total body Mg, is generally used to infer information about the status of Mg in the body. However, in many clinical situations, Mg stored in large levels, for example in bones, m

Intramedullary Fixation of Hand Fractures and Arthrodeses.

Intramedullary fixation has long been popular for fracture fixation in the upper extremity from the shoulder through the carpus. Recently, intramedullary fixation in the hand has gained increasing interest, specifically in the metacarpals and phalanges, corresponding with the development of improved cannulated headless screw technology. Along with the advantages of increased operative speed and less surgical dissection, which can promote rapid healing, many benefits exist, supporting their use despite some drawbacks. This article reviews the background and biomechanics of intramedullary fixati
