Rehabilitation after myocardial infarction.

The concept that many patients with asymptomatic coronary atherosclerotic heart disease, including myocardial infarction, can and should return toward normal living is the basis of the rehabilitation effort. The primary care physician is increasingly employing this approach in the care of patients with myocardial infarction. Much of the capacity to effect rehabilitation should be available in the community hospital, in the office of the primary physician, or through community, governmental, public, and voluntary health care agencies and facilities. Although some patients with complex diagnosti

How has Guinea learnt from the response to outbreaks? A learning health system analysis.

INTRODUCTION: Learning is a key attribute of a resilient health system and, therefore, is central to health system strengthening. The main objective of this study was to analyse how Guinea's health system has learnt from the response to outbreaks between 2014 and 2021. METHODS: We used a retrospective longitudinal single embedded case study design, applying the framework conceptualised by Sheikh and Abimbola for analysing learning health systems. Data were collected employing a mixed methods systematic review carried out in March 2022 and an online survey conducted in April 2022. RESULTS: Th

Opportunities and Challenges for PBPK Modelling in Specific Populations: Considerations for Paediatrics, Pregnancy, Lactation, Ethnic Differences, and Organ Impairment.

Physiologically based pharmacokinetic (PBPK) modelling represents a key component of model-informed drug development (MIDD), providing a mechanistic framework to predict drug disposition in populations often underrepresented in clinical research. By integrating drug-specific properties with detailed physiological and biochemical parameters, PBPK models bridge knowledge gaps created by ethical, logistical, and safety constraints, enabling evidence-based dosing and supporting more inclusive pharmacotherapy. This chapter examines the application of PBPK modelling across populations in which physio

Pregnancy and reproductive healthcare among adolescent girls in the child welfare system.

OBJECTIVE: This study examines the prevalence and predictors of first pregnancy and the use of long-acting reversible contraceptives (LARCs) among adolescent girls aged 12-17 involved in the child welfare system. METHODS: The study leverages linked longitudinal administrative data from the child welfare, juvenile legal, and Medicaid claims systems of Pennsylvania. The sample comprised 28,016 girls born between 2000 and 2005 who received Medicaid and were involved in the child welfare system during adolescence. Survival analysis was used to predict onset of first pregnancy and uptake of LARC.

Considerations from the pharmaceutical industry (IQ MPS Affiliate) workshop on animal microphysiological systems and 3Rs in drug development.

Most complex in vitro models (CIVM) and microphysiological systems (MPS) are composed of human cells, with the goal of evaluating diseases, efficacy, safety, and pharmacokinetic questions specifically for humans. The hope with CIVM/MPS is that they will eventually improve our predictivity of clinical responses and reduce or replace animal use in research, supporting the 3Rs concept of only using animals in research when necessary. Given the potential of animal-based models to advance this field by comparing existing in vivo animal data with new animal-based MPS responses, there are currently

National Task Force on Fetal Alcohol Syndrome and Fetal Alcohol Effect: defining the national agenda for fetal alcohol syndrome and other prenatal alcohol-related effects.

Prenatal alcohol exposure can lead to serious birth defects and developmental disabilities. A need exists to develop effective strategies for both children with fetal alcohol syndrome (FAS) or other prenatal alcohol-related effects and for women at high risk for having an alcohol-exposed pregnancy. Since the syndrome was identified approximately 30 years ago, advancements have been made in FAS diagnostics, surveillance, prevention, and intervention, but a substantial amount of work remains. Collaborations among partners in federal state, and local agencies, academia, clinical professions, scho

A pilot-wellness-informed ecological framework for human-autonomy teaming in future fighter aviation.

Future fighter operations are expected to combine increasingly capable autonomy, collaborative aircraft, dense sensor information, and sustained physiological and cognitive demands. This Perspective proposes a pilot-wellness-informed ecological framework in which the pilot, a pilot-specific digital twin, an intelligent flight-management system, the aircraft, and external agents are treated as interdependent elements within a systems-of-systems framework. The framework does not replace situation awareness, distributed cognition, joint cognitive systems, adaptive automation, or human-autonomy te

Incidence of waterborne lead in private drinking water systems in Virginia.

Although recent studies suggest contamination by bacteria and nitrate in private drinking water systems is of increasing concern, data describing contaminants associated with the corrosion of onsite plumbing are scarce. This study reports on the analysis of 2,146 samples submitted by private system homeowners. Almost 20% of first draw samples submitted contained lead concentrations above the United States Environmental Protection Agency action level of 15 μg/L, suggesting that corrosion may be a significant public health problem. Correlations between lead, copper, and zinc suggested brass comp

The healthcare system in Sweden.

