Calcaneal Insufficiency Fracture Secondary to Celiac Disease-Induced Osteomalacia: A Rare Cause of Heel Pain.

UNLABELLED: Plantar fasciitis is a common cause of plantar heel pain; however, a broad spectrum of disorders may also present with plantar heel pain. A detailed history, physical examination, laboratory testing, and imaging studies may be necessary to reach an accurate diagnosis. Herein, the clinical presentation of a 33-year-old woman with calcaneal insufficiency fracture secondary to celiac disease-induced osteomalacia is presented, and its diagnosis and treatment are discussed. Calcaneal insufficiency fractures should be kept in mind in a patient with celiac disease that presents with heel …

Influence of knee flexion angle and age on triceps surae muscle fatigue during heel raises.

The triceps surae (TS) muscle-tendon unit is 1 of the most commonly injured in elite and recreational athletes, with a high prevalence in middle-aged adults. The performance of maximal numbers of unilateral heel raises is used to assess, train, and rehabilitate TS endurance and conventionally prescribed in 0° knee flexion (KF) for the gastrocnemius and 45° for the soleus (SOL). However, the extent of muscle selectivity conferred through the change in the knee angle is lacking for heel raises performed to volitional fatigue. This study investigated the influence of knee angle on TS muscle fatig…

Influence of knee flexion angle and age on triceps surae muscle activity during heel raises.

Triceps surae and Achilles tendon injuries are frequent in sports medicine, particularly in middle-aged adults. Muscle imbalances and weakness are suggested to be involved in the etiology of these conditions, with heel-raise testing often used to assess and treat triceps surae (TS) injuries. Although heel raises are recommended with the knee straight for gastrocnemius and bent for soleus (SOL), the extent of muscle selectivity in these positions is not clear. This study aimed to determine the influence of knee angle and age on TS muscle activity during heel raises. Forty-eight healthy men and …

A comparative assessment of interface pressures generated by four surgical theatre heel pressure ulcer prophylactics.

Current heel protection devices used in the operating room do not comply with the consensus document of the European and National (North American) Pressure Ulcer Advisory Panels. A complying prototype has been tested. Prospective cohort study comparing interface pressures. While using the prototype device, the heel interface pressure is significantly [mean 0·0 mmHg, standard deviation (SD) 0·0] less than the viscose elastic gel (VEG) mat (mean 174·8 mmHg, SD 64·5), the Action(®) heel block (mean 182·3 mmHg, SD 70·8) and the theatre table (mean 193·2 mmHg, SD 57·1). At the Achilles tendon, the …

Low-energy extracorporeal shock wave therapy for painful heel: a prospective controlled single-blind study.

The aim of this prospective single-blind pilot study was to explore the pain-alleviating effect of low-energy extracorporeal shock wave therapy (ESWT) in painful heel associated with inferior calcaneal spurs. Thirty patients who suffered from persistent symptoms for more than 12 months qualified for low-energy ESWT and were assigned at random to two groups, real or simulated ESWT. Before beginning the treatment, any other therapy was stopped for a period of 6 weeks. The shock waves were applied by a experimental device allowing exact localization through an integrated fluoroscopy unit. Patient…

The painful heel. Comparative study in rheumatoid arthritis, ankylosing spondylitis, Reiter's syndrome, and generalized osteoarthrosis.

This study presents the frequency of severe and mild talalgias in unselected, consecutive patients with rheumatoid arthritis, ankylosing spondylitis, Reiter's syndrome, and generalized osteoarthosis. Achilles tendinitis and plantar fasciitis caused a severe talalgia and they were observed mainly in males with Reiter's syndrome or ankylosing spondylitis. On the other hand, sub-Achilles bursitis more frequently affected women with rheumatoid arthritis and rarely gave rise to severe talalgias. The simple calcaneal spur was associated with generalized osteoarthrosis and its frequency increased wit

An Achilles' heel for methicillin-resistant Staphylococcus aureus? Re-evaluating β-lactam susceptibility of MRSA.

Methicillin-resistant Staphylococcus aureus (MRSA) infections (i.e., carrying the mec gene) are challenging to treat due to their inherent resistance to most antistaphylococcal β-lactams. Given the poor outcome of severe MRSA infections and limitations of the current approved treatments, there is growing interest in repurposing β-lactams as part of a treatment strategy for these infections. Here, we review the current literature on β-lactam susceptibility phenotypes in MRSA. We describe four phenotypes: oxacillin-susceptible, sensitivity to combinations of β-lactam-β-lactamase inhibitors, NaHC

Neanderthal's heel: How the Neanderthal's external calcaneal shape uniquely differs from that of modern humans.

