A rare neoplasm of the testis: capillary hemangioma.

Intratesticular vascular neoplasms are extremely rare tumors and mostly seen in children or young adults. We reported a case of capillary hemangioma of the testis to attract attention to testicular hemangioma and also to prevent invasive surgery of the testis. The patient was an 18-year-old boy with a testicular mass. Scrotal sonography revealed a varicocele in the left testis and a simple cyst in the left epididymis. There was a solid hypoechoic neoplastic area 75 mm in diameter in the right testis. The laboratory findings including tumor markers and serum levels of alpha-fetoprotein and beta

Successful Abdominal Wall Reconstruction Following Liposuction Catastrophe: A Case Report and Literature Review.

BACKGROUND: Devastating abdominal injuries may result from liposuction, which can lead to necrotizing fasciitis and loss of abdominal domain. This may necessitate reconstructing the abdominal wall to improve the patient's quality of life. OBJECTIVES: The study aims to raise awareness among the readers of this catastrophic complication. Guidelines for preoperative assessment, and intraoperative measures to reduce abdominal complications along with the importance of early recognition, diagnostic tools, and treatment strategies were provided. METHODS: We reported a patient who underwent abdomin

Diagnosis and management of indeterminate testicular lesions.

The sophistication and accessibility of modern-day imaging result in frequent detection of small or equivocal lesions of the testes. Traditionally, diagnosis of a testicular lesion with any possibility of malignancy would usually prompt radical orchidectomy. However, awareness is growing that a substantial proportion of these lesions might be benign and that universal application of radical orchidectomy risks frequent overtreatment. Given the potentially profound effects of radical orchidectomy on fertility, endocrine function and psychosexual well-being, particularly in scenarios of an abnorm

Generalized Prediction of Unconsciousness during Propofol Anesthesia using 3D Convolutional Neural Networks.

Neuroscience has generated a number of recent advances in the search for the neural correlates of consciousness, but these have yet to find … 100,000 people receive general anesthesia every day, from which up to 1% is affected by unintended intraoperative awareness [1]. Despite this, brain-based monitoring of consciousness is not common in the clinic, and has had mixed success [2]. Given this context, our aim is to develop and explore an automated deep learning model that accurately predicts and interprets the …

Complicated Meckel's diverticulum concealed by massive mucinous ovarian carcinoma - A case report of the cautionary tale of surgical tunnel vision.

INTRODUCTION: Cognitive bias, especially tunnel vision, in the clinical setting can predispose to catastrophic outcomes termed "never events". CASE PRESENTATION: A 58-year-old woman presented with abdominal pain on the background of chronic abdominal distension. Imaging indicated a massive ovarian lesion with presumed small bowel obstruction (SBO) secondary to mass effect. She underwent resection and staging workup. In addition to the 26.5 kg mucinous ovarian adenocarcinoma (MOC), she was intraoperatively found to have a Meckel's diverticulum (MD) with a band adhesion to proximal bowel (as th

Black thyroid in a dog on long-term doxycycline therapy.

OBJECTIVE: To increase awareness of black thyroid in dogs and to prevent unnecessary total thyroidectomy. A benign condition called "black thyroid" has been documented in greater than 250 people on chronic minocycline therapy, and rarely in animals. To our knowledge this is the first report of black thyroid in an animal secondary to doxycycline therapy. STUDY DESIGN: Case report. ANIMAL: One 10 year-old female spayed Collie-cross dog. METHODS: A dog on long-term doxycycline underwent a right parotid sialoadenectomy and left thyroidectomy to remove associated tumors. Black pigmentation of bo

Pulmonary embolism after shoulder arthroscopy.

Deep venous thrombosis and pulmonary embolism events are common complications following lower-extremity orthopedic surgery. Conversely, deep venous thrombosis and pulmonary embolism events are rare following upper-extremity surgery, specifically shoulder arthroscopy. The purpose of this article is to emphasize the necessity of performing a thorough preoperative workup to uncover possible risk factors for deep venous thrombosis/pulmonary embolism despite the rare occurrence of developing a pulmonary embolism following shoulder arthroscopy. This article describes 3 patients who developed a nonfa

Development and internal validation of a prediction model for hypoxic hepatitis after coronary artery bypass grafting with cardiopulmonary bypass.

BACKGROUND: Hypoxic hepatitis (HH) after on-pump coronary artery bypass grafting (CABG) is associated with poor outcomes, but dedicated prediction tools are lacking. We developed and internally validated a parsimonious model for severe postoperative aminotransferase elevation compatible with HH. METHODS: This retrospective single-center study included 626 consecutive adults who underwent CABG with cardiopulmonary bypass at Beijing Anzhen Hospital between June 2020 and June 2023. The outcome was peak postoperative alanine aminotransferase or aspartate aminotransferase ≥800 U/L after exclusion

Incidental diagnosis of an accessory ovary during cesarean section: a case report and literature review.

INTRODUCTION AND IMPORTANCE: An accessory ovary refers to the presence of more than two ovaries, with a third ovary located near a eutopic ovary and directly connected to the normal ovary. Due to its rarity, limited evidence exists regarding optimal management, particularly when it is diagnosed during pregnancy. CASE PRESENTATION: A 31-year-old multigravida woman underwent a cesarean section at term for obstetric indications. The pregnancy had been uneventful, and no adnexal abnormality had been identified during the antenatal period. During intraoperative exploration, two adjacent ovaries wi

Congenital Chylous Ascites in a Neonate with Isolated Aqueductal Stenosis: A Case Report and Literature Review.

