Conversion for Severe Malnutrition After Single Anastomosis Duodeno-Ileostomy with Sleeve Gastrectomy (SADI-S) to Duodenojejunal Bypass (DJB): a Case Series and Literature Review.

Single Anastomosis Duodeno-ileostomy with Sleeve Gastrectomy (SADI-S) is an emerging bariatric procedure that can sometimes lead to severe malnutrition. This report describes two cases of conversion surgery for post-SADI-S malnutrition and reviews management strategies. Both patients, who had undergone sleeve gastrectomy followed by SADI-S for weight recurrence, developed severe protein-energy malnutrition with hypoalbuminemia (1.6-1.8 g/dL) requiring parenteral nutrition, which normalized following conversion surgery. They underwent laparoscopic conversion to Roux-en-Y duodenojejunal bypass w…

The Efficacy and Safety of Laparoscopic Single-Anastomosis Duodeno-ileostomy with Sleeve Gastrectomy (SADI-S) in Mid- and Long-Term Follow-Up: a Systematic Review.

This systematic review of 10 studies aimed to investigate the mid- and long-term results of duodeno-ileostomy with sleeve gastrectomy (SADI-S) according to the PRISMA guideline. Related articles, which reported outcomes of laparoscopic SADI-S with follow-up ≥ 3 years, were selected and analyzed. The percentage of excess weight loss (EWL) was 70.9-88.7%, and 80.4% at 6, and 10 years, respectively. The more common late complications were malabsorption (6.3%) and gastroesophageal reflux disease (GERD) (3.6%). The remission rates of hypertension, diabetes, GERD, obstructive sleep apnea, and dyslip…

Comparison of a low Hartmann's procedure with low colorectal anastomosis with and without defunctioning ileostomy after radiotherapy for rectal cancer: results from a national registry.

AIM: This study used a national registry to compare the outcome after a low Hartmann's procedure (LHP), defined as removal of most of the rectum to leave a short anorectal stump and an end colostomy, and low anterior resection (LA) with or without a diverting ileostomy (DI) in rectal cancer patients all of whom had received preoperative neoadjuvant radiotherapy (RT). METHOD: Patients who underwent LHP or LA with or without DI for rectal cancer after RT between 2009 and 2013 were identified from the Dutch Surgical Colorectal Audit. The postoperative outcome was compared between the three group…

A Hybrid Laparoscopic-Robotic Technique for Single Anastomosis Duodeno-Ileostomy With Sleeve Gastrectomy (SADI-S): A Consecutive Case Series Demonstrating Feasibility and Early Outcomes.

INTRODUCTION: Single anastomosis duodenoileostomy with sleeve gastrectomy (SADI-S) provides durable weight loss and metabolic improvement, particularly in patients with severe obesity. Despite outcomes comparable to other metabolic and bariatric surgery procedures, its adoption has been limited by the technical complexity of duodenal mobilization and hand-sewn duodeno-ileal anastomosis. METHODS: We describe a standardized hybrid laparoscopic-robotic approach to SADI-S developed at our institution. Laparoscopy is used for sleeve gastrectomy, bowel measurement, and initial duodenal mobilization…

Higher BMI increases risk of stoma-site incisional hernia and other complications following diverting loop ileostomy and reversal: a systematic review and meta-analysis.

BACKGROUND: Diverting loop ileostomies (DLI) are commonly used to protect high-risk anastomoses. While a higher body mass index (BMI) is kno … elated complications, particularly stoma-site incisional hernia. If feasible, mesh placement at the ileostomy site should be strongly considered for patients with a BMI > 25 to reduce the risk of stoma-site incisional hernia. Weight loss prior to DLI and reversal should be encouraged.

A novel technique for the construction of an end ileostomy to prevent stoma outlet obstruction after rectal resection and total colectomy: a single-center retrospective study.

PURPOSE: Stoma outlet obstruction (SOO) is an early postoperative complication of rectal cancer. We devised a novel surgical technique: end-ileostomy, to reduce SOO. Here, we describe the surgical technique used for constructing an end ileostomy and assess its impact on SOO. METHODS: The subjects of this retrospective study were 78 patients who underwent low anterior resection or total colectomy with a defunctioning ileostomy (DI) between 2018 and 2022. The surgical procedure for end ileostomy can be summarized as follows: First, the ileum was separated using a linear stapler, approximately 3…

Post-surgical morbidity in early versus late closure of defunctioning ileostomy after rectal cancer surgery: A systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: A defunctioning ileostomy is frequently created to avert the implications of a colo-rectal anastomotic dehiscence in rectal cancer surgery. The timing of closure of the ileostomy remains debatable as it is believed that early closure (EC) may be beneficial to most patients than the standard practice of late closure (LC). This meta-analysis was performed to compare surgical outcomes in patients who underwent EC versus LC. METHODS: Randomised controlled trials (RCT) which evaluated the effect of EC versus LC of ileostomy on surgical outcomes in rectal cancer patients was searched on…

Preoperative use of angiotensin-converting enzyme inhibitors, angiotensin II receptor blockers and diuretics increases the risk of dehydration after ileostomy formation: population-based cohort study.

BACKGROUND: Readmission rates following ileostomy formation are high. Dehydration and consecutive renal failure are common causes of readmission, potentially pronounced by drugs affecting the homeostasis. The aim of the study was to assess the risk of dehydration after ileostomy formation in patients treated with angiotensin-converting enzyme inhibitors (ACEI), angiotensin II receptor blockers (ARB) or diuretics. METHOD: This nationwide population-based cohort study used data derived from the Colorectal Cancer Data Base of several Swedish healthcare registers. The study included all patients …

Delay in loop ileostomy reversal surgery does not impact upon post-operative clinical outcomes. Complications are associated with an increased loss of microflora in the defunctioned intestine.

Loop ileostomy is a common surgical procedure to allow downstream tissue healing, with the aim of re-joining the bowel approximately 12 months later. The reversal procedure is associated with a substantial morbidity up to 40%. Our previous research demonstrated that defunctioned ileum becomes atrophied, with extensive microbial dysbiosis. This study sought to investigate the potential influence of delaying ileostomy reversal surgery upon both clinical and pathological outcomes. Post-operative clinical data was recorded, including routine blood test results, duration of hospital stay, length of…

CAGS and ACS evidence based reviews in surgery. Is a diverting loop ileostomy and colonic lavage an alternative to colectomy for the treatment of severe Clostridium difficile-associated disease?

The term “evidence-based medicine” was first coined by Sackett and colleagues as “the conscientious, explicit and judicious use of current best evidence in making decisions about the care of individual patients.”1 The key to practising evidencebased medicine is applying the best current knowledge to decisions in individual patients. Medical knowledge is continually and rapidly expanding. For clinicians to practise evidence-based medicine, they must have the skills to read and interpret the medical literature so that they can determine the validity, reliability, credibility and utility of indiv

Parastomal hernia outcomes after sutured versus unsutured composite techniques for specimen-extraction-site loop ileostomy following laparoscopic low anterior resection: the UNSEAL randomized controlled trial.

BACKGROUND: Protective loop ileostomy created at the specimen-extraction site is a minimally invasive option after laparoscopic low anterior resection for rectal cancer. Whether different composite techniques influence parastomal hernia risk remains uncertain. METHODS: This prospective, open-label, single-center randomized controlled trial enrolled patients undergoing laparoscopic low anterior resection for rectal cancer with protective specimen-extraction-site loop ileostomy. Patients were randomized intraoperatively to a sutured composite technique, consisting of fascial/peritoneal fixation…
