Patterns of recurrence in women with advanced and recurrent epithelial ovarian cancer treated with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

OBJECTIVE(S): To identify recurrence patterns and outcomes in women with advanced or recurrent epithelial ovarian cancer (EOC) after cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). METHODS: This is an IRB-approved single-institution cohort study of women who underwent CRS+HIPEC for advanced or recurrent EOC followed in a prospective registry from 1/12/2014-3/1/2020. Recurrence locations were defined as pelvic, upper abdominal (UA) and/or extra-peritoneal (EP). Univariate analysis assessed associations between recurrence location, progression-free survival (P…

Laparoscopic Hyperthermic Intraperitoneal Chemotherapy is Safe for Patients with Peritoneal Metastases from Gastric Cancer and May Lead to Gastrectomy.

BACKGROUND: Laparoscopic hyperthermic intraperitoneal chemotherapy (LS-HIPEC) is a novel strategy for patients with gastric adenocarcinoma (GA) metastatic to the peritoneum. We evaluated the safety profile of LS-HIPEC for patients with positive peritoneal cytology (PPC) or carcinomatosis from GA. METHODS: Outcomes were reviewed of patients with stage IV GA with peritoneal involvement who received LS-HIPEC from June 2014 to January 2017. LS-HIPEC included a 60-minute perfusion of mitomycin-C (30 mg) and cisplatin (200 mg) with inflow temperatures of 41-42 °C and outflow temperatures of 39-40 °…

Analysis of patient selection policy and pattern of recurrence after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for colorectal carcinomatosis.

BACKGROUND: Actual cure rate and patterns of recurrence after cytoreductive surgery (CRS) associated to hyperthermic intraperitoneal chemotherapy (HIPEC) for patients with colorectal peritoneal carcinomatosis (PC) are not yet well explored. Moreover, the selection policy to this resource-consuming treatment is still a matter of debate. METHODS: From a dataset of 400 CRS+HIPEC performed between October 1996 and December 2015, we selected 54 consecutive patients with colorectal PC. Exclusion criteria were age>70, PS>2, or disease progression during chemotherapy. From 2004, we also excluded pati…

Effects of Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy (HIPEC) in the Treatment of Goblet Cell Carcinoma: A Prospective Cohort Study.

BACKGROUND: Goblet cell carcinoma (GCC) of the appendix is a rare disease. Treatment options vary according to disease staging. Cytoreductive surgery in combination with hyperthermic intraperitoneal chemotherapy (CRS + HIPEC) may improve survival in patients with peritoneal spreading. OBJECTIVE: The aim of this study was to examine the prognosis of patients with appendiceal GCC treated per protocol, and to evaluate the results of CRS+HIPEC in cases of peritoneal spreading. METHODS: From 2009 to 2016, a total of 48 GCC patients were referred to the European Neuroendocrine Tumour Center of Exc…

Hyperthermic intraoperative intraperitoneal chemotherapy (HIPEC) with primary or secondary cytoreductive surgery in the treatment of advanced epithelial ovarian cancer.

BACKGROUND: Peritoneal dissemination is the most common route of spread of epithelial ovarian cancer (EOC). Cytoreductive surgery (CRS) followed by platinum-based systemic chemotherapy is the current standard treatment in advanced stages, with suboptimal results. The aim of this study is to analyze the outcome of advanced EOC treated with CRS plus hyperthermic intraoperative intraperitoneal chemotherapy (HIPEC) combined with systemic chemotherapy. METHODS: We analyze a cohort of women treated with CRS plus HIPEC for peritoneal carcinomatosis secondary to EOC from May 2007 to December 2014. We

Liposomal Bupivacaine Incisional Infiltration for Postoperative Analgesia After Cytoreductive Surgery with Hyperthermic Intraperitoneal Chemotherapy: A Randomized Placebo-Controlled Trial.

INTRODUCTION: Moderate-to-severe postoperative pain is common after cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC). Evidence that liposomal bupivacaine provides clinically important advantages over conventional local anesthetics is inconsistent, and evidence in CRS + HIPEC remains limited. This trial evaluated the incremental efficacy and safety of liposomal bupivacaine incisional infiltration added to patient-controlled intravenous analgesia (PCIA) compared with saline infiltration plus PCIA. METHODS: In this single-center, randomized, participant- and ass…

Secondary cytoreductive surgery (S-CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) in recurrent ovarian cancer: Current evidence and clinical perspectives.

Recurrent epithelial ovarian cancer (rOC) remains a major therapeutic challenge because of its high relapse rate and the progressive development of chemoresistance. Secondary cytoreductive surgery (S-CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have been explored as locoregional strategies to overcome the limitations of systemic therapy, but their clinical value varies substantially according to platinum sensitivity, surgical completeness, and patient selection. In platinum-sensitive rOC (PS-rOC), evidence from randomized controlled trials (RCTs) demonstrates that S-CRS provides …

Treatment of peritoneal disease arising from mucinous vs. non-mucinous appendiceal neoplasms with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

OBJECTIVES: Non-mucinous appendiceal neoplasms (NMAN) are rare. The role of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) in treating peritoneal dissemination from NMAN is poorly defined. We hypothesise that histology impacts survival and compared the disease characteristics and short- and long-term outcomes of mucinous and non-mucinous appendiceal neoplasms treated with CRS/HIPEC. METHODS: We retrospectively reviewed a prospective database of 228 patients with peritoneal disease from appendiceal primaries proceeding to CRS/HIPEC from 01/01/2008 to 30/06/20…

Normal CEA Levels After Neoadjuvant Chemotherapy and Cytoreduction with Hyperthermic Intraperitoneal Chemoperfusion Predict Improved Survival from Colorectal Peritoneal Metastases.

BACKGROUND: Normal carcinoembryonic antigen (CEA) levels (≤ 2.5 ng/ml) after resection of localized colorectal cancer or liver metastases are associated with improved survival, however, these trends are understudied for colorectal peritoneal metastases (CRPM). PATIENTS AND METHODS: We conducted a retrospective single-institution study of patients with CRPM undergoing cytoreductive surgery with hyperthermic intraperitoneal chemoperfusion (CRS/HIPEC) with and without neoadjuvant chemotherapy (NACT). CEA was measured before and after NACT and within 3 months after CRS/HIPEC. RESULTS: A total of

Quality of Life After Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy (CRS/HIPEC): Cancer Survivors' Perspective Through In-Depth Interviews.

BACKGROUND: CRS/HIPEC patients face unique quality of life (QoL) challenges due to advanced disease (peritoneal carcinomatosis), the extent of procedure, and risk for long-term complications. Standard QoL questionnaires are generic, focusing on tumor type and standard treatments, and likely do not capture this select population's full experience, suggesting the need for tailored instruments. We aimed to characterize the QoL challenges faced by CRS/HIPEC cancer survivors and determine whether these were captured by a standard QoL questionnaire. PATIENTS AND METHODS: An anonymous, semi-structur
