Cloning of Antibodies from Patients with Heparin-Induced Thrombocytopenia.

We discuss two antibody cloning protocols-single B-cell culture (Nojima culture) and single B-cell expression cloning-to isolate antibodies associated with heparin-induced thrombocytopenia (HIT) from patients displaying clinical manifestations of the condition. The objective of the antibody cloning study is to provide valuable insights into the molecular mechanisms underlying the B-cell response and antibody-mediated pathogenesis in HIT, aiming to improve the diagnosis and management of this condition.

Destabilization of PF4-antigenic complexes in heparin-induced thrombocytopenia.

Heparin-induced thrombocytopenia (HIT) is initiated by antibodies that recognize large antigenic complexes composed of multiple molecules of cationic platelet factor 4 (PF4) and polyanions such as unfractionated heparin (UFH) that bind to each other primarily through electrostatic interactions. We asked whether the formation and stability of these HIT antigenic or ultralarge immune complexes (ULICs) would be inhibited by biocompatible synthetic polycationic molecules shown previously to dissociate UFH from antithrombin III and to inhibit polyphosphates. Members of this family of molecules, des…

Molecular regulation of coagulation by heparin-bound plasma Latexin.

Latexin (LXN), a naturally occurring low-molecular-weight protein, plays crucial roles in coagulation, inflammation, and thrombosis, making it highly relevant to hematological and cardiovascular research. In this study, we investigated the physiological function of LXN in coronary artery disease (CAD), with a particular focus on its heparin-dependent regulation of the coagulation process. Clinical evaluations showed that plasma LXN concentrations are significantly varied among patients with chronic coronary syndrome (CCS) versus acute coronary syndrome (ACS), which indicates its possible diagn…

Heparin-Induced Thrombocytopenia Following Tinzaparin Administration: A Case Report.

Heparin-induced thrombocytopenia (HIT), a severe immune-mediated reaction, presents a significant diagnostic challenge, particularly with atypical symptoms like syncope or when induced by low-molecular-weight heparin (LMWH). This report describes the case of a 77-year-old male who developed severe thrombocytopenia following tinzaparin administration for bridging therapy after an elective endovascular aneurysm repair (EVAR). The patient presented to the emergency department with syncope, an uncommon manifestation of HIT. Initial investigations revealed isolated thrombocytopenia, and a high clin…

Anti-PF4 ELISA-Negative, SRA-Positive Heparin-Induced Thrombocytopenia.

Heparin products are frequently used in the inpatient setting to prevent and treat venous thromboembolism, but they simultaneously put patients at risk of developing heparin-induced thrombocytopenia (HIT). The 4Ts score determines the pretest probability of HIT. Diagnosis is made with a screening antiplatelet factor (PF4) immunoassay and the serotonin-release assay (SRA) as a confirmatory test. Anti-PF4 assays have high sensitivity (98%) but lower specificity (50%) and result in frequent false-positive tests. We present a rare case from our institution of a patient with anti-PF4-Polyanion ELIS…

Electronic alerts to improve management of heparin-induced thrombocytopenia.

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is a difficult clinicopathologic diagnosis to make and to treat. Delays in identification and appropriate treatment can lead to increased morbidity and mortality. OBJECTIVES: To use electronic health alert interventions to improve provider diagnosis and management of heparin-induced thrombocytopenia through guideline-based, accurate care delivery. METHODS: This quality improvement initiative developed 3 electronic health record-based interventions at our 750-bed academic medical center to improve the initial management of suspected HIT betwe…

Heparin in sepsis: current clinical findings and possible mechanisms.

Sepsis is a clinical syndrome resulting from the interaction between coagulation, inflammation, immunity and other systems. Coagulation activation is an initial factor for sepsis to develop into multiple organ dysfunction. Therefore, anticoagulant therapy may be beneficial for sepsis patients. Heparin possesses a variety of biological activities, so it has a broad prospect in sepsis. Previous studies suggested that patients with sepsis-induced disseminated intravascular coagulation and high disease severity might be suitable for anticoagulant therapy. With the development of artificial intelli…

Proteomic profiling for biomarker discovery in heparin-induced thrombocytopenia.

