Malarial nephropathy.

Malaria is widely prevalent in the tropics. Clinically significant renal and renal-related disorders commonly occur in infection with Plasmodium falciparum and P. malariae. Falciparum malaria causes fluid and electrolyte disorders, transient and mild glomerulonephritis, and acute renal failure (ARF). It appears that ARF is mediated by a complex interaction of mechanical, immunologic, cytokine, humoral, acute phase response, nonspecific factors, and hemodynamic factors. Parasitized erythrocytes play a central role in all aforementioned pathogenic factors of ARF. Antimalarial drugs are still the

Host-microbiome archetypes differentiate infection from pathogen carriage in the human lower airway.

Distinguishing lower respiratory tract infection (LRTI) from incidental pathogen carriage (IPC) is clinically challenging. The immunologic and microbial factors defining the states of LRTI and IPC are poorly understood. Here, we perform host-microbe metatranscriptomic profiling of tracheal aspirates from 326 mechanically ventilated children with clinically adjudicated LRTI (n = 207), IPC (n = 70), or non-infectious respiratory failure (n = 49). In the airway microbiome, LRTI shows reduced alpha diversity and taxonomic richness, while IPC displays greater bacterial abundance, enrichment in resp

Environmental triggers of pediatric type 1 diabetes: A protocol for a systematic review and meta-analysis on the impact of COVID-19 infection and vaccination.

INTRODUCTION: The study aimed to systematically evaluate whether coronavirus disease (COVID-19) infection and/or vaccination act as environmental triggers for new-onset type 1 diabetes (T1D) in children and adolescents. MATERIAL AND METHODS: We will conduct a systematic review and meta-analysis in accordance with PRISMA 2020 guidelines. PubMed, Embase, Scopus, Web of Science, and Cochrane Library will be searched for studies from 2020 onward. Eligible studies must report new-onset T1D in individuals aged 0-19 years following confirmed SARS-CoV-2 infection or COVID-19 vaccination, with a clear

Emerging haemostatic agents and patient blood management.

The transfusion of allogeneic blood products has been considered as a life-saving procedure for patients suffering from major traumatic injury and those who are undergoing major surgery. The safety of blood products has improved in terms of infectious complications over the last three decades due to advanced donor screening procedures and tests. Nevertheless, non-infectious complications including a blood-type mismatch, volume overload and immunologic and non-immunologic reactions to blood products can adversely affect clinical outcomes. It is thus important to implement a patient-specific str

Role of genetic factors in organ-specific autoimmune diseases induced by manipulating the thymus or T cells, and not self-antigens.

There are accumulating demonstrations that manipulation of the T-cell immune system, such as elimination of a particular T-cell subpopulation from the periphery or removal of the thymus during a critical neonatal period, can elicit activation/expansion of pathogenic self-reactive T cells from the remaining T cells and produce a wide spectrum of organ-specific autoimmune diseases in otherwise normal mice or rats. The genetic makeup of the hosts appears to play a key role in determining which self-reactive T-cell clones are prone to be activated under such circumstances, since a comparable degre

Respiratory illness in persons with human immunodeficiency virus infection. The Pulmonary Complications of HIV Infection Study Group.

Although the pulmonary complications of advanced human immunodeficiency virus (HIV) infection have been well described, there is little information on respiratory manifestations of earlier disease. This report describes the respiratory disorders diagnosed over an 18-month period in a cohort of persons with or at risk for HIV infection with variable immunologic status. Cohort members were followed routinely and evaluated for respiratory disease by standard diagnostic algorithms. The 18-month incidence of each respiratory diagnosis was determined, and for frequent diagnoses, incidence by transmi

Immunological specificity and mechanism of action of IgG lupus anticoagulants.

Although observations have implied that lupus anticoagulants have immunologic specificity toward anionic phospholipids, this assumption has been directly demonstrated in only one patient with a monoclonal IgM paraprotein. We tested the generality of this hypothesis directly by isolating five IgG lupus anticoagulants from patients with lupuslike syndromes and/or thrombosis. IgG lupus anticoagulant fractions were isolated free of other plasma proteins and free of contaminating phospholipid by adsorption to and elution from cardiolipin-cholesterol-dicetyl phosphate liposomes, followed by chromato

[Exogenous allergic alveolitis of farmers (farmer's lung) from the viewpoint of an alpine pulmologic practice].

Extrinsic allergic alveolitis of the rural population is a relevant disease of the alpine region. Observations in a pulmonologic practice suggest that the disease may represent itself in a acute and a chronic form. Six patients with acute alveolitis, 8 cases of chronic farmer's lung, 16 cases of late disease stages and 33 patients of sensitization (with positive immunologic data) without overt disease are described. Additional 31 patients with bronchopulmonary symptoms but without specific antibodies served as controls. The entire patient population was actively working in farming jobs. Case h

Spontaneous murine lupus-like syndromes. Clinical and immunopathological manifestations in several strains.

MRL/1 and BXSB male mice have a systemic lupus erythematosus (SLE)-like disease similar to but more acute than that occurring in NZB X W mice. The common elements of lymphoid hyperplasia, B-cell hyperactivity, autoantibodies, circulating immune complex (IC), complement consumption, IC glomerulonephritis with gp70 deposition, and thymic atrophy were found in all three kinds of SLE mice. On the basis of these common elements, SLE seen in these mice can be considered a single disease in the same sense that human SLE is one disease. The differences in the SLE expressed in the different mice are no

Immunologic evaluation of patients with cancer by delayed hypersensitivity reactions.

