[Hemophagocytic syndrome with syndrome of inappropriate secretion of antidiuretic hormone (SIADH)].

A 69-year-old female was admitted to the hospital because of fever, hyponatremia and anemia. Laboratory data showed hemoglobin 8.6 g/dl, indirect bilirubin 1.9 mg/dl and Na 122 mEq/l. Urine osmolality was elevated and urinary excretion of sodium was increased. Furthermore, antidiuretic hormone (ADH) level was elevated. Renal function and hormonal data were within normal limit. Therefore, she was diagnosed as having syndrome of inappropriate secretion of antidiuretic hormone (SIADH). On the other hand, bone marrow aspiration showed hemophagocytosis and the diagnosis of hemophagocytic syndrome (

[Hypotonic hyponatremia: therapeutic and controversial aspects].

Inappropriately slow or excessive correction of severe (< 115 mEq/l) hyponatremia could induce a high morbidity and mortality rate. In acute (< 48 hr) hyponatremia, there is an increased risk for death and permanent brain damage due to grand mal seizure and respiratory arrest. Indeed, if correction is delayed, the ability of brain to adapt to hyponatremia by limiting the amount of brain swelling is not sufficient and brain stem herniation occurs. Menstruant women are particularly likely to experience these complications. In chronic (> 48 hr) hyponatremia, the extrusion of intracerebral osmolyt

High and dry--low and wet: the key to DI and SIADH.

The pediatric critical care nurse is often faced with children who have underlying neurologic problems. Anticipating the occurrence of diabetes insipidus (DI) and syndrome of inappropriate secretion of antidiuretic hormone (SIADH) in these children is essential. There is much confusion, however, surrounding recognition and management of these distinctly different although related problems. The key to differentiating these clinical entities may be found in the simple statement, high and dry; low and wet.

SIADH during disseminated Herpes varicella-zoster infections: relationship to vidarabine therapy.

Three patients with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) during the course of disseminated Herpes Varicella-Zoster (HVZ) virus infections are reported. In one patient and two previously reported patients, SIADH occurred at or shortly after admission, prior to antiviral drug therapy. In two patients and one previously reported patient, SIADH began or hyponatremia worsened after vidarabine therapy was begun. Therefore, SIADH may occur during the course of untreated, disseminated HVZ infection. However, the relatively high fluid volume required to dilute vidarab

Steroid therapy in inappropriate secretion of antidiuretic hormone due to malignant thymoma.

The authors present the case of a 48-year-old female known to have malignant thymoma with metastases for 6 years and treated continuously for 4 years by corticosteroids with marked regression of tumor. The patient ceased medication on her own accord, and was admitted with extensively enlarged tumor and metastases in association with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). In view of the previous excellent response of the thymoma to corticosteroids, the same treatment was given again with disappearance of the tumor and the features of SIADH. The authors are unaw

[Tuberculous meningitis in intensive care].

A case of tuberculous meningitis in a 2-year-old boy is reported. The main critical care problems are irregular breathing, raised intracranial pressure and syndrome of inappropriate secretion of antidiuretic hormone. Cranial CT-Scan and ventricular shunting are shown to be of high importance for this disease.

[Surgery of the hypophyseo-hypothalamic lesions and the circadian rhythm of ADH secretion].

Various clinical disorders of water and electrolytes metabolism were reported in relation to decrease or increase of the plasma anti-diuretic hormone (ADH), however, the estimated ADH value was not infrequently found inconsistent with clinical features. In order to determine the influence from surgical intervention to the hypophyseo-hypothalamic system, continuous estimation of the urinary secretion and specific gravity was performed with computerized analyzer, and it was found that there were at least 5 patterns of postoperative change in rhythmicity of ADH secretion. Most interesting finding

[Partial defect in the secretion of antidiuretic hormone and disproportionate polydipsia (author's transl)].

A 20-year-old patient was evaluated because of polydipsia and polyuria; by means of the dehydration test a partial defect in the secretion of antidiuretic hormone (ADH) was demonstrated, since the urinary osmolality after the administration of exogenous vasopressin was superior by 25 percent to the maximum spontaneous urinary osmolality reached after a period of fluid restriction. Nevertheless, there was also a component of psychogenic polydipsia because the daily basal fluid intake was superior to 15 liters, and in view of the fact that the urinary osmolality could reach 600 mOsm/kg, the endo

Demeclocycline in the treatment of the syndrome of inappropriate secretion of antidiuretic hormone.

Fourteen patients with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) have been treated with demethylchlortetracycline (demeclocycline) 1200 mg daily. In 12 patients the underlying lesion was malignant. The serum sodium returned to normal (greater than 135 mmol/l) in all patients after a mean of 8.6 days (SD +/- 5.3 days). Blood urea rose significantly from the pretreatment level of 4.2 +/- 2.3 mmol/l to 10.1 +/- 5.1 mmol/l at ten days (P less than 0.001). The average maximum blood urea was 13.4 +/- 6.8 mmol/l. In four patients the urea rose above 20 mmol/l, and in two

Hyponatremia and anti-diuretic hormone in Legionnaires' disease.

BACKGROUND: Medical textbooks often list Legionnaires' disease as a differential diagnosis of the syndrome of inappropriate secretion of anti-diuretic hormone (ADH) (SIADH), but evidence supporting this association is largely lacking. We tested the hypothesis whether hyponatremia in patients with Legionnaires' disease would be caused by increased CT-ProVasopressin. METHODS: We measured CT-ProVasopressin and sodium levels in a prospective cohort of 873 pneumonia patients from a previous multicentre study with 27 patients having positive antigen tests for Legionella pneumophila. RESULTS: Patie

Early-Onset Hyponatremia Presenting as Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) Post-transsphenoidal Pituitary Resection.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a disorder in which an excessive amount of antidiuretic hormone (ADH) is released, leading to water retention and subsequent hyponatremia. Delayed hyponatremia, typically beginning on postoperative day (POD) 4, is a well-recognized complication following transsphenoidal resection of pituitary tumors and is thought to result from excess release of stored ADH due to surgical trauma. In this rare case of early symptomatic hyponatremia secondary to SIADH, a 61-year-old postmenopausal woman with no significant past medical history

Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) Associated with Mediastinal Schwannoma.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is the most common cause of euvolemic hypo-osmotic hyponatremia. There are several etiologies of SIADH including neuroendocrine tumor, pulmonary disease, infection, trauma, and medications. Here, we report a case of SIADH associated with a schwannoma involving the mediastinum in a 75-year-old woman who presented with nausea, vomiting, and general weakness. Laboratory testing showed hypo-osmolar hyponatremia, with a serum sodium level of 102mmol/L, serum osmolality of 221mOsm/kg, urine osmolality of 382mOsm/kg, urine sodium of 55

Recovery from SIADH-associated osteoporosis: a case report.

INTRODUCTION: Recent studies show an association between hyponatremia and osteoporosis. We have previously reported a case of severe male osteoporosis due to chronic syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Here, we provide a follow-up on this case after cure of the condition that further supports the causal relationship. THE CASE: A 38-year-old man had been diagnosed with severe osteoporosis most likely due to chronic SIADH. The SIADH was believed to be idiopathic. A magnetic resonance imaging scan, however, revealed a tumor in the sinus, and biopsies showed an es
