BACKGROUND: A 47-year-old woman who presented with hip pain as a result of osteoarthritis was treated with indometacin. Ten days after beginning treatment she developed jaundice and ascites. All medications were discontinued, but her symptoms continued to worsen.
INVESTIGATIONS: Physical examination, laboratory tests and radiologic examinations, including liver chemistry tests, viral serologies, serum iron studies, ceruloplasmin and alpha-fetoprotein analyses, autoimmune serologies, ascites studies, abdominal CT, liver ultrasound and liver biopsy.

A 68-year-old woman with Felty's syndrome was admitted to our hospital due to breathlessness. She was diagnosed as having rheumatoid arthritis at age 59 years. Abdominal computed tomography indicated ascites, splenomegaly and liver atrophy. A 55-year-old woman with psoriasis vulgaris was treated with oral 5-methoxypsoralen and UVA photochemotherapy. After 40 treatments over 3 months she became unwell with hepatitis attributable to the psoralen. Six years earlier she developed cholestatic hepatitis to flucloxacillin. A previous history of drug-induced reactions should be sought before prescribing further drugs with similar adverse effects.

Renal tissues from 43 of 49 children with hepatitis B virus-associated glomerulonephritis (HBV-GN) were examined for HBV DNA by in situ hybridization (ISH) assay within the last 10 years. HBV DNA was identified in 41 of the 43 cases (95.3%). A 42-year-old man was treated with interferon-alpha for chronic hepatitis B; during the fourth week of treatment he developed an exacerbation of liver disease, and nuclear and smooth muscle autoantibodies, which were previously negative, were detected in very high titers. After discontinuation of interferon therapy, ALT values subsided promptly and autoantibodies disappeared within a few months. This sequence of events strongly suggests a direct relationship between IFN treatment and a self-limited hepatitis with autoimmune markers in this case.

We report the case of a 68-year-old male HVC+ diagnosed as cholestatic hepatitis induced by glybenclamide, following a non-insulin dependent diabetes mellitus treatment. We studied 28 patients with chronic delta hepatitis for the presence of hepatitis delta virus (HDV) RNA in serum. The hot start polymerase chain reaction (PCR) method, in which the reaction begins at 60-80 degrees C, showed a higher sensitivity than conventional PCR reaction. Hepatitis E seems to be responsible for most epidemics of non-A, non-B hepatitis in developing countries. While hepatitis E is not endemic in the United States, it should be considered in the differential diagnosis of the traveler with non-A, non-B hepatitis. We describe a patient with acute hepatitis E, confirmed by serologic testing, after recently returning from India and discuss current knowledge about the pathogenesis, clinical features, and diagnosis of hepatitis E.

In carriers of HBsAg (Hepatitis B virus surface antigen) the incidence of Hepatitis D virus infection was studied in 72 (61.3%) males and 45 (38.5%) females, totally in 117 cases with a mean age of 34.8 years. There were 26 (22.2%) asymptomatic carriers of HBsAg where in other chronic carriers the diagnosis were as follows; 29 (24.7%) chronic persistent hepatitis, 20 (17.0%) chronic lobular hepatitis, 23 (19.6%) chronic active hepatitis and 19 (16.2%) posthepatic cirrhosis. The incidence of HDV serologic markers were found to be positive in 19 (16.2%) out of 117 cases.

We report on a 7-year-old boy with nephrotic syndrome due to a membranous glomerulonephritis. The disease was caused by a persistent hepatitis B antigenemia without formation of antibodies. It was possible to show diffuse deposits of HBs antigen in the glomerular basement membrane and the mesangium by immunohistology. Therapy with cyclophosphamide resulted in an acute exacerbation of the hepatitis and, consequently, in a seroconversion. Subsequently, the nephrotic syndrome and the HBs antigenemia disappeared, and the hepatitis healed.
