The Prevalence and Associated Risk Factors for Neonatal Thrombocytopenia Among Newborns Admitted to the Neonatal Intensive Care Unit.

Background Thrombocytopenia is the most prevalent hematological condition in neonates that develops in the neonatal intensive care unit (NICU). This set of illnesses is caused by either decreased platelet production due to placental insufficiency, increased platelet breakdown (consumption), or a combination of the two causes. Based on platelet count, it is defined as mild, moderate, or severe thrombocytopenia, with early and late onset. Purpose The purpose of this study is to determine the prevalence of thrombocytopenia and the factors that contribute to it in newborns hospitalized in the neon

Neonatal Nurse Practitioner Use of Ultrasonography to Verify Umbilical Venous Catheter Placement in the Neonatal Intensive Care Unit.

BACKGROUND: Literature dating back to 1982 supports the use of ultrasound to confirm UVC position as a more reliable method compared to x-ray. The use of ultrasound to confirm UVC position remains infrequent in the NICU. PURPOSE: To determine the feasibility of training an NNP in the acquisition and interpretation of a parasternal short-axis (PSAX) image of the heart to confirm UVC position. METHODS: A retrospective review of 48 charts evaluated descriptive data using standard technique of an x-ray to confirm UVC placement. An NNP was trained to acquire and interpret a parasternal short-axis

Early neonatal drug utilization in preterm newborns in neonatal intensive care units. Italian Collaborative Group on Preterm Delivery.

The pattern of drug use in preterm newborns admitted to neonatal intensive care units (NICU) was monitored as part of a large multicenter study including a representative sample of Italian NICUs. All prescriptions from the admission of the mother through the 1st week of the neonate's life were carefully documented on standardized ad hoc forms, with particular attention to the timing and duration of each prescribed drug. The 706 babies included in the surveillance program received an average of 1.7 drugs during the early neonatal period, and were exposed to an average of 3.4 drugs over the whol

Feasibility and Impact of a Pilot Neonatal Cuddler Program on Preterm Infants in the Neonatal Intensive Care Unit.

OBJECTIVE: Preterm infants experience daily stress in the neonatal intensive care unit (NICU). Positive sensory experiences mitigate stress responses, but parental participation can be limited by external pressures. This study aimed to describe the impact of a neonatal cuddler program (NCP) on preterm infants' growth. STUDY DESIGN: A multidisciplinary program trained volunteers to interact with NICU infants when family was not at bedside. Infants were followed prospectively throughout admission. Intervention data included frequency, type, and duration of activity throughout the study period.

The First Gap in Life-Course CKD Care: Kidney Follow-up after Neonatal Intensive Care Unit Discharge.

BACKGROUND: CK care depends on pathways that connect risk recognition, evaluation, follow-up, and specialist input. Although CKD is often recognized later in life, CKD risk may originate at birth in infants born preterm or with low birth weight. These infants may encounter the first gap in the life-course CKD care pathway at neonatal intensive care unit (NICU) discharge, when early-life kidney risk should be linked to longitudinal follow-up. We aimed to describe post-discharge kidney follow-up practices for these infants in Japan and identify gaps among routine care, current evidence, and rece

Alarm Fatigue and Missed Nursing Care Among Neonatal and Paediatric Intensive Care Nurses: A Cross-Sectional Study.

BACKGROUND: Intensive care units (ICUs) are high-technology environments where patient safety is monitored through various devices. However, the high frequency of alarms can lead to alarm fatigue, especially in neonatal and paediatric ICUs due to rapid physiological fluctuations. This condition may decrease nurses' attention and lead to missed nursing care. AIMS: The primary aim of this study was to determine the levels of alarm fatigue and missed nursing care, along with the underlying reasons for missed care, among neonatal and paediatric intensive care nurses. The secondary aim was to exam

Reducing mother-infant separation through early skin-to-skin care in a neonatal intensive care unit setting.

INTRODUCTION: In neonatal care, mother-infant separation is common even when infants are stable or require only short-term support. Early skin-to-skin (ESTS) care may reduce separation and improve stability, bonding and breastfeeding. We aimed at implementing the practice as a protective quality improvement project. METHODS: The study was conducted in a level-2 neonatal intensive care unit (NICU) in Denmark, embedded in routine practice. ESTS was offered whenever feasible to preterm infants, GA ≥ 30 weeks or term infants with low birth weight or respiratory symptoms. ESTS was initiated immedi

Palliative Care in the Neonatal Intensive Care Unit: An Evolutionary Concept Analysis of Uncertainty in Anticipated Loss.

BACKGROUND: Although the concepts of uncertainty and anticipated loss have been explored in a variety of contexts, advances in genetic testing and life-sustaining technology rendered changes in the care of medically complex infants. The separate concepts no longer have the descriptive power to clarify new phenomena endured by parents in the changing neonatal landscape. A current concept analysis examining uncertainty in anticipated loss is necessary to generate knowledge concurrently with deviations observed in the neonatal intensive care unit. PURPOSE: To explore the concept of uncertainty i