New analytical techniques can assess hundreds of proteins simultaneously with high sensitivity, facilitating the observation of their complex interplay and role in disease mechanisms. We hypothesized that proteomic profiling targeting proteins involved in thrombus formation, inflammation, and the immune response would identify potentially new biomarkers for heparin-induced thrombocytopenia (HIT). Four existing panels of the Olink proximity extension assay covering 356 proteins involved in thrombus formation, inflammation, and immune response were applied to randomly selected patients with susp…

Bullous hemorrhagic dermatosis: A rare cutaneous reaction of heparin.

Bullous hemorrhagic dermatosis is a rare cutaneous reaction of heparin, a commonly used anticoagulant. Exact etiopathogenesis remains elusive but immune related mechanisms as well as dose dependent relationship have been proposed. Clinically, it is characterized by asymptomatic, tense hemorrhagic bullae on extremities or abdomen occurring 5-21 days after initiation of therapy. We report bilateral symmetrically grouped lesions, in a previously unreported distribution of this entity in both the forearms in a 50-year-old male admitted with acute coronary syndrome on oral ecosprin, oral clopidogre…

Laboratory diagnosis of heparin-induced thrombocytopenia: A retrospective experience.

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is a severe complication of heparin therapy associated with thrombosis that requires a quick diagnosis. Therefore, laboratory assays must provide an accurate and swift answer. This work aims to evaluate the performances of an ELISA assay, especially when combined with 4T risk score, and a functional assay. METHODS: Data were collected for 894 patients treated by heparin who underwent anticoagulant switch because of HIT suspicion and were examined by a multidisciplinary expert team who confirmed or ruled out HIT diagnosis. All patients were te…

Native aortic valve thrombosis due to heparin-induced thrombocytopenia.

A 65-year-old man with unstable angina was transferred to our department for coronary artery bypass grafting (CABG). Preoperative computed tomography indicated a large thrombosis in the left coronary cusp (LCC). CABG concomitant thrombectomy was performed under cardiopulmonary bypass using unfractionated heparin. Although the LCC was filled with a large thrombus, the left coronary artery ostium was not obstructed. The platelet count decreased by 68%, 12 days after starting heparin in the previous hospital. There is a high probability that this patient had HIT which was not recognized before su…

AFM investigation of APAC (antiplatelet and anticoagulant heparin proteoglycan).

Antiplatelet and anticoagulant drugs are classified antithrombotic agents with the purpose to reduce blood clot formation. For a successful treatment of many known complex cardiovascular diseases driven by platelet and/or coagulation activity, the need of more than one antithrombotic agent is inevitable. However, combining drugs with different mechanisms of action enhances risk of bleeding. Dual anticoagulant and antiplatelet (APAC), a novel semisynthetic antithrombotic molecule, provides both anticoagulant and antiplatelet properties in preclinical studies. APAC is entering clinical studies w

[Heparin-induced thrombocytopenia: a case report and literature overview].

Heparin-induced thrombocytopenia (HIT) is an immunologically-mediated complication, which usually follows heparin exposition, less frequently exposition to other drugs or even occurs spontaneously. The type of heparin, its dose and mode of application as well as the exposition time, major trauma or operation, and obesity represent the main risk factors for HIT. The probability of HIT correlates with so-called 4T-score. A confirmatory laboratory diagnostic should be exclusively reserved for patients with a medium to a high probability of HIT development (more than 3 points in 4T-score). The scr…

Heparin, Heparan Sulphate and the TGF-β Cytokine Superfamily.

Of the circa 40 cytokines of the TGF-β superfamily, around a third are currently known to bind to heparin and heparan sulphate. This includes TGF-β1, TGF-β2, certain bone morphogenetic proteins (BMPs) and growth and differentiation factors (GDFs), as well as GDNF and two of its close homologues. Experimental studies of their heparin/HS binding sites reveal a diversity of locations around the shared cystine-knot protein fold. The activities of the TGF-β cytokines in controlling proliferation, differentiation and survival in a range of cell types are in part regulated by a number of specific, se…

Heparin-induced thrombocytopenia associated with acute liver graft failure.

An orthotopic liver transplantation (OLT) is of a proven benefit in an acute liver failure (ALF). Heparin-induced thrombocytopenia (HIT) is strongly associated with thromboembolic complications. We present the case of a 56-year-old patient who underwent an OLT owing to an ALF of unknown aetiology. HIT type II with consecutive hepatic and portal vein thrombosis caused progressive graft failure. Total hepatectomy and porto-caval shunt were performed to reduce the toxic effects of liver cell necrosis such as multiorgan failure involving the respiratory, renal and cardiovascular systems. A suitabl…