Background and Clinical Significance: Ventriculoperitoneal (VP) shunting remains the standard definitive treatment for progressive neonatal obstructive hydrocephalus. Congenital chylous ascites is an uncommon neonatal condition, most often related to developmental lymphatic abnormalities. The concurrence of hydrocephalus requiring VP diversion with congenital chylous ascites is exceptionally rare and may first become apparent during abdominal access for shunt placement. Awareness of this possibility is clinically important because milky peritoneal fluid at shunt surgery can mimic gastrointesti

Adult Acute Appendicitis Complicated by Intestinal Malrotation: A Case Analysis.

BACKGROUND Intestinal malrotation (IM) is a rare congenital anomaly resulting from incomplete midgut rotation during embryonic development. Typically, it is diagnosed in infancy due to symptoms such as bilious vomiting, failure to thrive, or signs of bowel obstruction. However, in some cases, it remains undetected until adulthood, when it can present either incidentally or with complications, including volvulus, chronic abdominal pain, or acute bowel obstruction. Although its exact incidence in adults is unclear, studies estimate it to be approximately 0.2% to 0.5%, underscoring its rarity in

Current evidence on the diagnosis and management of spilled gallstones after laparoscopic cholecystectomy.

BACKGROUND: Despite improvements in intraoperative and postoperative outcomes of laparoscopic cholecystectomy (LC), spilled gallstones (SGs) after LC remain a significant yet often overlooked complication, occurring in 1% to 40% of cases. This review discusses the most recent updates regarding the risk factors, presentations, complications, diagnosis, management, and prognosis of SGs after LC. METHODS: A comprehensive systematic review was conducted using MEDLINE/PubMed, Google Scholar, Cochrane Library, and the Web of Science databases, with the range of search dates being between January 20

Impact of Intraoperative Hypothermia on Microsurgical Free Flap Reconstructions.

BACKGROUND: Patients requiring microsurgical defect reconstruction are highly susceptible to intraoperative hypothermia, given oftentimes long operative times and exposure of large skin surface areas. While the impact of hypothermia has been extensively studied across various surgical fields, its role in the setting of microsurgical free flap reconstruction remains elusive. This study evaluates the effects of hypothermia on outcomes of free flap reconstructions. METHODS: Within 7 years, 602 patients underwent 668 microvascular free flap reconstructions. The cases were divided into two groups

Perianesthetic Management of Patients With Thyroid-Stimulating Hormone-Secreting Pituitary Adenomas.

BACKGROUND: Thyroid-stimulating hormone (TSH)-secreting pituitary adenomas are a rare cause of secondary hyperthyroidism. Anesthetic management of these patients has not been formally described in the literature. MATERIALS AND METHODS: Patients who underwent resection of a TSH-secreting pituitary adenoma during 1987 to 2012 at a single institution were identified. Preoperative thyroid hormone state, anesthetic management, and outcome were recorded. Hemodynamic associations with intraoperative events were compared between those who were hyperthyroid and euthyroid at the time of surgery. RESUL

Echinococcosis: Unexpected Occurrence and the Diagnostic Contribution of Routine Histopathology.

Echinococcus is the smallest of the major tapeworms. Humans are incidental hosts who become infected by exposure to soil contaminated with ova from the feces of the canine definitive hosts. The major species are E. granulosus (cystic echinococcosis) and E. multilocularis (alveolar or sylvatic echinococcosis), both common worldwide but unusual and not reportable in the United States. Human disease is characterized by slowly growing visceral cystic masses often containing daughter cysts and hydatid sand. In nonendemic areas, the clinical evaluation includes imaging, serology, and surgery; metast

First branchial cleft anomalies: presentation, variability and safe surgical management.

First branchial cleft (FBC) anomalies are uncommon. The aim of this retrospective clinical study is to describe our experience in dealing with these sporadically reported lesions. Eighteen cases presenting with various FBC anomalies managed surgically during an 8-year period at a tertiary referral medical institution were included. Ten were males (56 %) and eight females (44 %) with age range 3-18 years. Anomaly was right-sided in 12 cases (67 %). None were bilateral. Nine patients (50 %) had prior abscess incision and drainage procedures ranging from 1 to 9 times. Two also had previous unsucc

Risk of patients with severe aortic stenosis undergoing noncardiac surgery.

Aortic stenosis (AS) is a major risk factor for perioperative cardiac events in patients undergoing noncardiac surgery. We previously showed that selected patients with AS who were not candidates for, or refused, aortic valve replacement could undergo noncardiac surgery with acceptable risk. We extended our previous experience over a subsequent 5-year period by retrospectively analyzing the perioperative course of all patients with severe AS (aortic valve area index < 0.5 cm2/m2 or mean gradient > 50 mm Hg), determined with Doppler echocardiography or cardiac catheterization, who underwent non

Evidence-Based Nursing Practices to Prevent Perioperative Unintentional Hypothermia in Elective Surgeries: An Observational Study.

Although evidence-based recommendations for preventing perioperative unintentional hypothermia have long been available, observational studies evaluating their implementation in hospital practice and the effectiveness of preventive interventions in at-risk patients are limited. This study aimed to determine the scope of nursing interventions for preventing perioperative unintentional hypothermia and examine the relationship between patient characteristics and these practices. This study employed a descriptive observational design. Patients undergoing elective surgery in the surgical units (ort

Dynamic aortic regurgitation caused by suction-induced right coronary cusp entrapment in a ruptured right sinus of Valsalva aneurysm: a case report.

BACKGROUND: Aortic regurgitation (AR) is usually caused by intrinsic cusp pathology or aortic root dilatation. Beat-to-beat variability in AR severity, sometimes accentuated by rhythm disturbances, is uncommon and may be overlooked without rhythm-aware, multimodality imaging across multiple cardiac cycles. CASE SUMMARY: A 30-year-old woman with prior ventricular septal defect repair presented with recurrent palpitations and frequent ventricular premature contractions (PVCs).
