Health, workforce, and cost trade-offs of HIV programme prioritization strategies in Malawi.

BACKGROUND: Malawi has achieved substantial progress towards HIV epidemic control, reaching the UNAIDS 95-95-95 targets. However, declining external financing and health-workforce constraints threaten sustainability. Programme reconfiguration is being considered to reduce resource use. METHODS: We used the Thanzi La Onse (TLO) individual-based health system simulation model to evaluate HIV programme changes in Malawi from 2025 to 2050. HIV testing, treatment monitoring, prevention interventions, and tuberculosis preventive therapy (TPT) were assessed individually and in combination, and compa

Stakeholder-Perceived Needs in Early Community Nursing Implementation: A Qualitative Study.

Background/Objectives: Health systems across Europe are increasingly challenged by population ageing, multimorbidity, and persistent inequities in access to care. Community Nursing has emerged as a promising approach to strengthening preventive, community-based services, yet evidence on stakeholder-perceived needs during early implementation remains limited. This study aimed to explore how different types of needs are perceived and articulated during the early implementation of Community Nursing in Austria. Methods: An interpretive descriptive qualitative study was conducted as part of an Aust

Nouakchott Declaration: Promoting Organ Donation and Transplantation in Africa.

BACKGROUND AND HYPOTHESIS: Africa faces a growing crisis of end-stage organ failure driven by the rising burden of non-communicable diseases and fragile health systems. Behind these figures are preventable deaths, disrupted families, and widening inequities in access to lifesaving care, an inequity that organ transplantation could address. However, access to transplantation remains extremely limited across the continent. According to the 2024 Global Observatory on Donation and Transplantation (GODT) report, only 544 kidney transplants were performed in Africa, representing approximately 0.5% o

Evidence to inform resource allocation for tuberculosis control in Myanmar: a systematic review based on the SYSRA framework.

Myanmar represents an extreme example of the difficulties in optimally allocating resources for maximum public health benefit, on the basis of limited information. At the recent Myanmar Health Forum 'Investing in Health' much of the discussion revolved around what to invest in, how health systems could be strengthened, and what research and capacity building areas the international donor community should prioritise for support. Funding for infectious disease control, particularly HIV and tuberculosis, is being channelled to the country at an unprecedented rate, but very little research has bee

Recent developments in family psychoeducation as an evidence-based practice.

Among potential resources for people with serious mental illnesses (SMI) and their families, professionally delivered family psychoeducation (FPE) is designed to engage, inform, and educate family members, so that they can assist the person with SMI in managing their illness. In this article, we review research regarding FPE outcomes and implementation since 2001, updating the previous review in this journal (McFarlane, Dixon, Lukens, & Lucksted, Journal of Marital and Family Therapy 2003; 29, 223). Research on a range of FPE variations continues to return mostly positive effects for adults wi

Impact of COVID-19 on Movement Disorders Patients in the Outpatient Setting.

Introduction The coronavirus disease 2019 (COVID-19) pandemic may have been the only worldwide infectious outbreak that many patients have experienced in their lifetime. The impact of COVID-19 on patients with chronic neurodegenerative disease, including movement disorders, is largely unknown. Since patients with movement disorders rely heavily on inpatient therapies and are closely followed by their medical team, it would be reasonable to see an increase in stress during a period of prolonged isolation. This study aimed to determine the emotional impact, stress, and burden of the COVID-19 pan

Utilizing the CFIR to Evaluate Rural COVID-19 Vaccine Uptake: Extension Personnel Perspectives.

Background/Objectives: Rural populations in the United States experienced consistently lower COVID-19 vaccination rates than urban populations during the pandemic. While barriers to vaccination have been well documented, less is known about facilitators of uptake in rural settings. This study aimed to identify facilitators of COVID-19 vaccination in rural communities from the perspectives of Washington State University (WSU) Extension personnel and to examine these factors using the Consolidated Framework for Implementation Research (CFIR) Outer Setting domain. Methods: A qualitative descripti

Context makes a difference: a qualitative study exploring how to implement a multifactorial falls prevention intervention in the community.

