Carpal tunnel syndrome among grocery store workers.

The California Department of Health Services evaluated carpal tunnel syndrome (CTS), a median nerve entrapment condition associated with forceful and repetitive wrist motion, among grocery store workers at a large California supermarket where a CTS cluster had been reported. Forceful and repetitive wrist motion was measured, in three exposure levels, through a job classification scheme based upon type of work tasks and average time per week spent performing these tasks. A medical questionnaire and measurements of median sensory nerve conduction were used to measure CTS. CTS prevalence was 23%

Current epidemiologic evidence and case reports of occupationally acquired HIV and other bloodborne diseases.

Studies in which noninfected persons shared households with human immunodeficiency virus (HIV)-infected family members and friends, and had substantial though non-sexual contact with them revealed no transmissions of the HIV virus. These results suggest a similar low risk for healthcare workers, particularly those who have little or no contact with blood. However, occupational transmission of HIV, when it does occur, is most commonly caused by needlestick injuries. Nurses make up the largest group--nearly 80%--of hospital workers who contract HIV infection on the job. Generally, the risk ratio

A longitudinal study of predictors of research-defined carpal tunnel syndrome in industrial workers: findings at 17 years.

In 1984, we initiated a prospective study of factors associated with research-defined carpal tunnel syndrome (CTS) in 471 industrial workers. Medical history, lifestyle factors and job tasks were assessed by questionnaire and CTS case status was based on both symptoms and electrophysiologic findings. Participants were re-examined in 1989, 1994 to 1995 and 2001 to 2002. This study reports both baseline and aggregated risk factors associated with increased risk of CTS by 2001 to 2002 for 166 participants successfully re-examined after 17 years. In analyses of baseline risk factors, fewer repetit

Insights into modifiable risk factors of atrial fibrillation: a comprehensive Mendelian randomization study.

BACKGROUND: Numerous observational studies suggest that modifiable risk factors contribute to the onset of atrial fibrillation (AF). This study aims to assess the causal relationship between 46 modifiable risk factors and AF. METHODS: Univariable, multivariate, and mediation Mendelian randomization (MR) analyses were employed to examine the causal relationship between 46 modifiable risk factors and AF. Summary-level data from genome-wide association studies (GWAS) meta-analysis and FinnGen consortium were utilized for both discovery and replication. The combined results were analyzed using a

Pulmonary complications of primary immunodeficiencies.

In the fifty years since Ogden Bruton discovered agammaglobulinemia, more than 100 additional immunodeficiency syndromes have been described. These disorders may involve one or more components of the immune system, including T, B, and NK lymphocytes; phagocytic cells; and complement proteins. Most are recessive traits, some of which are caused by mutations in genes on the X chromosome, others in genes on autosomal chromosomes. Until the past decade, there was little insight into the fundamental problems underlying a majority of these conditions. Many of the primary immunodeficiency diseases ha

Influence of Motion Therapy in the Prevention of Lumbar Pain Syndrome Relapse.

INTRODUCTION: Lumbar pain syndrome is one of the most common conditions in clinical practice, more common than 290 other pathological conditions, which affect up to 84% of adults in a certain period of their life. The origin of the lower back pain can be classified as mechanical, neuropathic and secondary due to another illness. Patient education and information, muscle strengthening exercises, maintenance of routine daily physical activity and pain therapy are the basis of acute non-specific pain syndrome therapy. AIM: To determine the success of the motion therapy procedure in the preventio

Possibilities and limitations of rehabilitation after traumatic apallic syndrome in children and adolescents.

Report on long-term follow-up studies in 23 children and 2 adolescents who survived severe brain injury and a resulting apallic syndrome. Neurological examinations and psychiatric observations were performed in all patients, and psychological tests were administered whenever possible. EEGs were recorded in all cases, and 11 patients had CT scans. 6 patients died after surviving the acute phase. 2 survived in state of stabilized unconsiousness. Recovery from the apallic syndrome was observed in 17 patients, but complete recovery did not occur in a single case. 7 patients (group A) retained seve

Pulmonary Infections and Surgical Complications in a Young Girl with Signal Transducer and Activator of Transcription 3 Loss-of-Function Mutation Hyperimmunoglobulin E Syndrome: A Case Report.

Introduction: Hyperimmunoglobulin E syndromes (HIESs) are characterized by a high serum immunoglobulin E (IgE) level, eczematoid rashes, recurrent staphylococcal skin abscesses, and recurrent pneumonia and pneumatocele formation. Autosomal dominant HIES is the most common form of HIES and mainly occurs due to loss-of-function mutations in the Signal Transducer and Activator of Transcription 3 (STAT3) gene (STAT3 LOF). Case Presentation: We report the case of an 11-year-old Peruvian girl diagnosed with STAT3 LOF caused by p.R382W mutation. She presented with recurrent staphylococcal pneumonia a

Retrospective cohort mortality study of workers engaged in motion picture film processing.

