Evaluation of patient experience from participation in the randomized INFLUENCE trial on fluorescence-guided sentinel node surgery.

BACKGROUND: Recruitment to surgical trials is limited by strong patient and surgeon preferences, lack of equipoise, and anxiety surrounding randomization. There is minimal research exploring patient perspectives on randomization within surgical oncology, especially involving novel technologies like fluorescence-guided surgery. This study assessed patient experience following participation in the INFLUENCE trial, which compared indocyanine green fluorescence with standard tracer methods for sentinel lymph node localization in early breast cancer. METHODS: A retrospective questionnaire, distrib

Shifting gender norms to improve HIV service uptake: Qualitative findings from a large-scale community mobilization intervention in rural South Africa.

BACKGROUND: Interventions to improve HIV service uptake are increasingly addressing inequitable and restrictive gender norms. Yet comparatively little is known about which gender norms are most salient for HIV testing and treatment and how changing these specific norms translates into HIV service uptake. To explore these questions, we implemented a qualitative study during a community mobilization trial targeting social barriers to HIV service uptake in South Africa. METHODS: We conducted 55 in-depth interviews in 2018, during the final months of a three-year intervention in rural Mpumalanga

Potent and Quick Responses to Conspecific Faces and Snakes in the Anterior Cingulate Cortex of Monkeys.

Appropriate processing of others' facial emotions is a fundamental ability of primates in social situations. Several moods and anxiety disorders such as depression cause a negative bias in the perception of facial emotions. Depressive patients show abnormalities of activity and gray matter volume in the perigenual portion of the anterior cingulate cortex (ACC) and an increase of activation in the amygdala. However, it is not known whether neurons in the ACC have a function in the processing of facial emotions. Furthermore, detecting predators quickly and taking avoidance behavior are important

Visual Stimuli Evoked Action Potentials Trigger Rapidly Propagating Dendritic Calcium Transients in the Frog Optic Tectum Layer 6 Neurons.

The superior colliculus in mammals or the optic tectum in amphibians is a major visual information processing center responsible for generation of orientating responses such as saccades in monkeys or prey catching avoidance behavior in frogs. The conserved structure function of the superior colliculus the optic tectum across distant species such as frogs, birds monkeys permits to draw rather general conclusions after studying a single species. We chose the frog optic tectum because we are able to perform whole-cell voltage-clamp recordings fluorescence imaging of tectal neurons while they resp

Sensitive detection of chloroplast movements through changes in leaf cross-polarized reflectance.

We show that the positioning of chloroplasts in plant cells affects the ability of photosynthetic organs to depolarize linearly polarized light through diffuse reflection. We apply this phenomenon for the detection of blue-light-induced chloroplast movements using light reflected from leaves and green stems. High-light-induced chloroplast avoidance leads to an increase in reflectance, while chloroplast accumulation in low light has the opposite effect. The changes in reflectance are correlated with an increase in the leaf's ability to depolarize linearly polarized light. Inclusion of polarizat

Noninvasive Positive Pressure Ventilation in the Adult Acute Care Setting.

BACKGROUND: Noninvasive positive pressure ventilation (NIPPV) is a method of mechanical ventilation that provides respiratory support for patients with varying acute or chronic respiratory conditions and can be an effective intervention that may result in the avoidance of endotracheal intubation. NIPPV relies on the combination of a patient's ability to spontaneously breathe and the assistance of a ventilator to produce an inspiratory positive airway pressure, resulting in increased tidal volumes as well as improved ventilation and gas exchange. OBJECTIVES: This manuscript aims to provide a c

Attachment classification and early adversity predict perinatal partial hospital treatment response.

BACKGROUND: Depression and anxiety are common in the perinatal period. While most of those affected respond well to treatment, a subpopulation is more resistant. Understanding more about individuals who do not respond well to available treatments may improve care for this group. METHODS: We administered entry and exit self-report measures to 178 women who participated in a specialized partial hospitalization program for perinatal individuals. Baseline measures of anxiety, obsessive symptoms, sleep quality, early life adversity, and adult attachment security were examined as potential predicto

College Students' Trust, Betrayal, and Needs During and After Their Worst Nonassault-Related Healthcare Experiences Differ Based on Sexual Violence Exposure.

The BITTEN theoretical framework conceptually links patient's past healthcare betrayal and trauma experiences with their current and future healthcare interactions. BITTEN was used to examine whether healthcare experiences, behaviors, and needs differ between those with and without a history of sexual violence exposure. College students at two public universities in the southeastern United States (n = 1,381; 59.5% White, 61.0% women) completed measures about their self-selected worst or most frightening but nonassault-related healthcare experience. Tinnitus in COVID-19 Pandemic.

Tinnitus is a symptom that can significantly affect the quality of life of every patient. In patients with tinnitus, one of the most important diagnostic element is medical history data. Patients classify the pitch and loudness of tinnitus graphically using the visual analog scale (VAS). Visual analog scale and similar questionnaires constitute a valuable addendum of the clinical picture in patients with tinnitus. It seems essential, especially in the light of the information presented in the case report on the probability of correlation between tinnitus and stress and depression associated wi

The Perceptions and Rehabilitation Experience of Older People After Falling in the Hospital.

PURPOSE: Falls are a major cause of disability and mortality due to injury. To reduce fall rates and improve health outcomes, it is important to design services based on patient experience and engagement. This study aimed to explore the experiences of older patients who fell during their hospital stay. DESIGN: Five patients from two rehabilitation wards in the United Kingdom participated in this qualitative study. METHODS: Semistructured interviews, incident reports, and medical records provided information about each fall. Thematic, discourse, and descriptive analysis were used to analyze d

Subthalamic Neural Activity Patterns Anticipate Economic Risk Decisions in Gambling.

