Practical tips and tricks for assessing prosthetic valves and detecting paravalvular regurgitation using cardiac CT.

Paravalvular leaks are an uncommon but serious complication of prosthetic valves. Transthoracic echocardiography is used in the assessment of prosthetic valves but can be limited by acoustic shadowing from the prosthesis and poor acoustic windowing. Small case series have previously shown cardiac CT to have promising results in detecting paravalvular leaks. We assessed 32 valves in our institution on cardiac CT using echocardiography results as standard and developed methods for improved evaluation of prosthetic valves. These include optimizing prescan drug therapy for heart rate control, opti

A radial force-independent bioprosthesis for transcatheter tricuspid valve implantation in a preclinical model.

BACKGROUND: Interventional treatments of tricuspid regurgitation have been emerging as minimally invasive alternatives. Among those reported transcatheter tricuspid devices, the radial force between the device and native tricuspid annulus is the common principle to be employed for device immobilization. However, this immobilization mechanism may potentially lead to adverse consequences. We developed a radial force-independent stent valve device for implantation at native tricuspid annular site without inducing stress on either myocardial tissue or the bioprosthesis. METHODS: We designed a rad

No fibrous cap and pannus formation in an On-X mechanical aortic valve.

Mechanical cardiac valves placed in the aortic position can show different amounts of pannus under the aortic ring. A 68-year-old patient had undergone mitral annuloplasty with a 30-mm rigid ring and aortic valve replacement with a 21-mm On-X mechanical valve for mitral regurgitation and aortic valve stenosis 10 years and 3 months previously. In the second cardiac surgery for mitral valve replacement to treat mitral stenosis, the previously replaced On-X mechanical valve revealed no fibrous cap on the sewing cuff and no pannus on the lower border of the valve ring. These characteristic finding

[Emergent Re-do Mitral Valve Replacement for Prosthetic Valve Endocarditis with Increasing Obstruction of Mitral Valve].

We report a surgical case of active infective prosthetic biologic mitral valve endocarditis with hemodynamic deterioration with increasing obstruction of mitral valve. The patient developed rapid hemodynamic deterioration, therefore, we performed emergent re-do mitral valve replacement using SJM prosthetic valve in the setting of supra-annular position. The postoperative course was uneventful.

Transcatheter valve repair and replacement.

There is significant interest in developing transcatheter therapy for valvular heart disease (VHD). Numerous devices have been developed for the percutaneous treatment of pulmonary and aortic stenosis as well as mitral regurgitation. Several of these devices have progressed to randomized clinical trials. These ongoing trials for aortic stenosis and mitral regurgitation will provide important insights into the durability of these therapies as well as the results following standard surgical repair. The field of transcatheter valve therapy is rapidly evolving, and this review aims to summarize th

Valved stents for transapical pulmonary valve replacement.

OBJECTIVES: Pulmonary valve insufficiency remains a leading cause for reoperations in congenital cardiac surgery. The current percutaneous approach is limited by the size of the access vessel and variable right ventricular outflow tract morphology. This study assesses the feasibility of transapical pulmonary valve replacement based on a new valved stent construction concept. METHODS: A new valved stent design was implanted off-pump under continuous intracardiac echocardiographic and fluoroscopic guidance into the native right ventricular outflow tract in 8 pigs (48.5 +/- 6.0 kg) through the r

[Clinical use of artificial heart valve PLANIKS].

The first clinical results of a new repeated-disc artificial heart valve PLANIKS application were summarized. In prosthesis the useful hydrodynamic parameters, durability, reliability, convenience and easiness while implantation conduction are combined harmoniously, the sterility is guaranteed. PLANIKS permits to correct adequately the hemodynamics disorders in the early postoperative period and may be recommended for the clinical practice use.

Has percutaneous aortic valve replacement taken center stage in the treatment of aortic valve disease?

Modern biomedical advances have propelled percutaneous valve replacement into an effective and powerful therapy for many heart valve diseases, especially aortic valve stenosis. Experiences so far suggest that outcomes for new percutaneous valve replacement surgery compare favorably with that of traditional valve surgery in selected patients with severe symptomatic aortic stenosis. The inception of percutaneous aortic valve replacement (PAVR) began in 1992 when the potential for treating valve diseases was demonstrated through a modern technique of endoluminal deployment of a catheter-mounted c

Complicated infective endocarditis: surgical treatment.

The destructive intracardiac complications of infective endocarditis present a continuing problem even though the mortality from the disease is decreasing. Osler in 1885 correctly described it as a malignant process. Tissue necrosis secondary to infection can cause destruction of valve leaflets and abscess formation in the valve annulus; the process may extend into adjacent parts of the heart and may even perforate into the pericardial cavity. For surgery to succeed it is necessary to excise all necrotic tissue, to replace the valve, and repair annular or other defects and suture the prosthesi

Case report: Self-expanding transcatheter valve implantation (Acurate Neo 2) in a very small native aortic annulus.

Transcatheter aortic valve replacement (TAVR) is a treatment of choice in patients with symptomatic severe aortic valve stenosis (AS) and intermediate-to-high surgical risk. The presence of a small aortic annulus (SAA) has been associated with a higher incidence of prosthesis-patient mismatch (PPM) when surgical aortic valve replacement (sAVR) is performed. TAVR might be a treatment option offering better hemodynamics with a lower incidence of PPM. When a severe AS with a SAA is treated, TAVR-related risk as the coronary obstruction and the annulus rupture, must be also prevented. We present a

[An operative case of left ventricular-right atrial communication complicated with aortic valve stenosis and regurgitation].

A 70-year-old woman suffering from aortic valve stenosis and regurgitation and tricuspid valve regurgitation was admitted to our hospital. At the operation, the right atrium and both ventricles were dilated, and a thrill was felt over the aortic root and right atrium. The aortic valve was bicuspid and heavily calcified. After aortic leaflets were resected, a defect was seen in the left ventricle. Then the right atrium was opened. The defect was located in the atrioventricular membranous septum just above the septal leaflet of the tricuspid valve. The defect was diagnosed to be a left ventricul
