RATIONALE: Hip osteoarthritis is a major public health issue. International treatment guidelines recommend exercise to manage symptoms. This is an update of a review first published in 2009 and last updated in 2014.
OBJECTIVES: To determine the effects of land-based exercise on pain, physical function, quality of life, participant-reported treatment success, study withdrawals, and adverse events in people with hip osteoarthritis.
SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, and two trial registries from February 2013 (search date for last update) to 5 February 2025. We also checked …

Developmental dysplasia of the hip (DDH) is the most common joint condition in infants, encompassing a complex spectrum of pathologic states that can result in hip instability and dislocation. The most significant risk factors for DDH are breech positioning in the third trimester and family history of hip dysplasia. The condition is more common in females. Diagnosis is based on age-specific physical examination maneuvers and imaging studies. The Ortolani and Barlow maneuvers are the recommended physical examination techniques used to screen infants up to 3 months of age. Ultrasonography is the…

Cartilage lesions of the hip are frequently encountered in the setting of multiple concomitant hip pathologies. For patients with symptoms refractory to comprehensive nonoperative management, numerous surgical treatments are available. To date, the ideal management strategy for hip chondral defects remains controversial, with patients best suited by an individualized approach accounting for the lesion being treated as well as any potentially modifiable underlying factors such as femoroacetabular impingement or dysplasia. Studies on the treatment of hip chondral defects continue to be published…

Hip fractures are a frequent cause of hospitalization in the elderly population and can lead to significant morbidity and mortality. As the population continues to age, the incidence of hip fractures is expected to increase. The internist/hospitalist plays a critical role in the care of this population as many patients have multiple medical comorbidities. Management of the fragility hip fracture patient requires knowledge of several perioperative topics including preoperative risk assessment, risk reduction strategies, the optimal timing of surgical repair, venous thromboembolism prevention, a…

Hip pain is a common complaint in sports, and narrowing the differential diagnosis can be difficult. Many etiologies are secondary to overuse and respond well to nonsurgical treatment. The increased use of point-of-care ultrasound has helped provide timely and accurate diagnoses and some guided treatments. The hip is in close proximity to the abdomen and pelvis, and clinicians should be familiar with nonmusculoskeletal pain generators. This article is a comprehensive review of hip pain etiologies in athletes.

BACKGROUND: Synovial cysts of the hip are relatively rare lesions comparing to other joints. Patients are usually asymptomatic, but in some cases symptoms such as pain and/or compression of vessels or nerve could be present. Purpose of the study was to define clinical features and optimal management of synovial cyst of the hip joint through an accurate review of the literature.
METHODS: We present three consecutive cases treated with three different therapeutic strategies: surgical excision, wait-and-see and needle aspiration. An accurate review of the literature has been performed to identif…

The treatment of chondral hip injuries is challenging. However, for young patients with hip disorders, orthopedic surgeons now have the opportunity to intervene early in the development of debilitating joint disease. As understanding of the hip joint continues to evolve, more effective treatment strategies are emerging. There are several reportedly successful options for surgical treatment. This article reviews the clinical presentation of chondral injuries and the surgical modalities, arthroscopic and open, available to treat them.

Developmental dysplasia of the hip (DDH) causes anatomical changes that cause early coxarthrosis. Although risf factors have been determined, the aetiology and physiopathology remains exactly unknown. Neonatal screening with physical examination and ultrasound have been stablished in order to diagnose this disease early in life. A diagnosis in the first months of life is essential as it enables a normal hip to form and prevent the appearance of early coxarthrosis. Treatment principles are to be able to reduce the hip without provoking avascular necrosis of the femoral head, and to normalize th…

Osteoblastoma occurring in the region of the hip is very rare, and vague symptoms with uncharacteristic radiographic features often lead to misdiagnosis. Because of radiographic and histological similarities, it must be carefully distinguished from osteoid osteoma, aneurysmal bone cyst, giant cell tumor, and osteosarcoma. Computed tomography is the preferred imaging modality as it is able to detect the nidus and images will not exhibit the flare phenomenon caused by surrounding inflammation seen with magnetic resonance imaging. For hip joint lesions in weight-bearing areas, intralesional curet…