The Swedish population is characterized by high life expectancy and low avoidable mortality rates. This review outlines the Swedish healthcare system, which offers universal access to all residents and has a long tradition of reforms for social equity. Responsibility for healthcare is shared between the state, the regions, and the municipalities. The Ministry of Health and Social Affairs provides the overall healthcare framework; additionally, several governmental agencies are directly involved in healthcare and public health initiatives. The 21 regions organize, finance, and provide most prim

A review of radiation countermeasures focusing on injury-specific medicinals and regulatory approval status: part I. Radiation sub-syndromes, animal models and FDA-approved countermeasures.

PURPOSE: The increasing global risk of nuclear and radiological accidents or attacks has driven renewed research interest in developing medical countermeasures to potentially injurious exposures to acute irradiation. Clinical symptoms and signs of a developing acute radiation injury, i.e. the acute radiation syndrome, are grouped into three sub-syndromes named after the dominant organ system affected, namely the hematopoietic, gastrointestinal, and neurovascular systems. The availability of safe and effective countermeasures against the above threats currently represents a significant unmet me

[The institutionalization of public health in Spain].

This article reviews the long and complex process of institutionalizing public health in Spain, from the mid-nineteenth century to the present, highlighting its persistent structural fragility despite the accumulation of regulatory advances. Beginning with the 1855 Health Act, conceived as a response to cholera epidemics, the paper describes the gradual assumption by the State of responsibilities for protecting collective health, in parallel with the late consolidation of the Spanish state itself. It shows how, throughout the twentieth century, public health was often subordinated to a view of

Creating integrated service systems for homeless persons with mental illness: the ACCESS Program. Access to Community Care and Effective Services and Supports.

The Access to Community Care and Effective Services and Supports (ACCESS) demonstration program was initiated in 1993 by the U.S. Department of Health and Human Services as part of a national agenda to end homelessness among persons with serious mental illness. Demonstration projects have been established in nine states to develop integrated systems of care for this population. This paper provides an overview of the ACCESS program and presents definitions of services integration and systems integration. Evaluating the effectiveness of integration strategies is a critical aspect of the program.

Re-thinking global health sector efforts for HIV and tuberculosis epidemic control: promoting integration of programme activities within a strengthened health system.

BACKGROUND: The global financial crisis threatens global health, particularly exacerbating diseases of inequality, e.g. HIV/AIDS, and diseases of poverty, e.g. tuberculosis. The aim of this paper is to reconsider established practices and policies for HIV and tuberculosis epidemic control, aiming at delivering better results and value for money. This may be achieved by promoting greater integration of HIV and tuberculosis control programme activities within a strengthened health system. DISCUSSION: HIV and tuberculosis share many similarities in terms of their disease burden and the recommend

Airborne infection control in India: Baseline assessment of health facilities.

BACKGROUND: Tuberculosis transmission in health care settings represents a major public health problem. In 2010, national airborne infection control (AIC) guidelines were adopted in India. These guidelines included specific policies for TB prevention and control in health care settings. However, the feasibility and effectiveness of these guidelines have not been assessed in routine practice. This study aimed to conduct baseline assessments of AIC policies and practices within a convenience sample of 35 health care settings across 3 states in India and to assess the level of implementation at e

Decision Framework for Prioritizing Systematic Reviews Versus Primary/translational Research in Low- and Middle-income Countries.

Systematic reviews and meta-analyses (SR/MAs) are widely regarded as the highest level of evidence and play a crucial role in informing clinical practice, public health policy, and guideline development. However, the rapid proliferation of evidence syntheses has raised concerns regarding research redundancy, publication-driven incentives, and the potential diversion of resources away from primary and translational research, particularly in low- and middle-income countries (LMICs). In settings characterized by large and diverse populations, evolving health systems, and substantial unmet health

Barriers and facilitators to antibiotic stewardship in Nigeria's private healthcare sector: A qualitative interview study with national health and regulatory interest holders.

Antimicrobial resistance (AMR) poses a growing global health concern, particularly in low- and middle-income countries (LMICs) such as Nigeria, where stewardship efforts have largely overlooked the private healthcare sector. Given that most Nigerians seek care from private providers, understanding the barriers and opportunities for implementing antibiotic stewardship programs in this setting is critical. We conducted a qualitative study consisting of semi-structured interviews with 14 interest holders from national health and regulatory organizations in Nigeria. Participants were purposively s
