PURPOSE OF REVIEW: This article reviews the approach to a child or adolescent with headache, the criteria for common diagnoses, and the evidence base for treatments.
RECENT FINDINGS: The guidelines for acute and preventive treatment of migraine were updated in 2019. These guidelines summarize the available evidence and outline the questions that should be addressed in future research. The US Food and Drug Administration (FDA) approval of several new classes of drugs and devices to treat adult migraine in the past few years has resulted in ongoing or planned pediatric trials.

Headache during pregnancy is frequent and challenging. A secondary headache is diagnosed in one third of women, and most causes are vascular and infectious disorders. Anamnestic and clinical features must be investigated, and brain imaging is often required. Primary headaches are commonly modified during pregnancy. Migraine is mainly improved in this context, especially migraine without aura. Particular cautions about treatment must be applied, and non-pharmacological measures must be preferred. Tension-type headache and cluster headache seem to be less hormone-dependent.

This article endeavours to revise the key guidance and evidence on temporomandibular disorders (TMD), with a particular focus on myofascial pain. It highlights the important role that primary care dental practitioners play in providing holistic care during the patient's journey to manage this painful condition. Headache is an increasingly common symptom among children and adolescents with a prevalence of 58.4%, and it may have profound impact on everyday life. A poorer prognosis is seen for children, who are not referred to specialist care, and long-term follow-up is shown to indicate a better outcome. A thorough headache history and a full neurological examination is vital to a correct diagnosis. The acute and prophylactic treatment is poorly studied, but acetaminophen and ibuprofen are first-line choices in acute treatment, and prophylactic treatment should be carried out by specialists.

TTH are very common in general population and may be under diagnosed. They are more common in patients suffering from migraine headaches. In general underlying psychiatric conditions, such as anxiety and depression, are prevalent in TTH sufferers. For acute therapy NSAIDs are the main treatment option.
