Partial HPRT deficiency (Kelley-Seegmiller syndrome).

Hypoxanthine-guanine phosphoribosyl transferase (HPRT) deficiency is an X-linked defect of purine metabolism. Clinical manifestations are usually related to the degree of enzyme deficiency; complete HPRT deficiency (Lesh-Nyhan Syndrome) presenting with severe neurological or renal symptoms, or partial HPRT deficiency (Kelley-Seegmiller syndrome) manifesting as a gout-urolithiasis syndrome. We report a case of partial HPRT deficiency presenting as chronic tophaceous gout, mental retardation, nephrolithiasis and family history suggestive of X-linked inheritance, for its rarity.

Normal HPRT coding region in a male with gout due to HPRT deficiency.

A deficiency of the enzyme hypoxanthine-guanine phosphoribosyltransferase (HPRT; EC 2.4.2.8) is associated with a spectrum of disease that ranges from gouty arthritis (OMIM 300323) to the more severe Lesch-Nyhan syndrome (OMIM 300322). To date, all cases of HPRT deficiency have shown a mutation within the HPRT cDNA. In the present study of an individual with gout due to HPRT deficiency, we found a normal HPRT cDNA sequence. This is the first study to provide an example of HPRT deficiency which appears to be due to a defect in the regulation of the gene.

Cerebrospinal fluid purine metabolites after complex febrile convulsions.

Adenosine monophosphate, inosine monophosphate, inosine, adenosine, guanosine, adenine, guanine, hypoxanthine, xanthine and uric acid were determined in cerebrospinal fluid (CSF) of 15 children after complex febrile seizures (CFS) and in 27 after simple febrile seizures (SFS), and compared with those in a control group of 63 children. There was no statistically significant difference between the groups for any of these metabolites, suggesting that CFS and SFS neither significantly disturb the metabolism of nucleotides, nucleosides or bases nor significantly deplete neuron adenosine triphosphat

Effect of calmodulin inhibitors on viability and mitochondrial potential of Plasmodium falciparum in culture.

Calmodulin inhibitors are toxic for a variety of protozoa. Chlorpromazine, calmidazolium, and trifluoperazine inhibited the incorporation of [3H]hypoxanthine and [3H]phenylalanine into Plasmodium falciparum organisms in cultures with 50% inhibitory concentrations varying from 5.1 microM (with calmidazolium) to 48 microM (with chlorpromazine), the former being more sensitive than the latter. However, these concentrations also immediately dissipated rhodamine 123 from the parasite mitochondrion. Similar concentrations inhibit other protozoa, as well as mammalian cells, and the possibility that m

[The effect of phototherapy on serum uric acid (author's transl)].

20 newborn infants treated wih phototherapy for icterus neonatorum had analyzed their glutathione reductase activity in the erythrocytes, serum uric acid concentration and the urinary content of uric acid, xanthine and hypoxanthine before and after phototherapy. The activity of glutathione reductase is a sensitive indicator for the availability of riboflavine. There was a significant decrease of serum riboflavine and of serum uric acid during light therapy. The decrease of serum uric acid concentration is discussed as effect of direct photodecomposition on one hand and an inhibitory effect of

The Lesch-Nyhan syndrome: a family study.

Two brothers were found to have athetoid cerebral palsy, mental and growth retardation and evidence of self mutilation. One had passed a renal calculus and both had high serum uric acid levels. The diagnosis of Lesch-Nyhan syndrome was confirmed by the finding of low levels of hypoxanthine-guanine phosphoribosyl transferase in erythrocytes and by autoradiography of fibriblasts. The mother, maternal grandmother, a female sibling and a maternal aunt were identified as carriers of the X-linked mutation which was responsible for the enzyme deficiency in the two male siblings.

Lesch-Nyhan Syndrome in a Family with a Deletion Followed by an Insertion within the HPRT1 Gene.

Lesch-Nyhan syndrome (LNS) is a rare X-linked inherited neurogenetic disorder of purine metabolism in which the enzyme, hypoxanthine-guanine phosphoribosyltransferase(HGprt) is defective. The authors report a novel mutation which led to LNS in a family with a deletion followed by an insertion (INDELS) via the serial replication slippage mechanism: c.428_432delTGCAGinsAGCAAA, p.Met143Lysfs*12 in exon 6 of HPRT1 gene. Molecular diagnosis discloses the genetic heterogeneity of HPRT1 gene responsible for HGprt deficiency. It allows fast, accurate carrier detection and genetic counseling.

[Myogenic hyperuricemia].

Myogenic hyperuricemia is caused by over-production of uric acid, which based on the excessive degradation of purine derivatives in exercising muscle. This phenomenon is frequently observed in the patients of muscle glycogen storage disease (GSD) types III, V and VII. In these patients, impairment of muscle ATP production leads to over-production of AMP, followed by excessive release of inosine and hypoxanthine from the muscle. The molecular lesions of muscle GSDs have been well analyzed. Recent repertoire of these mutations were referred in respect of myogenic hyperuricemia.

