BACKGROUND Adnexal torsion is a gynecologic emergency that dictates immediate surgical intervention. Twisted ovarian cysts are rare in adolescents, but they can cause significant distress to patients. Idiopathic ovarian torsion is uncommon in adolescents and the incidence is reportedly higher in women aged 20 to 40 years. Most twisted cysts in adolescents are benign. Platelets are circulating blood elements with key roles in haemostasis and thrombosis. Platelets are activated by a range of stimuli including exposed subendothelial components. Haemostasis also depends upon the effects of inhibitory substances, including the gasotransmitter nitric oxide whose effects on platelets are well documented. Most preclinical studies on endogenous hydrogen sulphide signalling have given little consideration to the fact that the human body contains more bacterial cells than human cells, and that evolution provides the context for all biology.