Neanderthals exhibit unique morphological adaptations in their calcaneus compared to Homo sapiens, shedding light on distinct evolutionary pressures and functional requirements shaping their respective foot anatomy. This study assesses which calcaneal traits represent plesiomorphic, autapomorphic, and/or functional adaptations. Geometric morphometrics was used to analyze the external shape of 7 Neanderthal and 64 H. sapiens calcanei spanning early Upper Pleistocene hunter-gatherers to postindustrial populations. Shape, size, and allometric differences were assessed among groups, enabling detai

Evaluating the association between lifestyle factors and heel bone mineral density in different inflammatory states.

RATIONALE: It is unclear whether lifestyle factors affect bone mineral density (BMD) during different inflammatory states. OBJECTIVE: This study investigated the effects of coffee consumption, vitamin D (VD) intake, smoking, and alcohol consumption on heel BMD in adults with different inflammatory states. METHODS: The phenotypic data from 249,825 participants were analyzed using the UK Biobank cohort. The inflammatory status was evaluated using C-reactive protein (CRP) levels and the systemic immune-inflammation index. Linear regression analysis was used to examine the association between co…

Warming Prior to Heel Stick: Blood Sample Quality and Infant Comfort-A Randomized Controlled Trial.

BACKGROUND: Capillary blood sampling (heel stick) in infants is commonly performed in neonatal care units. Before the procedure, warming the infant's heel is often a customary practice, but no consensus exists on the most effective heel-warming method. PURPOSE: To compare the effects of routinely used warming methods (glove, gel pack, or blanket) applied prior to heel stick on blood sample quality and infant's comfort. METHODS: This prospective, double-blind, randomized controlled trial conducted in the neonatal intensive care unit included infants (postmenstrual age of ≥28 + 0 weeks and ≤43…

Graded stiffness offloading insoles better redistribute heel plantar pressure to protect the diabetic neuropathic foot.

BACKGROUND: Diabetic heel ulceration is a common, detrimental, and costly complication of diabetes. This study investigates a novel "graded-stiffness" offloading method, which consists of a heel support with increasing levels of stiffness materials to better redistribute plantar pressure for heel ulcer prevention and treatment. RESEARCH QUESTION: Is the novel "graded-stiffness" solution better able to redistribute heel pressure and reduce focal stress concentration areas of the heel? METHODS: Twenty healthy young men walked with four, 3D-printed, insole configurations. The configurations inc…

Management of Heel Pad Degloving Injury After Severe Foot Crush Injury: A Case Report Study.

Crush injuries of the foot and ankle are uncommon and they have a poor prognosis leading to some form of disability. Degloving injuries of the heel and foot after crush injuries are rare and very challenging to manage due to the need for reconstruction of both osseous and soft tissue architecture. We present a salvage strategy for an open injury to the foot with extensive soft tissue detachment from the plantar and dorsal surface. A 30-year-old man was transferred to the Emergency Department from a neighboring hospital with a crush injury to the foot that had resulted in a degloving injury of …

Kinematics and kinetics of single-limb heel rise in diabetes related medial column foot deformity.

BACKGROUND: Diabetes-related medial column foot deformities contribute to high plantar pressure, joint instability, ulceration and amputation. Impaired foot function may be an early indicator of foot structural incompetence and contribute to deformity progression. This study examines the ability of single-limb heel rise multi-segmental kinematics and kinetics to identify midfoot and hindfoot dysfunction in those with diabetes-related medial column foot deformity. METHODS: Single-limb heel rise foot kinematics and kinetics were examined in adults with diabetes mellitus and peripheral neuropath…

Relationship between plantar pressure and soft tissue strain under metatarsal heads with different heel heights.

BACKGROUND: Metatarsalgia due to increased plantar pressure under the metatarsal head (MTH) is often seen with wearing high-heeled shoes. However, the concomitant soft tissue strain has not been well discussed. The objective of our study was to explore the standing plantar pressure and corresponding soft tissue strain under the metatarsal heads (MTHs) with different heel heights. MATERIALS AND METHODS: Twenty-one healthy subjects (10 male and 11 female) participated. The plantar pressure and soft-tissue thicknesses were measured simultaneously with a load cell and a 10-MHz linear-array ultras…