BACKGROUND: Falls are a major problem in community-dwelling older people. Multifactorial falls prevention interventions are recommended as intervention. However, poor implementation impacts successful outcomes of multifactorial falls prevention interventions. The context is critical to understand variation in implementation and clinical outcomes. This study aims to analyze and describe determinants at all levels of the context that influence the implementation of a multifactorial falls prevention intervention in the community in Flanders (Belgium). METHODS: This study consists of semi-structu

Administrative perspectives on the implementation and sustainability of state-supported health insurance schemes in Nigeria: a descriptive qualitative study.

BACKGROUND: Since the state-supported health insurance schemes (SSHIS) began in Nigeria, the perspectives of implementers and other administrative actors have been under-documented in the program evaluations. Bridging this information gap is crucial to addressing the challenges impeding the scheme's impact. This study, therefore, investigated the administrative stakeholders' perspective on the implementation and sustainability of the SSHIS in Nigeria. METHODS: This study adopted a descriptive qualitative case study design and was conducted in six Nigerian states with functional SSHIS, each re

Nurses' perspectives on perioperative pulmonary rehabilitation for patients with lung cancer in the pre-implementation phase: a CFIR 2.0-guided study.

BACKGROUND: Despite strong evidence supporting perioperative pulmonary rehabilitation (PR) for patients undergoing lung cancer surgery, little is known about factors that may influence implementation prior to program rollout, particularly in resource-constrained settings. Guided by the updated Consolidated Framework for Implementation Research (CFIR 2.0), this study aimed to identify anticipated barriers and facilitators to inform program refinement and future implementation strategies. METHODS: A descriptive qualitative design was used. Semi-structured interviews were conducted with nurses f

Oral health payment systems around the world: structures, challenges, and trends, a scoping review.

BACKGROUND: The prevalence of dental and oral diseases is increasing globally, yet the utilization of dental services has not shown a corresponding rise. Cost remains a major barrier, as dental care ranks among the four most expensive health services globally. These costs are shaped by each country's health payment system, which plays a critical role in determining access to healthcare. This scoping review aims to map the existing evidence on oral health payment systems and examine how these systems influence access, utilization, equity, and financial protection in different settings. METHODS

Beyond Research Priorities: Stakeholder-Identified Health System Challenges From an Australian Childhood Cancer Priority-Setting Partnership.

BACKGROUND: Priority-setting partnerships (PSPs) are used to identify research questions that matter most to patients, caregivers, and clinicians. However, submissions that fall outside the scope of research prioritisation are rarely examined despite their potential to reveal important unmet needs. This study explored concerns relevant to practice and policy using the out-of-scope questions submitted during the Australian Child and Adolescent Cancer PSP. METHODS: We conducted the Australian Child and Adolescent Cancer PSP with the James Lind Alliance to identify research priorities for childh

Extending the continuum: Mapping post-6-week nurse/midwife-led interventions for postnatal women - A scoping review.

BACKGROUND: The postpartum period is a critical phase for both maternal and infant health. In many healthcare systems, postpartum care is limited to a single six-week visit, often leaving women with inadequate support as they undergo the physiological, psychological, and social adjustments required for motherhood. AIM: This scoping review maps the characteristics, delivery approaches, reported outcomes, and evidence gaps of nurse- and midwife-led continuity-of-care interventions that include care or follow-up beyond six weeks postpartum. Regulatory evolution and service delivery models of telepharmacy in Thailand: a review of pandemic responses and future policy directions.

BACKGROUND: The COVID-19 pandemic accelerated digital health adoption in Thailand, disrupting traditional healthcare delivery and prompting rapid pharmaceutical system reforms. Prior to 2020, telepharmacy was virtually non-existent due to strict regulations mandating in-person dispensing at licensed premises; however, the public health crisis prompted policy changes, leading to regulatory relaxation and the formal establishment of telepharmacy standards. To understand these transformative shifts, this review examines the regulatory evolution of telepharmacy in Thailand, classifies diverse serv
