The imbalance of Th17/Treg cells exists in asymptomatic hyperuricemia (the early stage of gout).

OBJECTIVES: Given that hyperuricemia is a metabolic condition with a prolonged asymptomatic period and strong associations with gout and various metabolic disorders, we investigated the role of lymphocyte subsets in asymptomatic hyperuricemia (aHUA) and explored their potential implications for immune regulation. METHOD: The study enrolled 59 male patients with aHUA, 29 with acute gout (AG), and 28 healthy male controls (HCs). Laboratory data, including blood cell counts, inflammatory markers, blood lipids, liver and renal function, and the percentage and absolute counts of lymphocytes and CD…

Uncovering the metabolic landscape of aqueous humor in posterior subcapsular cataract associated with hyperuricemia.

The purpose of this study is to investigate the metabolic alterations underlying posterior subcapsular cataract (PSC) associated with hyperuricemia and to identify key metabolic pathways contributing to disease progression. Untargeted metabolomic profiling was conducted on aqueous humor samples obtained from twelve cataract patients classified into four groups based on PSC status and serum uric acid levels. Comparative analyses among hyperuricemic PSC (HP), normouricemic PSC (NP), hyperuricemic non-PSC (HC), and normouricemic non-PSC (NC) groups were performed. A total of 46 differentially exp…

Urate-lowering therapy and kidney outcomes in patients with chronic kidney disease and hyperuricemia.

Hyperuricemia is considered a modifiable risk factor for the development and progression of chronic kidney disease (CKD). There remains controversy over the effects of urate-lowering therapy (ULT) on kidney outcomes in patients with CKD and hyperuricemia. We conducted a cohort study using a sequential target trial emulation framework to evaluate the composite kidney outcomes in patients with CKD and hyperuricemia initiating ULT versus supportive care alone (control). A total of 269,831 eligible person trials (56,936 unique persons) with CKD and hyperuricemia who had received supportive care we…

Triglycerides as a mediator in the hyperuricemia-diabetes link: insights from a hypertensive Chinese population.

BACKGROUND: This study investigates whether triglycerides mediate the association between hyperuricemia and Type 2 Diabetes Mellitus (T2DM) in a hypertensive Chinese population. By comparing individuals with and without diabetes and applying generalized structural equation modeling (GSEM), we aimed to clarify the indirect effects of hyperuricemia via triglyceride pathways. METHODS: A total of 274 hypertensive diabetic patients were assessed for clinical and biochemical profiles. The study included clinical categorization of DM status, demographic analysis, generalized structural equation mode…

Mechanisms Associated With Renal Injury in Hyperuricemia and Strategies for the Development of Natural Active Substances.

Hyperuricemia (HUA) is a metabolic condition resulting from an abnormality in the process of purine metabolism. Its occurrence has been on the rise globally. The results of relevant studies show that 5% to 12% of HUA patients will eventually develop gout, and one-third of these patients may involve the kidneys and develop kidney disease. Although the severe renal health hazards associated with excessive uric acid levels are well known, the specific molecular mechanisms remain unknown. Therefore, this paper provides insights into the mechanisms and related chain reactions of HUA leading to rena…

Multi-cohort analysis unveils novel microbial targets for the treatment of hyperuricemia and gout.

The gut microbiota plays a crucial role in the development of hyperuricemia (HUA) and gout. However, the variability in study designs and analytical methods has led to inconsistent conclusions across different studies. Here, we conducted a comprehensive analysis of the gut microbiota associated with HUA and gout by examining 368 16S rRNA sequencing data from four Chinese cohorts, including 159 healthy controls (HC), 136 HUA patients, and 73 gout patients. Our findings indicate that there were significant differences in the gut microbiota composition between the three groups. Specifically, the …

Uncovering the Potential Mechanisms and Effects of Hyperuricemia and its Associated Diseases on Male Reproduction.

Male fertility and metabolic disorders, including obesity and diabetes, are closely connected. Since hyperuricemia and metabolic syndrome are strongly related, male fertility and hyperuricemia may, to some degree, be associated. According to recent studies, hyperuricemia imposes various effects on sex hormones, semen quality, and male erectile dysfunction. Some researchers claim that uric acid worsens male semen and raises the risk of erectile dysfunction (ED), while others state that it safeguards both penile erection and male semen. Additionally, it has been shown that gout and metabolic syn…

Elucidating the role of gut microbiota dysbiosis in hyperuricemia and gout: Insights and therapeutic strategies.

Hyperuricemia (HUA) is a condition associated with a high concentration of uric acid (UA) in the bloodstream and can cause gout and chronic kidney disease. The gut microbiota of patients with gout and HUA is significantly altered compared to that of healthy people. This article focused on the complex interconnection between alterations in the gut microbiota and the development of this disorder. Some studies have suggested that changes in the composition, diversity, and activity of microbes play a key role in establishing and progressing HUA and gout pathogenesis. Therefore, we discussed how th…

Hyperuricemia as a Risk Factor in Hypertension among Patients with Very High Cardiovascular Risk.

Hypertension remains a global threat to public health, affecting the worldwide population. It is one of the most common risk factors for cardiovascular disease. Today's treatments focus on creating a hypotensive effect. However, there is a constant search for additional factors to reduce the potential of developing hypertension complications. These factors may act as a parallel treatment target with a beneficial effect in specific populations. Some studies suggest that uric acid may be considered such a factor. This study investigated the potential effect of uric acid concentrations over 5 mg/

Hyperuricemia and Its Association With the Severity and Complications of Congestive Heart Failure: A Systematic Review.

Several studies have shown that an association exists between hyperuricemia and heart failure. Despite several innovative management strategies, heart failure is a significant cause of mortality worldwide. Hyperuricemia in heart failure patients leads to poorer outcomes. Additionally, hyperuricemia can be a strong surrogate marker for increased oxidative stress in heart failure patients. This oxidative stress leads to vascular endothelial damage and is linked to worsening heart failure and subsequent mortality. Hence, the measurement of serum uric acid levels in these patients can predict the …

A metabonomic study to explore potential markers of asymptomatic hyperuricemia and acute gouty arthritis.

BACKGROUND: Acute gouty arthritis (AGA) is a metabolic disease with acute arthritis as its main manifestation. However, the pathogenesis of asymptomatic hyperuricemia (HUA) to AGA is still unclear, and metabolic markers are needed to early predict and diagnose. In this study, gas chromatography (GC)/liquid chromatography (LC)-mass spectrometry (MS) was used to reveal the changes of serum metabolites from healthy people to HUA and then to AGA, and to find the pathophysiological mechanism and biological markers. METHODS: Fifty samples were included in AGA, HUA, and healthy control group, respec…

Molecular Elucidation of a Urate Oxidase from Deinococcus radiodurans for Hyperuricemia and Gout Therapy.

Urate oxidase initiates the uric acid degradation pathways and is extensively used for protein drug development for gout therapy and serum uric acid diagnosis. We first present the biochemical and structural elucidation of a urate oxidase from the extremophile microorganism Deinococcus radiodurans (DrUox). From enzyme characterization, DrUox showed optimal catalytic ability at 30 °C and pH 9.0 with high stability under physiological conditions. Only the Mg2+ ion moderately elevated its activity, which indicates the characteristic of the cofactor-free urate oxidase family. Of note, DrUox is the