OBJECTIVE: To assess potential health risks associated with work in a large motion picture film-processing facility. METHODS: A retrospective cohort mortality study was conducted during 1960-2000 among 2646 film workers. Job family categories, created from detailed employee work history information, were used to evaluate chemical exposure patterns. RESULTS: Overall mortality was as expected (standardized mortality ratio [SMR] = 1.1; 95% confidence interval [CI] = 1.0-1.2). Statistically significant associations were found for suicides (SMR = 2.0; 95% CI = 1.2-3.0) among the hourly workers an

Defective interleukin-12/interferon-gamma pathway in patients with hyperimmunoglobulinemia E syndrome.

OBJECTIVE: Patients with the hyperimmunoglobulinemia E (hyper-IgE) syndrome are reported to have defective production of interferon gamma (IFN-gamma). Because IFN-gamma is a major activator of polymorphonuclear leukocytes (PMNs), this could result in defective PMN chemotaxis and markedly elevated IgE levels because of the unopposed action of interleukin (IL)-4. IL-12, an important enhancer of IFN-gamma production, also suppresses IgE production. This study assessed the IL-12/IFN-gamma pathway in patients with hyper-IgE syndrome. METHODS: Production of IL-12 and IFN-gamma by mononuclear cells

CD8+ T lymphocytes producing Th2-type cytokines (Tc2) in HIV-infected individuals.

Unusual high proportions of CD8+ T-cell clones able to produce IL-4, but no IFN-gamma (Tc2 cells), were generated from both skin biopsies and peripheral blood of HIV-infected individuals in advanced phase of infection. Tc2 cells showed unusual phenotypic and functional features, such as CD30 expression, B-cell helper activity for IgE synthesis and reduced cytolytic potential. Cloning CD8+ T-cell from HIV-infected individuals in autologous feeder cells resulted in a marked increase of CD8+ T-cell clones showing a Tc2 profile. The addition in bulk culture before cloning of both IL-12 and a neutr

Primary immunodeficiency diseases associated with increased susceptibility to viral infections and malignancies.

Primary immunodeficiencies (PIDs) are commonly characterized by an increased susceptibility to specific infections and, in certain instances, a higher than usual incidence of malignancies. Although improved diagnosis and early treatment of PIDs have reduced early morbidity and mortality from infection, the development of cancer remains a significant cause of premature death. The emergence of cancer in patients with PIDs often results from impairments in the immune response that lead to weakened surveillance against oncogenic viruses, premalignant or malignant cells, or both. Here we review the

Neutrophil hyperresponsiveness contributes to lung pathology in STAT3V463Δ mice.

Autosomal dominant hyper-IgE syndrome (AD-HIES), or Job's syndrome, is a rare primary immunodeficiency caused by dominant-negative mutations in STAT3. Patients experience recurrent pulmonary infections and chronic inflammation, leading to severe complications and heightened mortality risk. To investigate whether neutrophil-intrinsic dysfunction contributes to lung pathology in AD-HIES, we used a murine model expressing the STAT3V463Δ mutation, a common disease-associated variant. Following intratracheal infection with Pseudomonas aeruginosa, STAT3V463Δ mice exhibited pronounced alveolar damage

Impaired angiogenesis and extracellular matrix metabolism in autosomal-dominant hyper-IgE syndrome.

There are more than 7000 described rare diseases, most lacking specific treatment. Autosomal-dominant hyper-IgE syndrome (AD-HIES, also known as Job's syndrome) is caused by mutations in STAT3. These patients present with immunodeficiency accompanied by severe nonimmunological features, including skeletal, connective tissue, and vascular abnormalities, poor postinfection lung healing, and subsequent pulmonary failure. No specific therapies are available for these abnormalities. Here, we investigated underlying mechanisms in order to identify therapeutic targets. Histological analysis of skin w

Body mass index and anthropometric characteristics of the hand as risk factors for carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is the most common peripheral entrapment compressive neuropathy in the upper limbs. It is often correlated with personal factors of individuals, not only with certain medical conditions and jobs. The study aimed at clarifying the association of carpal tunnel syndrome with anthropometric characteristics of the hand and body mass index (BMI) as independent risk factors. A total of 100 subjects participated: 50 patients with electro-diagnostically confirmed CTS and 50 healthy volunteers without CTS symptoms as control group, each group including 37 women and 13 men.
