The impact of intraoperative hypothermia on early postoperative adverse events after radical esophagectomy for cancer: a retrospective cohort study.

OBJECTIVE: To investigate the correlation between intraoperative body temperature and postoperative adverse effects in patients who underwent esophagectomy procedures. DESIGN: Retrospective cohort study. SETTING: University Hospital. PARTICIPANTS: One hundred twenty-one patients undergoing esophagectomy were enrolled. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Various perioperative and intraoperative variables were recorded. Hypothermia was defined as a urinary bladder temperature<35°C. A Process-Based Standardization of an Intraoperative Escalation Protocol in Anesthesiology.

Prompt recognition and management of critical events is pivotal for the provision of safe anesthetic care. This requires a well-functioning team that focuses on effective communication, timely decision-making, and escalation of potential complications. We believe that variation in bedside care leads to "near-misses," adverse outcomes, and serious safety events (SSEs). The principles of an escalation culture have been used successfully in other highly reliable industries such as aviation, military, and manufacturing. We discuss here the introduction of a unique and compelling thought-process fo

Intraoperative Neuromonitoring in Thyroid Surgery: A Systematic Review.

This study aimed to assess the efficacy of intraoperative neurophysiologic monitoring (IONM) in preventing recurrent laryngeal nerve palsy (RLNP) during thyroid surgery. When IONM results in false positives, it seeks to evaluate contributing factors. A systematic review was conducted gauging the predictive power of neuromonitoring in determining RLN function intraoperatively, its reductions of temporary and permanent RLNP rates, and surgeons' response to the technology. MEDLINE, EMBASE, and PubMed were searched for RLN monitoring in thyroid surgery following a set of inclusion/exclusion criter

[Is deep anesthesia dangerous?].

Regarding the question of an adequate depth of anesthesia, over the past decade anesthesiologists have focused on the prevention of intraoperative consciousness in combination with explicit memory. Recent studies approached the topic from a different way postulating that deep anesthesia, quantified as time with a bispectral index (BIS)< 45, is associated with increased postoperative mortality and four out of the five published studies revealed such a correlation. However, the finding is limited by a suboptimal study design, e.g. none of the studies presented randomized data. Furthermore, it is

Evaluation and treatment of dural tears in lumbar spine surgery: a review.

Incidental durotomy is a frequent complication of lumbar spinal surgery. The number and complexity of spinal procedures is increasing, leading to a greater prevalence of dural tears; therefore, it is imperative that spine surgeons be familiar with safe and effective closure techniques. Occasionally, a tear may not be recognized during the procedure, so that one must recognize the signs and symptoms of a cerebrospinal fluid leak postoperatively. Several newer treatment concepts show promise. The current study represents an extensive review of the recent literature on the prevalence, mechanism,

The spring balance: a simple monitoring system for fluid overload during hysteroscopic surgery.

Fluid overload may arise during hysteroscopic surgery, caused by absorption of fluid used to distend the uterus. Continuous monitoring of the fluid balance is required to prevent this serious complication. Commercial equipment does not serve this purpose adequately. We describe a simple solution. We weigh the inflow and outflow volumes together on a spring balance; this method provides information about fluid balance, since any fluid lost as spillage or by absorption is reflected by a loss of weight on the balance. The method is simple, accurate, and inexpensive, and the components can be purc

A Calculated Risk: Performing Laparoscopic Cholecystectomy for Acute Cholecystitis on Patients with End Stage Renal Disease.

End-stage renal disease (ESRD) is a multifactorial disease linked to socioeconomic status and associated with worse surgical outcomes. We explore intraoperative and postoperative outcomes in patients with cholecystitis undergoing laparoscopic cholecystectomy (LC). The Nationwide Inpatient Sample from 2005 to 2012 was used to identify patients undergoing LC for cholecystitis using ICD-9 codes. Outcomes of interest were mortality, common bile duct injury, conversion to open, intraoperative complications, postoperative complications, length of stay (LOS), and total charge. Univariate analysis was…

[Measures to prevent ureteric injury in rectal cancer surgery].

The majority of ureteric injury is iatrogenic during surgical procedures especially pelvic and retroperitoneal operations. Approximately 10% of ureteric injury is associated with colorectal procedures. The major cause is anatomical anomaly. The types of injuries mainly include contusion, clamp injury, ligation injury, cautery, cut injury and distorted traction to an acute angle. The injuries are mainly located in the lower segment of the ureter. An accurate evaluation of the risk of ureteric injury before rectal cancer operation, a better understanding of anatomy in both normal and abnormal co

Usefulness of intraoperative endoscopy in pediatric surgery: state of the art.

INTRODUCTION: Intraoperative endoscopy is a procedure that supports open and laparoscopic surgery, helping the surgeon to identify the presence of endoluminal gastrointestinal lesions which need to be treated, with a correct diagnosis and an adequate therapy. MATERIAL AND METHODS: A search on PubMed was performed using "intraoperative esophagoscopy", "intraoperative duodenoscopy", and "intraoperative enteroscopy" as Mesh terms. The applied exclusion criteria were: papers written before 2000, not concerning pediatric or gastrointestinal pathology, literature-review articles, language different

[Does intraoperative nerve monitoring reduce the rate of recurrent nerve palsies during thyroid surgery?].

Two different aspects of the influence of neuromonitoring on the possible reduction of post-operative recurrent laryngeal nerve palsies require critical examination: the nerve identification and the monitoring of it's functions. Due to the additional information from the EMG signals, neuromonitoring is the best method for identifying the nerves as compared to visual identification alone. There are still no randomized studies available that compare the visual and electrophysiological recurrent laryngeal nerve detection in thyroid operations with respect to the postoperative nerve palsies. Evolving concepts in prevention of spinal cord injury during operations on the descending thoracic and thoracoabdominal aorta.

Spinal cord injury following operations on the descending thoracic or thoracoabdominal aorta remains a major problem. In certain subsets of patients, the risk of postoperative spinal cord injury is substantial. Although several adjuncts have been employed clinically to eliminate or reduce the frequency of this complication, none have proven to be completely effective. An important reason for this is the failure of these techniques to reliably and noninvasively localize the level of origin of arteries from the aorta that are critical to spinal cord circulation. Since postoperative spinal cord i

[Continuous intraoperative neuromonitoring (cIONM) in head and neck surgery-a review. German version].

Although the history of intraoperative neuromonitoring (IONM) dates back to the 19th century, the method did not evolve further than the mere differentiation of nerves until recently. Only the development of continuous IONM (cIONM) has allowed for non-stop analysis of excitation amplitude and latency during surgical procedures, which is nowadays integrated into the software of almost all commercially available neuromonitoring devices. The objective of cIONM is real-time monitoring of nerve status in order to recognize and prevent impending nerve injury and predict postoperative nerve function.
