Syphilitic paroxysmal cold haemoglobinuria. Case report and study of the Coombs test.

A case of congenital syphilis which presented with paroxysmal cold haemoglobinuria is described. The patient had aortic incompetence, and the significance of this finding is discussed. The Coombs test was studied, and positive results appear to be due to interaction between antiglobulin serum and complement adsorbed on to the test cells, this adsorption being promoted by the cold antibodies.

ACUTE KIDNEY DAMAGE AS A COMPLICATION OF MALARIA CAUSED BY PL. MALARIA AND PL. FALCIPARUM: CLINICAL CASES.

We have described two clinical cases of severe malaria caused by different pathogens: Pl. falciparum and Pl. malaria, common to which there was a severe course, complicated by acute renal failure and hemolytic anemia. In a detailed analysis of both clinical cases, Patient 1 had acute kidney damage arose after the increase of anemia and thrombocytopenia, in combination with hemoglobinuria. This shows that the leading mechanism of kidney injure in this case is acute tubular necrosis, due to the toxic effects of free hemoglobin and sequestration in the capillaries of the glomerulus. A Patient 2 h…

Long-term outcome of a randomized controlled study in patients with newly diagnosed severe aplastic anemia treated with antithymocyte globulin and cyclosporine, with or without granulocyte colony-stimulating factor: a Severe Aplastic Anemia Working Party Trial from the European Group of Blood and Marrow Transplantation.

This follow-up study of a randomized, prospective trial included 192 patients with newly diagnosed severe aplastic anemia receiving antithymoglobulin and cyclosporine, with or without granulocyte colony-stimulating factor (G-CSF). We aimed to evaluate the long-term effect of G-CSF on overall survival, event-free survival, probability of secondary myelodysplastic syndrome (MDS) or acute myeloid leukemia (AML), clinical paroxysmal nocturnal hemoglobinuria, relapse, avascular osteonecrosis and chronic kidney disease. The median follow-up was 11.7 years (95% CI, 10.9-12.5). The overall survival ra…

[Paroxysmal nocturnal haemoglobinuria (PNH) and pregnancy (author's transl)].

A case report concerning paroxysmal nocturnal hemoglobinuria is presented. The role of hemorheologic factors in perfusion of the placenta is clearly demonstrated. Fetal monitoring and the application of hypervolemic hemodilution enabled prolongation of the pregnancy until a healthy neonate could be delivered by Ceasarean section at the 34th week of gestation.

Atypical Hemolytic Uremic Syndrome Secondary to Pancreatitis: A Case Report.

This is a report of an extremely rare case of an atypical hemolytic uremic syndrome (aHUS) that appears to have been triggered by acute pancreatitis. A 68-year-old man was examined at a medical institution because of sudden lower abdominal pain. The patient was diagnosed with acute pancreatitis on computed tomography. Hemoglobinuria and laboratory findings indicative of intravascular hemolysis were noted. Biochemical analysis revealed normal results for von Willebrand factor activity, antiplatelet antibodies, and ADAMTS13 (a disintegrin and metalloproteinase with thrombospondin type 1 motif, m…

[Administration of haptoglobin in ABO incompatible bone marrow transplantation].

Haptoglobin was administered i.v. in 2 cases of ABO incompatible bone marrow transplantation for the prevention of hemoglobinuria due to acute hemolysis. The first case was a 25-year-old woman with severe aplastic anemia. The transplantation was both major and minor mismatch, where the donor was type A and the recipient was type B. The second case was a 31-year-old man with chronic phase of chronic myelogenous leukemia. The transplantation was major mismatch, where the donor was type B and the recipient was type O. After the administrations of haptoglobin 8,000 units we transfused bone marrow …

PEG-bovine hemoglobin: safety in a canine dehydrated hypovolemic-hemorrhagic shock model.

An initial evaluation of PEG-bHb was performed using a modified hypovolemic shock model. PEG-bHb had a substantially longer intravascular half-life than native Hb and no measurable hemoglobinuria was observed in the canine. PEG-bHb allowed successful resuscitation with an oxygen carrying capacity of 14-22% over that of lactated Ringer's solution.

[A case of drug-induced immune hemolytic anemia during alpha-interferon therapy for renal cell carcinoma].

A Case of drug-induced immune hemolytic anemia during alpha-interferon (alpha-IFN) therapy for renal cell carcinoma is reported. A 61-year-old woman was admitted to Tochigi cancer center for the treatment of left renal cell carcinoma. She underwent left radical nephrectomy. From 7th post operative day, alpha-IFN (6 x 10(6) IU) was administered every other day. Diclofenac and indomethacin were administered for pain and high fever induced by alpha-IFN. Hemoglobinuria was first noted on 20th post operative day. Immune hemolytic anemia was suspected by blood examinations including Coombs' test, se…

Acute fatal Leptospira kirschneri infection in suckling lambs and calves in Uruguay.

Leptospira kirschneri is an emerging human and animal pathogen. Here we describe 2 unrelated outbreaks of acute fatal leptospirosis caused by L. kirschneri in suckling lambs and calves diagnosed following a flooding event in the spring of 2023 in northern Uruguay. In outbreak 1, the sheep flock was raised in a low-lying area previously used for rice cultivation; 20 lambs died after developing apathy, jaundice, and hemoglobinuria. In outbreak 2, 2 calves were found dead. At autopsy, 4 lambs and 2 calves had marked jaundice, hemoglobinuria, dark-red kidneys, and hepatomegaly. Microscopically, we…

Paroxysmal nocturnal haemoglobinuria. Experience over a 10 years period.

INTRODUCTION: Paroxysmal nocturnal hemoglobinuria (PNH) is a hemolytic, clonal and acquired disorder of the hematopoietic stem cell with a deficiency of all glycophosphatidyl-inositol (GPI) linked proteins. The aim of this retrospective study was to analyse haematological and biochemical data from 152 patients referred to our laboratory for diagnosis of PNH by flow cytometry (FC). METHODS: Patients and healthy donor (152 and 99 respectively) were studied. Ham, sucrose, lactate dehydrogenase (LDH), Iron, haptoglobin (Hp), blood cell morphology and Kaplow cytochemical stain for leukocyte alkali…