Molecular analysis of the mutations in five unrelated patients with the Lesch Nyhan syndrome.

We have identified the mutations in the hypoxanthine phosphoribosyltransferase (hprt) gene in five patients with the Lesch Nyhan syndrome (LN) by direct sequencing of hprt cDNA and genomic DNA. Three of the mutations affect splicing of exons 1, 2, and 9, respectively, while two are missense mutations in exons 3 and 8. All 5 mutations result in profound hprt deficiency as measured in fibroblast lysates. However, small differences in the clinical phenotype are seen between the patients. All these mutations are unique and have not been reported previously.

Pre-implantation diagnosis of HPRT-deficient male and carrier female mouse embryos by trophectoderm biopsy.

In an animal model for Lesch-Nyhan syndrome, the affected male embryos, as well as the carrier female embryos, have been successfully identified by biochemical microassay of a sample of trophectoderm cells taken from the mouse embryos at the blastocyst stage. The embryos were removed from the uterus, diagnosed and returned to the uterus within 2 days without the need for cryopreservation. The diagnosis was confirmed at 14 days gestation by analysis of the hypoxanthine phosphoribosyl transferase (HPRT) status of the fetuses. Live young were obtained from biopsied embryos after transfer.

Lesch-Nyhan disease: clinical experience with nineteen patients.

The clinical phenotype in Lesch-Nyhan disease has been analyzed in 19 patients studied in hospital. In each case the diagnosis was made on the basis of inactivity of the enzyme hypoxanthine guanine phosphoribosyltransferase in erythrocyte lysates. All had hyperuricemia, and the presence of 'orange sand' in the diaper was a prominent early complaint. All had self-mutilative behavior, of which the most characteristic form was biting the fingers or lips. All had the neurological syndrome of spasticity and choreoathetoid involuntary movements. All but one had less-than-normal intelligence.

Isolation of HPRT gene depleted human TK6 lymphoblastoid cell line through gamma irradiation.

Ionizing radiation induced DNA DSBs can cause large deletions in the human genome. This property may be utilized for gene disruption to create knockout mutants. The X-linked hypoxanthine-guanine phosphoribosyl transferase (HPRT) gene is commonly used as a reporter to evaluate the frequency and molecular nature of somatic mutations in the human population. Mutations at the HPRT locus are measured through a T lymphocyte cloning assay. This assay relies on lymphoblast feeder cells that support T lymphocyte proliferation via cell-to-cell contact and by helping in the expression of functional inter

Urinary guanidinoacetate and creatine levels in patients with HPRT deficiency.

Neurobehavioral manifestations of complete HPRT deficiency include severe action dystonia, choreathetosis, alteration of executive functions, and self-injurious behavior. Dystonic manifestations are also present in patients with partial HPRT deficiency. Pathophysiology of these manifestations is unknown. Guanidinoacetate is a neurotoxin implicated in certain dystonic syndromes. We have examined guanidinoacetate and creatine levels in urine from 11 HPRT deficient patients (9 with Lesch-Nyhan syndrome and 2 with partial deficiency). Urinary guanidinoacetate and creatine levels in HPRT deficient

Glucose infusion abolishes the excessive ATP degradation in working muscles of a patient with McArdle's disease.

A 23-year-old woman with McArdle's disease performed mild leg exercise on a bicycle ergometer. Saline or 10% glucose solution was infused throughout exercise. After exercise with saline infusion, her plasma concentrations of ammonia, hypoxanthine and creatine kinase increased greatly. Conversely, after exercise with glucose infusion, there were no appreciable changes in these plasma substances. In addition, she noticed that glucose infusion relieved her from muscle symptoms during exercise. These findings suggest that glucose infusion to patients with McArdle's disease ameliorates excessive AT

Muscarinic modulation of purine release from electrically stimulated rat cortical slices.

The release of 3H-labeled purines at rest and during electrical stimulation was investigated in slices of rat cortex prelabeled with [3H]adenine and perfused with Krebs solution. A linear relationship was found between radioactivity efflux and stimulation frequency from 2.5 to 20 Hz. At frequencies of less than 2.5 Hz, no increase in radioactivity efflux was detected. The amount of tritium released per pulse increased with stimulation frequency up to 10 Hz and declined at 20 Hz. The tritium efflux from the slices at rest and at a stimulation frequency of 10 Hz, analyzed by HPLC with ultraviole

Free radicals and pathogenesis during ischemia and reperfusion of the cat small intestine.

BACKGROUND/AIM: In spite of the interest in free radicals as mediators of ischemic damage, most information on these species in biological systems is derived from indirect measurements. Our aim was to obtain more direct information concerning sources of free radical production during ischemia and reperfusion. METHODS: We have performed simultaneous measurement of radical generation, purine metabolites, reduced glutathione, neutrophil infiltration and morphological appearance in the cat small intestine in vivo during 60 minutes of ischemia followed by 60 minutes of reperfusion. RESULTS: Radic