Economic decision-making is disrupted in individuals with gambling disorder, an addictive behavior observed in Parkinson's disease (PD) patients receiving dopaminergic therapy. The subthalamic nucleus (STN) is involved in the inhibition of impulsive behaviors; however, its role in impulse control disorders and addiction is still unclear. Here, we recorded STN local field potentials (LFPs) in PD patients with and without gambling disorder during an economic decision-making task. Reaction times analysis showed that for all patients, the decision whether to risk preceded task onset. We compared t

Narrative Length and Speech Rate in Battered Women.

Narrative length and speech rate of traumatic recollections have been previously associated with different emotions and adjustment trajectories after trauma. However, the evidence is limited and the results are mixed. The present study aimed to evaluate length (i.e., word count) and speech rate (i.e., words per minute) in narratives of events with different valence (i.e., neutral, positive, and negative/traumatic) by 50 battered women (trauma group) and 50 non-traumatized women (controls). The results showed that traumatic narratives by the trauma group were longer than those by the control gr

[Chronic peripheral arterial disease induced by cocaine].

Cocaine induced acute peripheral thrombosis, though a rare complication, has been described in the literature. Although there are reports describing the chronic effects of cocaine on the peripheral arterial system, there are no published cases of this complication when other risk factors are lacking. We report on a 22 year old female patient, with intranasal consumption of 3 grams of cocaine per week for a year, who consulted for intermittent claudication at 200 meters, associated to left lower limb pain and paresthesiae for the last two months. Arterial Doppler ultrasonography showed a stenos

Attention training for reducing spider fear in spider-fearful individuals.

Cognitive theorists propose that attentional biases for threatening information play an important role in the development and maintenance of anxiety disorders. If attentional biases for threat figure in the maintenance of anxiety, then the experimental reduction of the bias for threat (attention training) should reduce anxiety. We randomly assigned 41 spider-fearful individuals to receive either attention training (n=20) or control procedures (n=21). We used a modified dot-probe discrimination paradigm with photographs of spiders and cows to train attention. Training reduced attentional bias f

What can we learn about North Dakota's youngest drivers from their crashes?

For North Dakota teens, three of every four deaths are from motor vehicle crashes. Injury crash records for teen drivers were studied to gain insight regarding driver, vehicle, and road factors for public safety policy and program discussions. Results show 14-year-old drivers are three times more likely to die or be disabled in an injury crash than 17-year-old drivers, and that male drivers are 30% less likely to incur severe injury. As expected, seat belt use is a critical factor in severe injury avoidance. The likelihood for death or disablement is 165% greater for unbelted teen drivers than

Psychological sequelae of remote exposure to the September 11th terrorist attacks in Canadians with and without panic.

The purpose of the present investigation was to examine the psychological impact of remote exposure to the events and aftermath of the terrorist attacks in the USA on September 11th, 2001, and to assess what differences, if any, exist between individuals classified with probable panic disorder and those without. Telephone interviews were conducted with 122 residents of the capital city of the Canadian prairie province of Saskatchewan in spring 2002 in order to gather information regarding current mood, fears and avoidance behaviours as well as current post-traumatic stress disorder symptoms sp

Conservative interventions for carpal tunnel syndrome.

The assessment and conservative interventions in patients with carpal tunnel syndrome (CTS) are described in this paper. Information about surgical procedures and postoperative care has also been included. It is difficult to make definitive conclusions about the literature regarding success of treatment for CTS due to variations in outcome measures, severity of CTS, and inconsistencies in duration, dosage, and follow-up time for interventions. Based on what is known to date, this author recommends that patients with mild or moderate CTS be provided with a conservative program of splinting the

Effects of chlordiazepoxide (Librium) during pregnancy and lactation.

The decision to use psychotropic drugs during pregnancy and lactation must depend upon considerations of teratogenicity, effects on fetal and neonatal behavior and development, and a concern for the health and safety of the mother. Pregnancy itself can exacerbate anxiety symptoms, as well as alter the pharmacokinetics of antianxiety drugs; it thus presents a special problem to the clinician treating anxiety disorder in women. Since almost all psychotropic drugs cross the placenta, the use of medications during pregnancy and lactation requires critical attention to the timing of exposure, dosag

Donor selection: the exclusion of high risk donors?

Selection of donors is an important means to improve the overall safety of the blood supply. Since the AIDS epidemic emerged and after the introduction of sensitive screening tests for HIV, it became clear that blood donations given in the infectious 'window' period, formed the most important risk for recipients of blood products. Therefore, selection criteria became more and more stringent to exclude these high risk donors. Means to exclude high risk donors are non-remuneration, including a clear policy to provide no incentives which can be readily converted to cash, the avoidance of replacem

[Emergency laparoscopy in the examination of middle-aged and elderly patients with acute surgical diseases of the abdominal cavity].

Urgent laparoscopies were performed on 566 elderly and senile patients suspected of acute surgical diseases of abdominal organs. Laparoscopic examinations proved to be highly informative in 90.4%, little informative in 6.6%, erroneous in 3% of cases. Wide use of urgent laparoscopies in elderly and senile patients allowed to avoid laparotomies in 62.3% of the cases. High curative-diagnostic efficiency of laparoscopies added by laparoscopic cholecystostomy and antegrade cholecystocholangiography in patients with acute cholecystitis, cholecystopancreatitis, mechanical jaundice has been shown.
