The use of intrapleural (IP) analgesia for controlling post-operative pain in breast reconstruction is not well documented. The objective of this study was to assess the value of IP bupivacaine in those patients undergoing latissimus dorsi breast reconstruction. Twenty-six patients received a single bupivacaine IP injection at induction of anesthesia. Eighteen of these patients also received a 24-hour postoperative IP infusion. AIM: To study the effect of in-line filtering on in-line pressure measurement in a simulated infusion system.
METHODS: The experimental setup consisted of a syringe pump, syringe, intravenous (IV) line, and extension line. For aqueous fluids, a 0.2-μm infusion filter was used, and for lipid emulsions, a 1.2-μm filter. Infusion speed varied from 0.5 to 10.0 mL/h. First, the effect of clamping the extension line was studied on reaching alarm threshold. Second, the effect of aqueous and lipid fluids was evaluated on in-line pressure measurements.

Baclofen toxicity is a severe condition that can suppress brainstem reflexes, leading to coma and death. Therefore, emergency evaluation, correct diagnosis, and treatment are crucial to prevent any possible neurological sequelae. Here, we present a case of a patient with a previous baclofen pump, who was brought to the Emergency Department after being found unresponsive in bed. Brainstem reflexes were absent. The patient was taken to the emergency surgery room, and with the diagnosis of baclofen toxicity, the reservoir was removed from the pump, and cerebrospinal fluid drainage was initiated.
