[TOTAL ELBOW REPLACEMENT AS AN ALTERNATIVE TO SEVERELY COMMINUTED FRACTURES AROUND THE ELBOW].

BACKGROUND: Intra-articular elbow fractures are considered to be one of the most complex injuries in orthopedic trauma. Some are too comminuted for open reduction and internal fixation. Recently, total elbow replacement (TER) has gained popularity for the treatment of comminuted elbow trauma when other treatment options are not possible. METHODS: Since 2007-2013 we treated 18 patients with TER due to comminuted distal humeral fractures. We used the Disabilities of the Arm, Shoulder and Hand scoring system (DASH) to evaluate the patient's satisfaction. In addition we evaluated the elbow range

Comparison of parallel and orthogonal plating techniques and the predictive role of Hounsfield unit values in AO/OTA type 13-C distal humerus fractures in patients over 50.

BACKGROUND/OBJECTIVES: This study aimed to compare the clinical outcomes of parallel and orthogonal plating in the surgical treatment of AO/OTA type 13-C distal humerus fractures in patients over 50 years of age and to evaluate the predictive role of local bone quality, measured using Hounsfield Unit (HU), on these outcomes. METHODS: In this retrospective study, 80 patients over the age of 50 who underwent surgery for AO/OTA type 13-C distal humerus fractures between 2012 and 2022 were analyzed. Patients were categorized into two groups according to the plating technique: parallel(n = 33) or

Titanium vs. carbon fiber-reinforced intramedullary nailing for humeral bone tumors.

BACKGROUND: Multiple techniques have been described to treat humeral diaphyseal bone tumors requiring curettage or excision. Recent studies have suggested that carbon fiber-reinforced polyetheretherketone (CFR-PEEK) intramedullary nails (IMNs) may be preferable to titanium IMNs for patients with musculoskeletal tumors due to CFR-PEEK's high tensile strength, radiolucency, a modulus of elasticity closer to native bone, and improved postoperative surveillance/radiation dosing. In this study, we describe the rate of fixation failure for both CFR-PEEK and titanium humeral IMNs when used for humera

Anatomic considerations of locked humeral nailing.

To investigate the risk of axillary nerve injury by the proximal locking screws in antegrade nailing of humeral fractures, the anatomy of the axillary nerve was examined in 20 fresh anatomic specimen humeri, which subsequently were nailed antegrade with specially designed humeral locked nails. The axillary nerve was found to be on average 45.6 mm below the tip of the greater tuberosity; it was jeopardized by insertion of the lower proximal locking screw in one of the 20 specimens. Short humeri, humeri with small heads, or too deeply inserted nails may increase the risk of nerve injury; likewis

[Elbow arthroplasty].

Elbow arthroplasty is a rare treatment option and is primarily used to treat distal humeral fractures that are not amenable to open reduction and internal fixation, as well as degenerative and inflammatory arthritis. In Denmark, treatment has been centralised since 2008 to three specialised university hospitals to ensure a high quality of treatment. Elbow arthroplasty generally yields good functional and patient-reported outcomes, but with high rates of revision and complications. The current literature remains limited, particularly in terms of long-term outcomes.

[Typical liability damages of the upper extremities in childhood and their evaluation].

Legal appraisals of possible treatment errors in childhood and adolescence are rare, often refer to the treatment of trauma of the upper limbs and are frequently recognized. From the activity of the first author as an expert witness 31 expert opinions (25%) referred to the upper limbs. Supracondylar humeral fractures, radius neck fractures, radial condyle fractures and distal radius fractures were predominant. A primary treatment error was determined in 14 patients and a secondary error in 7 patients. This corresponds to a recognition rate of 68%. The aim of this study was the reflection and t

An uncommon variant of the Essex-Lopresti injury.

The Essex-Lopresti injury is an extremely rare condition combining radial head or neck fracture, distal radio-ulnar joint dislocation and rupture of the interosseous membrane. However, there are rare or unusual varieties or associated injuries. We report a case of a 46-year-old woman with a posterior dislocation of the radio-humeral joint, a radial shaft fracture, and a distal radio-ulnar joint dislocation. She underwent open reduction and internal fixation of the radial shaft fracture followed by an immobilization by a long elbow splint in supination for 6 weeks. At 18 months of follow-up, sh

The safety and feasibility of minimal invasive plate osteosynthesis (MIPO) of the posterior aspect of the humerus: A cadaveric study.

The aim of this study was to determine the feasibility of applying MIPO of the humerus via the posterior approach and to observe the tension of the radial nerve in different elbow positions. Two separate incisions were made on the posterior aspect of the humerus in ten fresh cadavers (20 humeri). The radial nerve was identified at the proximal incision and the distances through which the nerve could be elevated from the bone with the elbow in flexion and extension were measured. A 10-hole extra-articular distal humeral locking compression plate was inserted and fixed through the submuscular tu

Inferior shoulder dislocation after shortening osteotomy of the humerus. A case report.

We present a case of a post-surgical complete dislocation after excessive proximal humeral shortening osteotomy performed in a patient operated on for atrophic non-union of a proximal humeral fracture. This complication has not been previously reported. The dislocation occurred due to extensive laxity of the soft tissue envelope, predominantly the deltoid muscle, as well as rotator cuff tear. The reconstructive procedurę consisted of vertical duplication of the capsule, reinforcement of the repair with coracoacromial ligament, tenodesis of the long head of the biceps tendon to the conjoined te

Outcomes of intra-articular distal humerus open reduction and internal fixation based on severity of articular comminution.

BACKGROUND: The advent of modern, precontoured locking plates has improved treatment of distal humerus fractures (DHFs) by providing an anatomic fit and a reliable locking mechanism with interdigitating screws. However, severe articular comminution may predispose surgical failure despite anatomic reduction and a biomechanically strong construct. METHODS: A retrospective review was performed to identify patients who underwent distal humerus open reduction internal fixation (ORIF) between 2015 and 2020. Fractures were classified using the AO/OTA classification and further categorized by anatomi

The effect of osteoporosis management on proximal humeral fracture.

HYPOTHESIS AND BACKGROUND: Proximal humeral fractures comprise 10% of fractures in the Medicare population. The effect, if any, of treating osteoporosis to prevent these fractures has not been determined. The primary objective is to determine the effectiveness of a systematic osteoporosis screening and treatment program on the hazard of developing a fracture over the treatment period. The secondary aim is to determine demographic risk factors. METHODS: This is a retrospective cohort study in a health care organization serving 3.3 million members. Individuals selected for dual-energy x-ray abs

Reverse shoulder arthroplasty: State-of-the-art.

The reverse shoulder arthroplasty conceived by Paul Grammont in 1985 has gradually gained popularity as a treatment for multiple shoulder diseases. Unlike previous reverse shoulder prosthesis characterized by unsatisfactory results and a high glenoid implant failure rate, the Grammont design has immediately shown good clinical outcomes. This semi constrained prosthesis solved the issues of the very first designs by medializing and distalizing the center of rotation with an increased stability of the replacement of the component. The indication was initially limited to cuff tear arthropathy (CT

[Intramedullary Nailing of Supracondylar Femoral Fracture in Patient with Achondroplasia. Small Size Matters].

In patients with defect bone growth and dwarfism, fractures of the weight bearing skeleton are relatively rare due to their reduced mobility. When they do occur, their treatment and potential surgery are complicated. The commonly used therapeutic procedures are not applicable, the available implants are not suitable, if a surgery is necessary. An individual approach and often times also improvisation is needed. It is important to realize that these patients do not suffer from an intellectual disability and are fully aware of their physical impairment, which we must not make worse without an ef