Testing of delayed hypersensitivity responses to recall antigens, newly encountered antigens and tumor antigens has contributed to the understanding of several immunologic factors in human neoplasia. Patients with Hodgkin's disease tend to have depressed responses to both newly encountered and recall antigens. Patients with solid tumors are more likely to be deficient only in the response to newly encountered antigens. In patients who have intact response to recall antigens, reactivity to antigen preparations from tumor and control tissue may be studied. Tumor-associated or organ-associated an…

Immunosuppressive factors in porcine ciliary body.

Immunosuppressive factors locally present in the anterior tissues such as the ciliary body may contribute to the immunological privilege of the anterior chamber of the eye. Porcine ciliary body extracts contain at least three immunosuppressive factors (greater than 200 kDa, 25 kDa and 2.5 kDa) which can be separated by Sephacryl S300 gel filtration. On the basis of its molecular weight and reversal of inhibitory activity by anti-transforming growth factor beta (TGF beta) antibody, 25 kDa factor is identified with TGF beta. The high molecular factor is low in protein content and its activity is

Innate and adaptive immune dysregulation in women with recurrent implantation failure.

Recurrent implantation failure (RIF) is a condition where a woman fails to obtain pregnancy after multiple embryo transfer cycles, even with superior-quality blastocysts. There are various factors that can contribute to RIF, including immunologic disturbances. The immune system is extremely important during pregnancy. Immune cells such as T cells, B cells, natural killer (NK) cells, and macrophages (MQ) are present in the female reproductive tract and are accountable for regulating the immune response to invading pathogens and maintaining tissue homeostasis. Dysregulation of these immune cells

Immunological and molecular basics of the primary open angle glaucoma pathomechanism.

Glaucoma is a degenerative process of the optic nerve. Increased intraocular pressure is believed to be the main factor leading to the glaucomatous damage. The in vitro and in vivo animal glaucoma research models provide insight into the molecular changes in the retina in response to the injury factor. The damage is a complex process incorporating molecular and immunological changes. Such changes involve NF kB activity and complement activation. The processes affect the human antigen, JNK, MAPK, p53, MT2 and DBA/2J molecular pathways, activate the autophagy processes and compromise neuroprotec

Grand round: Autoimmune hepatitis.

Autoimmune hepatitis is a corticosteroid-responsive liver disease arising consequent to immunogenetic and environmental risk factors. The clinical course reflects relapsing and remitting, hepatocyte targeted immunologic damage, which is countered by reparative responses to cell injury. Appropriate and timely immunosuppressive therapy drives the disease into remission, albeit with inevitable side effects. Many challenges faced in the clinic reflect practice that must capture a heterogeneous disease presentation, course, and treatment response, as well as treatment tolerability. In this Grand Ro

Pathogenesis of dermatophytosis and tinea versicolor.

Dermatophytoses are infections caused by keratinophilic fungi known as dermatophytes. Several steps are required for infection to take place: contact, adherence, and invasion of keratin layers. The severity of the infection depends on the type of agent, environmental factors, and the host immunologic status. Tinea versicolor is caused by the Malassezia spp yeasts, which are microorganisms that belong to normal biota in seborrheic areas, but some contributing factors, such as the application of oily preparations, creams, an increase in ambient humidity, corticosteroid abuse, or genetic predispo

[Osteoporosis and inflammatory bowel disease].

Inflammatory bowel disease is a chronic disease with an unknown ethiology although multiple factors intervene such as individual, genetic and immunologic susceptibility, as well as different environmental factors. Like other multisystemic diseases, its clinical manifestations are diverse and it may affect other organs besides the gastrointestinal tract. In the last few years there is a growing interest for one of these extraintestinal manifestations, osteoporosis and osteopenia that may affect up to 42% of patients and can condition an important increase in morbility. Potential etiologies of biliary atresia.

Biliary atresia is the most common cause of neonatal cholestasis and the leading indication for pediatric liver transplantation worldwide. The disease is caused by a progressive inflammatory and fibrosing obliteration of the extrahepatic bile ducts. Although the cause of this obstruction is largely unknown, patient-based studies have identified environmental and genetic factors that may interact and orchestrate disease pathogenesis. Chief among these factors are infectious and immunologic processes. While infectious agents have varied in different patient populations, studies of liver specimen

[Arterial hypertension of the pregnant woman].

Pregnancy hypertension remains a frequent disease, sometimes seriously threatening mother and fetus. Its primum movens has been shown to be an abnormal placentation and/or trophoblastic invasion. This abnormality is triggered by various factors which can be immunologic, vascular, or abnormalities of hemostasis. This defective placentation results in a systemic endothelial disease with vasoconstriction and widespread thrombotic tendency. Antihypertensive treatment is of very poor efficiency, and even may worsen the fetal situation through underperfusion. Early prevention is clearly a better way

[Treatment of alcoholic liver diseases and psychiatric and psychosocial problems].

Only about 15% of the subjects abusing ethanol will eventually develop cirrhosis of the liver, suggesting that other factors in addition to the consumption of large quantities of ethanol play a role in the pathogenesis of alcoholic cirrhosis. Important contributors may be infection with hepatitis viruses, in particular HCV, protein-calorie malnutrition and immunologic factors. Abstinence improves the prognosis of patients with alcoholic cirrhosis, provided that the liver disease is not too far advanced. No pharmacotherapeutic intervention has shown a convincing improvement of the prognosis of …
